Clinical Burden of Anastomotic Leak After Elective Colorectal Cancer Surgery: A Multicenter Cohort Study.

Background: Anastomotic leak remains a major complication after colorectal cancer surgery. While most studies focus on prediction or prevention, less is known about the clinical course after leak diagnosis. We aimed to evaluate the occurrence, severity, management, and early postoperative consequences of anastomotic leak after elective colorectal cancer surgery. Methods: This retrospective multicenter cohort study included consecutive patients undergoing elective colorectal cancer resection with restoration of bowel continuity in five tertiary referral centers. Patients without an evaluable an

Deciphering Post-surgery Gut Microbial Dynamics in Colorectal Cancer through Multi-cohort Machine Learning.

Surgical resection, and its associated bowel preparation, remain the primary treatment for colorectal cancer (CRC), yet the associated effects on postoperative gut microbiota composition remain poorly characterized. This study profiled gut microbial compositional changes before and after colorectal surgery and identified microbial signatures associated with postoperative recovery. Four external study cohorts were integrated to strengthen biological interpretation of our internal cohort observations. Leveraging Machine learning (ML) combined with SHapley Additive exPlanations (SHAP), we identif…

Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for colorectal peritoneal metastases: a single-institution experience.

BACKGROUND: Peritoneal metastases (PM) from colorectal cancer are associated with poor prognosis. Cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) has been proposed for selected patients, although its role remains debated. This study evaluates the efficacy and safety of CRS plus HIPEC in a single institution. PATIENTS AND METHODS: This retrospective study included patients with colorectal PM treated with CRS and HIPEC between 2009 and 2024. Preoperative staging involved CT and/or MRI, and all cases were discussed in a multidisciplinary team. Disease

Effect of Preoperative Inflammatory Diet on Clinical and Oncologic Outcomes Following Colorectal Cancer Surgery.

Objectives: The dietary inflammatory index (DII), a validated tool for assessing the inflammatory potential of diet, has been widely identified as a significant risk factor for colorectal cancer (CRC). However, its role as a prognostic factor for CRC remains unexplored. This study examined the impact of preoperative dietary inflammation on clinical and oncologic outcomes following CRC surgery. Methods: The study population consisted of 126 patients who had surgical procedures for CRC and completed a food frequency questionnaire (FFQ) preoperatively between January 2018 and June 2020. Results:

ICG-FA for Anastomotic Leak Prevention During Minimally Invasive Colorectal Surgery: A Meta-analysis.

INTRODUCTION: Anastomotic leakage is a serious complication after colorectal surgery. Indocyanine green fluorescence angiography (ICG-FA) allows real-time perfusion assessment and may reduce leak rates; however, recent larger randomized clinical trials reported conflicting results. This meta-analysis aimed to clarify the effectiveness of ICG-FA in reducing anastomotic leakage and related complications following minimally invasive colorectal surgery. METHODS: We conducted a comprehensive search across the databases to identify relevant studies. The search results were imported into Covidence f…

Short-term outcomes of open versus laparoscopic surgery in patients with metachronous colorectal cancer.

BACKGROUND: Few studies have assessed the safety and efficacy of laparoscopic surgery in patients with metachronous colorectal cancer (MCRC). This study aims to evaluate the safety and outcomes of laparoscopic surgery in MCRC patients who have previously undergone colorectal cancer surgery. METHODS: We compared the short-term outcomes of open versus laparoscopic surgery in patients with MCRC between October 2007 and October 2022. RESULTS: Among the 59 MCRC patients, 25 underwent laparoscopic surgery, whereas 34 received open surgery. The proportion of patients who underwent prior open surger

Exposure to androgen deprivation therapy and risk of anastomotic leakage after colorectal cancer surgery.

AIM: The risk of anastomotic leakage after colorectal cancer surgery is higher in men, regardless of the anatomical location. Previous studies suggest that this might be due to hormonal differences. The aim of this work was to investigate whether androgen deprivation therapy influenced the incidence of anastomotic leakage. METHOD: This is a nationwide registry-based study of men operated on between 2007 and 2021 for colorectal cancer with an anastomosis. Exposure to androgen deprivation therapy (prescribed drugs or surgical castration) was related to anastomotic leakage using mixed-effects lo

Prognostic Value of the Modified Cachexia Index in Colorectal Cancer Patients Undergoing Curative Surgery.

BACKGROUND/AIM: The cachexia index (CXI) has been reported to be a useful indicator for predicting the prognosis of cancer patients. However, CXI calculation requires skeletal muscle index (SMI) measurements, which involves an analysis of computed tomography images using an imaging software program, which makes the calculation process highly complex and time-consuming. Recently, the modified cachexia index (mCXI), calculated using the urea-to-creatinine ratio (UCR) instead of SMI, has been reported to be a useful marker that is easier to calculate than CXI. This study aimed to evaluate the cor

Spinal anaesthetic with intrathecal diamorphine in robotic-assisted colorectal surgery: a retrospective cohort study.

Spinal anaesthesia (SA) with intrathecal diamorphine can be utilised as part of multimodal analgesia to reduce opioid use and enhance recovery in patients undergoing major abdominal operations. There is limited evidence that this also applies to patients undergoing robotic-assisted colorectal surgery (RACS). The aim of this study was to investigate perioperative opioid administration and post-operative complications in RACS patients receiving SA versus those who received conventional analgesia (CA). Data from 201 consecutive patients undergoing RACS at York and Scarborough Teaching Hospitals N…

Ureteral Stents Do Not Increase the Risk for Acute Kidney Injury After Colorectal Surgery.

BACKGROUND: Ureteral stents are thought to prevent or help identify ureteral injuries. Studies suggesting that ureteral stents increase the risk of postoperative acute kidney injury show inconsistent conclusions. The large ureteral stenting volume at our institution provided a unique opportunity for granular analysis not previously reported. OBJECTIVE: To determine whether prophylactic ureteral stenting at colorectal surgery increases acute kidney injury. DESIGN: Retrospective analysis of colorectal operations with prophylactic ureteral stents was compared to operations without stents. Adjus…

Risk stratification of acute kidney injury (AKI) following ureteral stent insertion for colorectal surgery.

INTRODUCTION AND OBJECTIVES: Ureteral stents have long been utilized during colorectal resections to assist in the identification of ureters intraoperatively and mitigate risk of ureteral injury. As these procedures have shifted toward robot-assisted laparoscopic methods, lighted stents have increasingly been used. The incidence of acute kidney injury (AKI) following bilateral ureteral stent placement has been reported to be as high as 41.9%. We sought to identify our single-institution risk and determine the extent to which age, sex, and stent type affected incidence of AKI. METHODS: A retro

Transrenal Ureter Embolization for Refractory Urine Leaks from Iatrogenic Ureteric Injury Following Colorectal Surgery.

We present the cases of two patients who underwent ureteral occlusion using coils and/or Amplatzer Vascular Plug with N-butyl cyanoacrylate glue after extensive advanced rectal surgery. Both patients had complex urine leaks unresponsive to urinary diversion. In view of the progress of the disease and the history of polysurgery, reconstructive surgery or anterograde ureteral stent insertion was not chosen. All patients had immediate resolution of urinary leakage after ureteral embolization, resulting in symptom relief throughout the follow-up period. There were no procedure-related complication
