Palliative care in urology.

Urological malignancies, especially prostate cancer, are relatively common, but patients may live many years before eventually dying of the disease. Caring for these patients is an important role for urologists, although medical training often does not adequately prepare urologists for the palliative care of patients with advanced malignancies. Palliative care is no longer equated with end-of-life care, but rather integrated throughout illness, even when cure is impossible. This article focuses on the various palliative treatments available for the 3 most common urological malignancies: prosta

Hematologic and oncologic emergencies. Doing the most good in the least time.

Broad categories of emergency hematologic and oncologic situations are metabolic crises, compressions and obstructions, and symptomatic cytopenias. In each instance, a decision to intervene should be made on the basis of findings on diagnostic assessment in combination with prognostic information. Management should be directly proportional to the possibility for cure, significant remission, or improved quality of life. Numerous diseases, including potentially curable cancer, can be modified and patients' quality of life substantially improved with appropriate emergency intervention. Fortunatel

High-Fidelity Synthesis of Temporomandibular Joint Cone-Beam Computed Tomography Images via Latent Diffusion Models.

Background: The development of robust artificial intelligence (AI) models for diagnosing Temporomandibular Disorders (TMDs) is severely constrained by data scarcity and patient privacy regulations. Cone-beam computed tomography (CBCT), the gold standard for assessing osseous changes in the temporomandibular joint (TMJ), inherently contains sensitive biometric facial features, making de-identification difficult without losing critical anatomical information. This study aims to develop and evaluate TMJCTGenerator, a specialized latent diffusion model (LDM) framework designed to synthesize high-f

Cerebrospinal fluid pressure dynamics reveal signs of effective spinal canal narrowing in ambiguous spine conditions.

Spinal canal narrowing with consecutive spinal cord compression is considered a key mechanism in degenerative cervical myelopathy (DCM). DCM is a common spine condition associated with progressive neurological disability, and timely decompressive surgery is recommended. However, the clinical and radiological diagnostic workup is often ambiguous, challenging confident proactive treatment recommendations. Cerebrospinal fluid pressure dynamics (CSFP) are altered by spinal canal narrowing. Therefore, we aim to explore the potential value of bedside CSFP assessments for qualitative and quantitative

Cervical pedicle screws: conventional versus computer-assisted placement of cannulated screws.

STUDY DESIGN: Prospective clinical study with postoperative radiologic control of pedicle screw placement in the cervical spine. OBJECTIVES: To evaluate whether cervical pedicle screws can be placed safely in a conventional technique when using cannulated screws and separate stab incisions. Also, to evaluate if accuracy and safety of pedicle screw placement can be improved using a computer-assisted surgery (CAS) system (VectorVision; BrainLAB AG, Heimstetten, Germany). SUMMARY OF BACKGROUND DATA: Pedicle screws are rarely used in the cervical spine compared to the use in lumbar and thoracic

Morphometric texture analysis of spinal trabecular bone structure assessed using orthogonal radiographic projections.

The measurement of bone microstructure as well as bone mineral density may improve the estimation of bone strength. Cubic specimens (N = 26, 12 mm X 12 mm X 12 mm) of human cadaver vertebrae were cut along three orthogonal anatomic orientations, i.e., superior-inferior (SI), medial-lateral (ML), and anterior-posterior (AP). Contact radiographs of the bone cubes along all three orientations were obtained and then digitized by a laser scanner with pixel size of 50 microns x 50 microns. The specimens were tested in compression along the 3 orthogonal orientations and the Young's modulus (YM) was c

Machine learning-based MRI radiomics identifies patients with degenerative cervical myelopathy and predicts baseline function.

PURPOSE: The diagnosis of Degenerative cervical myelopathy (DCM) relies on clinical evaluation and conventional MRI, yet early symptoms are subtle and non-specific, creating diagnostic uncertainty. High rates of asymptomatic cervical spinal cord compression in the older adults further complicate clinical decisions regarding surgical intervention. MRI-based radiomic analysis quantifies microstructural spinal cord pathology through texture analysis, and is a potential objective imaging biomarker. This study aimed to assess whether machine learning models using MRI radiomics can accurately classi

Subthalamic Deep Brain Stimulation: Mapping Non-Motor Outcomes to Structural Connections.

In Parkinson's Disease (PD), deep brain stimulation of the subthalamic nucleus (STN-DBS) reliably improves motor symptoms, and the circuits mediating these effects have largely been identified. However, non-motor outcomes are more variable, and it remains unclear which specific brain circuits need to be modulated or avoided to improve them. Since numerous non-motor symptoms potentially respond to DBS, it is challenging to independently identify the circuits mediating each one of them. Data compression algorithms such as principal component analysis (PCA) may provide a powerful alternative. Thi…

Spinal cord metrics derived from diffusion MRI: improvement in prognostication in cervical spondylotic myelopathy compared with conventional MRI.

OBJECTIVE: A major shortcoming in optimizing care for patients with cervical spondylotic myelopathy (CSM) is the lack of robust quantitative imaging tools offered by conventional MRI. Advanced MRI modalities, such as diffusion MRI (dMRI), including diffusion tensor imaging (DTI) and diffusion basis spectrum imaging (DBSI), may help address this limitation by providing granular evaluations of spinal cord microstructure. METHODS: Forty-seven patients with CSM underwent comprehensive clinical assessments and dMRI, followed by DTI and DBSI modeling. Conventional MRI metrics included 10 total qual

Predicting CTS Diagnosis and Prognosis Based on Machine Learning Techniques.

