A clinical carepath for obese pregnant women: a pragmatic pilot cluster randomized controlled trial.

Objectives: Obese women are at increased risks for complications during pregnancy, birth, and in their infants. Although guidelines have been established for the clinical care of obese pregnant women, management is sometimes suboptimal. Our goal was to determine the feasibility of implementing and testing a clinical carepath for obese pregnant women compared to standard care, in a pilot cluster randomized controlled trial (RCT). Methods: A pragmatic pilot cluster RCT was conducted, randomly allocating eight clinics to the carepath or standard care for obese pregnant women. Women were eligible

Effects of clinical pathway implementation on antibiotic prescriptions for pediatric community-acquired pneumonia.

BACKGROUND: Italian pediatric antimicrobial prescription rates are among the highest in Europe. As a first step in an Antimicrobial Stewardship Program, we implemented a Clinical Pathway (CP) for Community Acquired Pneumonia with the aim of decreasing overall prescription of antibiotics, especially broad-spectrum. MATERIALS AND METHODS: The CP was implemented on 10/01/2015. We collected antibiotic prescribing and outcomes data from children aged 3 months-15 years diagnosed with CAP from 10/15/2014 to 04/15/2015 (pre-intervention period) and from 10/15/2015 to 04/15/2016 (post-intervention per

A phase II trial of a surgical protocol to decrease the incidence of wound complications in obese gynecologic oncology patients.

OBJECTIVES: Obese women have a high incidence of wound separation after gynecologic surgery. We explored the effect of a prospective care pathway on the incidence of wound complications. METHODS: Women with a body mass index (BMI) ≥30 kg/m(2) undergoing a gynecologic procedure by a gynecologic oncologist via a vertical abdominal incision were eligible. The surgical protocol required: skin and subcutaneous tissues to be incised using a scalpel or cutting electrocautery, fascial closure using #1 polydioxanone suture, placement of a 7 mm Jackson-Pratt drain below Camper's fascia, closure of Camp

A comprehensive acute coronary syndrome algorithm for centers with percutaneous coronary intervention capability.

A comprehensive acute coronary syndrome (ACS) protocol was developed to improve the quality of care for patients admitted with definite or probable ACS. These protocols were constructed to streamline the practice for diverse clinicians who care for ACS patients across a variety of clinical settings. They are applicable in the emergency department, the cardiac catheterization laboratory, and the inpatient settings for hospitals with primary percutaneous coronary intervention capability. These protocols standardized the care by selecting the best therapy for each clinical scenario based on avail

Prosthetic treatment of hip fractures in the elderly patient.

As the elderly population in our society significantly increases, the incidence of displaced femoral neck fractures will increase proportionally. Three surgical procedures are available to treat such fractures: internal fixation, hemiarthroplasty (unipolar or bipolar), and total hip arthroplasty. Long-term costs and efficacy of these three procedures vary, primarily due to postoperative complications. Thus, it is imperative that all surgeons conduct a proper preoperative evaluation of each patient before choosing the optimal treatment plan. Internal fixation has been shown to be more beneficia

Fast-track paediatric cardiac surgery: the feasibility and benefits of a protocol for uncomplicated cases.

OBJECTIVE: Fast-track patient pathways for cardiac surgery are used in adult practice and by necessity is a mainstay in the developing world. We aimed to introduce a fast-track protocol for uncomplicated paediatric open-heart surgery cases and to subsequently review the results of this change in practice. METHODS: A fast-track protocol co-ordinated by the Advanced Nurse Practitioners was introduced in January 2006 for children aged over 6 months undergoing uncomplicated open-heart procedures. We conducted a review of prospectively collected data on all included patients. The setting was a ter

Clinical staging of acute limb ischemia as the basis for choice of revascularization method: when and how to intervene.

In acute lower limb ischemia, there are basically three management options: (1) clot removal by catheter-directed thrombolysis with or without percutaneous mechanical thrombectomy, (2) surgical thromboembolectomy followed by correction of underlying arterial lesions, and (3) anticoagulation with continued observation. Arterial embolic occlusion presents more abruptly and with more severe ischemia than arterial thrombosis, which occurs in narrowed arterial segments that have generally developed some degree of collateral circulation. The appropriate choice of treatment for acute limb ischemia de

Age- and experience-dependent expression of dynamin I and synaptotagmin I in cat visual system.

