Building community partnerships: case studies of Community Advisory Boards at research sites in Peru, Zimbabwe, and Thailand.

BACKGROUND: Differences in resources, knowledge, and infrastructure between countries initiating and countries hosting HIV prevention research trials frequently yield ethical dilemmas. Community Advisory Boards (CABs) have emerged as one strategy for establishing partnerships between researchers and host communities to promote community consultation in socially sensitive research. PURPOSE: To understand the evolution of CABs and community partnerships at international research sites conducting HIV prevention trials. METHODS: Three research sites of the HIV Prevention Trials Network (HPTN) we

Increasing communicative interactions of young children with autism using a voice output communication aid and naturalistic teaching.

We evaluated the effects of a voice output communication aid (VOCA) and naturalistic teaching procedures on the communicative interactions of young children with autism. A teacher and three assistants were taught to use naturalistic teaching strategies to provide opportunities for VOCA use in the context of regularly occurring classroom routines. Naturalistic teaching procedures and VOCA use were introduced in multiple probe fashion across 4 children and two classroom routines (snack and play). As the procedures were implemented, all children showed increases in communicative interactions usin

Community-based questionnaires and health statistics as tools for the cost-efficient identification of communities at risk of urinary schistosomiasis.

Self-administered questionnaires, distributed by existing administrative channels to village party chairmen, head-teachers and schoolchildren, showed good diagnostic performance for the qualitative assessment of urinary schistosomiasis endemicity. At a cost 34 times below that of the WHO-recommended parasitological screening strategy, the schoolchildren's questionnaire allowed the screening of 75 out of 77 schools of a rural Tanzanian district in six weeks, and the exclusion of schools not at high risk for urinary schistosomiasis with over 90% confidence. The headteacher and party questionnair

Community Experience of COVID-19 Hospitalizations.

Background Every American was affected by the COVID-19 pandemic. The novelty and virulence spurred the rapid evolution of treatment strategies. While large multicenter studies established key mortality risk factors and treatment efficacy, community hospital experiences are underrepresented. Methods This retrospective study examines clinical characteristics, mortality predictors, and outcome trends among 1,949 adults hospitalized with COVID-19 at a Southern California community hospital from January 31, 2020, through May 11, 2023. Results Overall mortality was 7.6%, and the mean length of stay

Precipitation timing matters for flowering communities.

Flowering times have long been a harbinger of the changing seasons and, more recently, of a changing climate. Among the cues that trigger flowering, precipitation timing has been relatively little-studied. Tsafon et al. show that the timing of precipitation shapes flowering in complex ways. Much work remains to disentangle this complexity.

Risk Communication in Healthcare: The Management of Misunderstandings.

Risk communication represents a nuanced discourse within the healthcare sector, characterized by the topics' sensitivity and the potential for misunderstandings between healthcare providers and patients. This delicacy stems from the complexity of effectively conveying information about risks. Consequently, a primary obstacle lies in fostering healthcare providers' understanding of implicit communication nuances inherent in pre-operative risk discussions. This study aims to address this gap in the literature by examining the topic through the lens of the philosophy of language, specifically uti

Distinguishable spreading dynamics in microbial communities.

A packed community of exponentially proliferating microbes will spread in size exponentially. However, due to nutrient depletion, mechanical constraints, or other limitations, exponential proliferation is not indefinite, and the spreading slows. Here, we theoretically explore a fundamental question: is it possible to infer the dominant limitation type from the spreading dynamics? Using a continuum active fluid model, we consider three limitations to cell proliferation: intrinsic growth arrest (e.g., due to sporulation), pressure from other cells, and nutrient access. We find that memoryless gr

Midwifery Communication for Childbirth: A Practical Approach.

