Cognitive training and brain stimulation in prodromal Alzheimer's disease (AD-Stim)-study protocol for a double-blind randomized controlled phase IIb (monocenter) trial.

BACKGROUND: Given the growing older population worldwide, and the associated increase in age-related diseases, such as Alzheimer's disease (AD), investigating non-invasive methods to ameliorate or even prevent cognitive decline in prodromal AD is highly relevant. Previous studies suggest transcranial direct current stimulation (tDCS) to be an effective method to boost cognitive performance, especially when applied in combination with cognitive training in healthy older adults. So far, no studies combining tDCS concurrent with an intense multi-session cognitive training in prodromal AD populati…

Cognitive Therapy and Task Concentration Training Applied as Intensified Group Therapies for Social Anxiety Disorder with Fear of Blushing-A Randomized Controlled Trial.

The current study examines the efficacy of intensified group therapy for social anxiety disorder with fear of blushing. Task concentration training (TCT) and cognitive therapy (CT) were applied during one weekend and compared with a waiting list condition in a randomized controlled trial including 82 patients. On a second weekend, another intervention was added (resulting in TCT-CT and CT-TCT sequences) to examine order effects. Task concentration training and CT were both superior to the waiting list and equally effective after the first therapy weekend. Also, no differences were found betwee

Exercise training plus calorie restriction causes synergistic protection against cognitive decline via up-regulation of BDNF in hippocampus of stroke-prone hypertensive rats.

One of the important organ damage of hypertension is cognitive decline. Cognitive function is determined by the function of hippocampus, and previous studies have suggested that the decrease in brain-derived neurotrophic factor (BDNF) in the hippocampus causes cognitive decline. Protection against cognitive decline is reported not only in pharmacological therapy but also in exercise training or calorie restriction. The aim of the present study was to determine whether exercise training plus calorie restriction cause synergistic protection against cognitive decline via BDNF in the hippocampus o

Home-based physical exercice with additional cognitive training for improving mobility in older adults: a secondary analysis of the COVEPIC randomized controlled trial.

BACKGROUND: Home-based physical exercise is an accessible strategy to help maintain physical functioning in adults over 50, but mobility gains may be limited without direct supervision. By improving cognitive processes involved in motor control, cognitive training may help enhance the effectiveness of home-based physical exercise in preventing age-related decline in mobility. This study compares the effects of a six-month home-based physical exercise, with or without cognitive training, on gait speed and balance. METHODS: 127 community-dwelling adults aged 50-87 years (mean 65.20 ± 7.93; 76% …

Effects of Dual-Task Training on Gait Ability in Older Adults with Mild Cognitive Impairment: A Randomized Controlled Trial Focused on Obstacle Negotiation.

Older adults with mild cognitive impairment (MCI) often show gait impairment during dual-task walking and obstacle negotiation. This assessor-blinded randomized controlled trial investigated whether dual-task gait training with visual adaptation, added to a general exercise program, improves gait and related functional outcomes in older adults with MCI. Forty participants aged 65 years or older who met the MCI criteria were randomly allocated to a dual-task gait training with visual adaptation group or a control group (n = 20 each). Spatiotemporal and adaptive gait parameters were assessed bef

Multiagent Large Language Model Framework for Psychotherapy Fidelity Assessment in Motivational Interviewing and Cognitive Behavioral Therapy Training: Cross-Sectional, Simulation-Based Evaluation Study.

BACKGROUND: Psychotherapy training is difficult to scale because manual rating of motivational interviewing (MI) and cognitive behavioral therapy (CBT) sessions is time-intensive, requires trained raters, and is subject to rater variability. Large language models (LLMs) may support simulation-based training and rubric-guided scoring, but early-stage evidence is needed before such systems can be applied to real learners. OBJECTIVE: This study aimed to conduct a simulation-based evaluation of a multiagent LLM framework for generating and scoring stylized MI and CBT training encounters. METHODS

Assessing modified olfactory training to prevent cognitive decline in MCI high-risk individuals: a multicentre randomised controlled trial protocol in Beijing tertiary hospitals.

INTRODUCTION: Mild cognitive impairment (MCI) presents a significant risk, with a 75-80% likelihood of progressing to dementia. Despite this high risk, there is currently no straightforward and effective treatment strategy to halt or reverse this progression. Olfactory dysfunction, in conjunction with subjective cognitive decline, offers a crucial opportunity for early intervention in older adults at risk of MCI. Conventional olfactory training (COT) has demonstrated potential in enhancing neuroplasticity, which is vital for cognitive health. Initial studies indicate that modified olfactory tr

Effect of BCI-Controlled Pedaling Training System With Multiple Modalities of Feedback on Motor and Cognitive Function Rehabilitation of Early Subacute Stroke Patients.

Brain-computer interfaces (BCIs) are currently integrated into traditional rehabilitation interventions after stroke. Although BCIs bring many benefits to the rehabilitation process, their effects are limited since many patients cannot concentrate during training. Despite this outcome post-stroke motor-attention dual-task training using BCIs has remained mostly unexplored. This study was a randomized placebo-controlled blinded-endpoint clinical trial to investigate the effects of a BCI-controlled pedaling training system (BCI-PT) on the motor and cognitive function of stroke patients during re

I thought it would be difficult, but this is actually something I can do - experiences with Virtual Reality-based cognitive training in persons with TBI.

INTRODUCTION: Virtual reality (VR) has been suggested as a promising technology for delivering cognitive training to persons with traumatic brain injury (TBI), as it can provide situations resembling everyday activities. Studies have demonstrated that persons with TBI manage utilizing VR in clinical settings; however, no studies have investigated VR use in home settings. The aim of this study was to explore how persons with TBI experience utilizing VR for rehabilitation at home and how they experience VR as cognitive training. METHODS: Individual qualitative interviews were conducted with ten…

Fecal Microbiota Transplantation from Young-Trained Donors Improves Cognitive Function in Old Mice Through Modulation of the Gut-Brain Axis.

The gut-brain axis is a bidirectional communication pathway that modulates cognitive function. A dysfunctional gut-brain axis has been associated with cognitive impairments during aging. Therefore, we propose evaluating whether modulation of the gut microbiota through fecal microbiota transplantation (FMT) from young-trained donors (YT) to middle-aged or aged mice could enhance brain function and cognition in old age. Twelve-month-old male mice received an initial FMT from YT (YT-Tr) or age-matched donors (Auto-Tr) following antibiotic treatment. Three months later, the mice received a second