Carpal tunnel syndrome (CTS) is a clinical disease that occurs due to compression of the median nerve in the carpal tunnel. The determination of the severity of carpal tunnel syndrome is essential to provide appropriate therapeutic interventions. Machine learning (ML)-based modeling can be used to classify diseases, make decisions, and create new therapeutic interventions. It is also used in medical research to implement predictive models. However, despite the growth in medical research based on ML and Deep Learning (DL), CTS research is still relatively scarce. While a few studies have develo

Radiomics Approach for Prediction of Recurrence in Non-Functioning Pituitary Macroadenomas.

OBJECTIVES: A subset of non-functioning pituitary macroadenomas (NFPAs) may exhibit early progression/recurrence (P/R) after surgical resection. The purpose of this study was to apply radiomics in predicting P/R in NFPAs. METHODS: Only patients who had undergone preoperative MRI and postoperative MRI follow-ups for more than 1 year were included in this study. From September 2010 to December 2017, 50 eligible patients diagnosed with pathologically confirmed NFPAs were identified. Preoperative coronal T2WI and contrast-enhanced (CE) T1WI imaging were analyzed by computer algorithms. For each i

Femoral neck fractures in patients with prior lower-limb amputation: a two-case series and narrative review of surgical decision-making.

BACKGROUND: Femoral neck fractures in patients with pre-existing lower-limb amputation are rare clinical conditions that pose unique technical challenges and surgical decision-making dilemmas for orthopedic surgeons. Altered hip biomechanics, reduced bone mineral density, and dependence on prosthetic ambulation complicate preoperative planning and postoperative rehabilitation, and no unified therapeutic consensus has been established in clinical practice ( 1- 5). CASE PRESENTATION: We report two distinct clinical cases of femoral neck fractures in amputees managed with different surgical stra

Correcting fast irregular motion in PET: maximum-likelihood motion and activity (MLMA) reconstruction.

PURPOSE: Positron emission tomography (PET) imaging naturally suffers from motion blur due to long acquisitions. As such, motion-compensation provides a promising solution to improve image quality. Traditional methods for motion-correction often involve a combination of gating and data-binning, assuming that motion is periodic, to accumulate sufficient counts per motion-state frame. Irregular motion can be estimated but it requires complex motion-capture systems or elaborate data-driven algorithms, which are difficult to configure (physical or model setup) and hampered by the high noise of sho

Understanding the Mechanical Properties of Pituitary Adenomas for Optimized Surgery.

Pituitary adenoma (PA) is a common brain tumor located in a small cavity at the cranial base. It disrupts hormonal balance and compresses the optic nerves, leading to abnormal body growth, sexual dysfunction, vision loss, and mortality if untreated. Its surgical resection is highly challenging due to its small size, heterogeneous structure, deep location, and indistinct interface with surrounding nerves, arteries, and brain tissues. Mechanical properties of tumor tissues play a crucial role in their microstructure, growth, and progression. However, data on the mechanical properties of PA tissu

Design of resonant cavity for linear cavity single frequency fiber laser based on temperature controlled Fiber Bragg grating.

In response to the limited tuning ability of traditional linear cavity single frequency fiber lasers caused by fixed cavity length and static feedback mechanism, a resonant cavity design for linear cavity single frequency fiber lasers based on temperature controlled fiber gratings is proposed. It achieves synergistic improvement in linewidth compression, power stability, and tuning range through thermal optical coupling modeling and intelligent algorithm collaborative search. The experimental results show that the average line width of the proposed scheme is 0.86 Hz (compared to 1.97 Hz and 1.

Intelligent low carbon reinforced concrete beam design optimization via deep reinforcement learning.

Reinforced concrete (RC) beam design currently faces significant challenges from the substantial carbon footprint of cementitious materials and the lack of practical automated tools for simultaneous structural-environmental optimization. To address this, this paper proposes an innovative two-stage framework by utilizing artificial neural networks (ANNs) and deep reinforcement learning (RL) to automate the design of sustainable and low-carbon RC beams. In the first stage, following a comprehensive analysis of 14 machine learning algorithms, an ANN was selected for its superior predictive accura

Artificial intelligence and machine learning in the management of patients with degenerative cervical myelopathy: a systematic review.

INTRODUCTION: Degenerative cervical myelopathy (DCM) is a debilitating condition caused by compression of the spinal cord. Despite established surgical treatments, accurate diagnosis and prognostication remain challenging in part due to the variability in clinical presentation and lack of screening tools. Machine learning (ML) has emerged as a promising approach to address these challenges through its predictive capabilities for diagnosis, decision-making, and prognostication. Given the recent advent of ML, there is a need to systematically synthesize its applications to the treatment of patie

Cycle contrastive adversarial learning with structural consistency for unsupervised high-quality image deraining transformer.

In overcoming the challenges faced in adapting to paired real-world data, recent unsupervised single image deraining (SID) methods have proven capable of accomplishing notably acceptable deraining performance. However, the previous methods usually fail to produce a high quality rain-free image due to neglecting sufficient attention to semantic representation and the image content, which results in the inability to completely separate the content from the rain layer. In this paper, we develop a novel cycle contrastive adversarial framework for unsupervised SID, which mainly consists of cycle co
