BACKGROUND: Type 1 diabetes mellitus (T1DM) increases fracture risk beyond bone mineral density (BMD), implicating matrix quality. Whether ultrashort echo time magnetization transfer (UTE-MT) MRI can track collagen alterations in T1DM and their associations with mineralization is unknown.
PURPOSE: To evaluate UTE-MT-derived magnetization transfer ratio (MTR) for monitoring in vivo cortical collagen alterations in T1DM and to investigate its associations with mineralization and multiscale bone properties.
STUDY TYPE: Longitudinal animal model.
ANIMAL MODEL: 18 male New Zealand white rabbits

Peri-implant marginal bone stability remains a major determinant of long-term implant success, yet clinical studies report early marginal bone changes ranging from near-stable conditions in some protocols to approximately 1-2 mm during the first year in more traditional series, underscoring considerable biological variability. In the present review, a remodeling-dominant state refers to turnover-led peri-implant adaptation with limited net cortical gain, whereas modeling-driven apposition refers to uncoupled surface bone addition and cortical reinforcement. We conducted a structured narrative

Background/Objectives: Full-endoscopic transforaminal approach lumbar interbody fusion using the percutaneous endoscopic transforaminal lumbar interbody fusion (PETLIF) system is increasingly adopted as a minimally invasive technique for degenerative lumbar disorders. Screw trajectory selection may influence tissue handling, muscle injury, and postoperative pain. Comparative evidence between percutaneous pedicle screws (PPS) and cortical bone trajectory (CBT) screws in PETLIF is limited. This retrospective, non-randomized comparative study evaluated short-term outcomes between CBT and PPS fixa…

UNLABELLED: Fractures occur when loads exceed bone capacity. Cortical Bone Mechanics Technology (CBMT) noninvasively assesses ulnar bending and yields flexural rigidity (EI), a mechanical property strongly associated with whole-bone bending strength. No framework places CBMT-reported EI on a fall-demand scale.
METHODS: We developed an illustrative, reduced-order scaling framework to estimate the CBMT-reported EI required for modeled ulnar bending capacity to match bending-equivalent demand across forward-fall-on-an-outstretched-hand (FOOSH) scenarios. This central required-EI estimate was den…

BACKGROUND: The use of the interference screw (IFS) for the cortical fixation of tendon grafts in knee ligament reconstruction may lead to converging tunnels in the multiligament reconstruction setting. It is unknown whether alternative techniques using modern suture anchor (SA) or bone staple (BS) fixation provide sufficient primary stability.
PURPOSE: To assess the primary stability of cortical fixation of tendon grafts for medial collateral ligament (MCL) reconstruction using modern SA and BS methods in comparison with IFS fixation.
STUDY DESIGN: Controlled laboratory study.

Low energy availability in female athletes often causes hypothalamic amenorrhea and osteoporosis, in turn promoting stress fractures. Mechanisms underlying these conditions remain unclear. Here we show that model mice subjected to food restriction (FR) or FR-plus-voluntary running exercise exhibit significantly reduced bone mineral density, cortical bone parameters and uterine weight than do control mice, and that these parameters worsen in the FR-plus-exercise group. Relative to controls, FR and FR-plus-exercise groups showed significantly lower mineral apposition rate and osteoclast number a…

Immobilization as a result of long-term bed rest can lead to gradual bone loss. Because of their distribution throughout the bone matrix and remarkable interconnectivity, osteocytes represent the major mechanosensors in bone and translate mechanical into biochemical signals controlling bone remodeling. To test whether immobilization affects the characteristics of the osteocyte network in human cortical bone, femoral diaphyseal bone specimens were analyzed in immobilized female individuals and compared with age-matched postmenopausal individuals with primary osteoporosis. Premenopausal and post…

The goal of this paper is to predict the micro-architectural parameters of cortical bone such as pore diameter (ϕ) and porosity (v) from ultrasound attenuation measurements using an artificial neural network (ANN). Slices from a 3-D CT scan of human femur are obtained. The micro-architectural parameters of porosity such as average pore size and porosity are calculated using image processing. When ultrasound waves propagate in porous structures, attenuation is observed due to scattering. Two-dimensional finite-difference time-domain simulations are carried out to obtain frequency dependent atte…

Cortical bone trajectory (CBT) is a novel pedicle insertion technique with comparable or superior mechanical properties and reduced invasiveness compared to traditional methods. We describe the screw accuracy, complications, and learning curve associated with CBT use. A prospective cohort study was performed involving 22 patients who underwent lumbar fusion with CBT screw placement. A total of 100 cortical screws were placed. Post-operative CT scans were reviewed to assess the adequacy of screw placement and calculate the incidence of vertebral body and pedicle breaches from cortical screw pla…

Thoracic and lumbar cortical bone trajectory pedicle screws have been described in adult spine surgery. They have likewise been described in pediatric CT-based morphometric studies; however, clinical experience in the pediatric age group is limited. The authors here describe the use of cortical bone trajectory pedicle screws in posterior instrumented spinal fusions from the upper thoracic to the lumbar spine in 12 children. This dedicated study represents the initial use of cortical screws in pediatric spine surgery. The authors retrospectively reviewed the demographics and procedural data of …

Bone marrow-derived mesenchymal stromal cells (BM-MSCs) represent a promising tool for stem cell-based therapies. However, the majority of MSCs fail to reach the injury site and have only minimal therapeutic effect. In this study, we assessed whether hypoxia/reoxygenation (H/R) preconditioning of human BM-MSCs could increase their functional capacity and beneficial effect on ischemic rat cortical neurons. Human BM-MSCs were cultured under hypoxia (1% O2) and with long-term reoxygenation for various times to identify the optimal conditions for increasing their viability and proliferation. The e