During labor and birth, inadvertent negative suggestions from care givers and support people are common and may lead to negative experiences known as nocebo effects. Counteracting nocebo effects with the use of effective communication techniques can reduce distress, enhance birth experiences, and improve health outcomes. There are very few examples in the literature describing how effective communication techniques can be used by midwives and other birth attendants when providing care to individuals in labor and birth. This article provides a practical structured communication approach to faci

Beyond the Counter: The Community Pharmacist of the Future.

Fifteen years ago, a community pharmacist's role in the United States was dispensing medication. Five years ago, it expanded to include administering routine vaccines. Fifteen years from now, the authors posit that it will include minor acute illness evaluation and treatment, chronic disease prevention, and complex disease management, including medication prescribing and administration. This evolution, which just began recently in the United States but is well under way in many other countries, would help to relieve pressure on primary care, control costs, improve outcomes, and increase access

Treatment of acute COVID-19 in the community.

COVID-19 infection in an otherwise healthy person is usually a mild to moderate self-limiting illness, but certain populations are at higher risk of severe disease (e.g. older age, severe immunocompromise, certain comorbidities). Adults with acute COVID-19 should be assessed for disease severity and underlying risk factors to guide appropriate management. Infection control, including case isolation, is important to reduce spread. Antiviral therapy is reserved for patients most likely to benefit, given its high cost, potential adverse effects and uncertain clinical benefit. Patients showing sig

The role of siderophores for bacterial community building.

Iron is an essential trace element for most organisms on earth including bacteria. However, iron in the environment generally occurs in its ferric (Fe3+) form and is therefore not bioavailable. To capture ferric iron, many bacteria produce siderophores - a class of secreted secondary metabolites that scavenge iron from natural sources and allow iron uptake via specific transporters. As secreted molecules, siderophores inevitably affect other community members, and recent evidence suggests that siderophores play important roles in shaping interaction networks between strains and species in comp

Community Water Fluoridation and Birth Outcomes.

IMPORTANCE: Community water fluoridation (CWF) is a widely implemented public health intervention aimed at preventing dental caries. However, concerns have emerged about potential unintended birth outcomes of fluoride exposure, particularly birth weight, which is a widely accepted summary measure of infant health and has been found to be associated with later-life health and human capital. OBJECTIVE: To evaluate the association between prenatal exposure to CWF and birth outcomes. DESIGN, SETTING, AND PARTICIPANTS: This cohort study was conducted using an event-study analysis with a differenc

Cardiometabolic Disease in Rural Southeastern Communities.

BACKGROUND: Cardiometabolic disease disproportionately affects rural communities, but early behavioral and physiological risk indicators are understudied in these populations. OBJECTIVES: The aim of this study was to characterize cardiometabolic health in a rural southeastern U.S. community. Primary objectives included assessing the prevalence of blood pressure and glycemic alterations and examining associations between modifiable factors such as sleep, nutrition, and educational attainment. A secondary objective was to assess community interest in future hypertension research. METHODS: A cr

Planned Community Birth and Birth Outcomes.

IMPORTANCE: Studies examining community births (ie, births that occur outside of a hospital setting, such as at home or in a birth center) often misclassify transfers to hospitals as hospital births. Oregon uniquely tracks planned birth location at labor onset. OBJECTIVES: To examine the association between planned place of delivery and perinatal outcomes in Oregon. DESIGN, SETTING, AND PARTICIPANTS: This was a population-based cohort study of singleton, nonbreech infants without lethal anomalies born at 37 to 44 weeks' gestation in Oregon between January 2012 and December 2020. Data were an

Perceptions of Deaf education in the Deaf community.

This article explores perceptions about education for the Deaf community within Deaf communities in Ireland, Finland, and the UK. Ireland, Finland, and the UK all provide legal recognition of sign languages in various forms. However, it is important to understand how these laws impact Deaf people, what their experiences with the law is, and how might the law be altered to improve the lives of people it purports to serve. Interview data from Deaf people across the three countries is presented here which explores these experiences. When asked about what they might improve in the law, an overwhel