Dynamin I (Dyn I) and Synaptotagmin I (Syt I) are essential for endocytosis-exocytosis processes, thus for neurotransmission. Despite their related function at presynaptic terminals, Dyn I and Syt I displayed opposite expression patterns during visual cortex maturation in the cat. Dyn I was more abundantly expressed in adults, while Syt I exhibited higher levels in kittens of postnatal day 30 (P30). In area 17 this developmental difference was most obvious in layers II/III. Layer VI displayed a strong hybridization signal for both molecules, independent of age. In addition, Syt I levels were h

The evidence for antidepressant use in bipolar depression.

Mood elevation, which includes mania, hypomania, and mixed states, was previously considered the defining symptom of bipolar disorder, but bipolar depression by comparison is actually a much more substantial challenge to diagnose and treat. Recent studies, including research by the Systematic Treatment Enhancement Program for Bipolar Disorder (STEP-BD), found that patients with bipolar disorder spend longer periods of time in depressive episodes and are more likely to relapse to depression compared with mania or hypomania. However, the treatment of bipolar depression is hampered by the limited

A randomized controlled trial of an asthma clinical pathway for children in general practice.

AIMS: To evaluate the effect of the implementation of an asthma clinical pathway on asthma in children in general practice. METHODS: A randomized, controlled trial involving 270 general practitioners. One group of general practitioners implemented the asthma clinical pathway for children (intervention group) and the control group continued with their usual asthma medical care management. The main outcome measures were admissions to hospital for asthma and attendance at the Children's Emergency Department. Compliance with the guidelines was assessed by examining asthma drug prescriptions. RES

Factors affecting cost and length of stay associated with the ileal pouch-anal anastomosis.

PURPOSE: The purpose of this study was to evaluate the costs associated with the ileal pouch-anal anastomosis procedure and identify those factors that most affected or determined such costs. Specifically evaluated were the costs, complication rates, and length of stay associated with the ileal pouch-anal anastomosis done as a one-stage, two-stage, two-stage modified, or three-stage procedure. METHODS: Costs from the hospital accounting database and clinical data from retrospective chart review of 135 ileal pouch-anal anastomosis patients from a ten-year period were compiled. Overall costs an

Exploring Clinical Pathways to a Metastatic Lung Cancer Diagnosis: A Latent Class Analysis Using North Carolina Multipayer Claims Data.

BACKGROUND: Non-small cell lung cancer (NSCLC) is the leading cause of cancer mortality in the United States and North Carolina (NC), accounting for more than 5,600 deaths annually in NC. Nearly half of patients are diagnosed with metastatic disease, and the 5-year survival rate is 12%. This study identified clinical pathways preceding the diagnosis of metastatic NSCLC using the North Carolina Central Cancer Registry (NCCCR) linked with multipayer utilization data. METHODS: We identified individuals diagnosed with de novo metastatic NSCLC between 2015 and 2019 in the NCCCR and linked them to

Understanding child mental health care pathways in community-based services: a qualitative study in a French child and adolescent medico-social centre (CMPP).

BACKGROUND: Child and adolescent mental health is increasingly recognized as a major public health concern worldwide. However, access to timely and coordinated mental health care remains uneven, particularly for children experiencing psychosocial, developmental or learning difficulties. Fragmentation across health, social care and education systems often contributes to discontinuous care pathways, especially in resource-constrained and rural settings. While a substantial body of literature on child mental health services originates from high-income countries, the organization of services diffe

A multidisciplinary care pathway for colorectal carcinoma: Impact on treatment decisions.

INTRODUCTION: Colorectal carcinoma predominantly affects older adults (defined as ≥75 years). A proportion of these patients are frail and have multimorbidity, reflecting multidimensional vulnerability related to comorbidities and functional status. These factors complicate treatment decisions, especially regarding surgical interventions and systemic therapies. In response, a multidisciplinary clinical care pathway was established at Catharina Hospital in 2021 to facilitate individualized treatment management for older and frail patients with colorectal cancer. This pathway was led by physicia
