Disseminated cryptococcosis in systemic lupus erythematosus: Case report.

We report a rare case of disseminated cryptococcosis gattii infection with initial central nervous system manifestations in a 28-year old male under regular treatment for systemic lupus erythematosus since 2020. He presented with headache evolving into hearing loss, hypogeusia, amblyopia and mental confusion. This case highlights the challenge of diagnosing opportunistic fungal infections in patients with chronic immuno-suppression, emphasizing how early targeted management is critical to mitigate severe neurological sequelae and reduce mortality.

Pulmonary Cryptococcosis Mimicking Lung Cancer: A Diagnostic Challenge.

Pulmonary cryptococcosis, although rare, maybe seen in both immunocompromised and immunocompetent patients. Cryptococcosis presenting as a lung mass mimicking lung cancer is very rare. Here, we report our experience with pulmonary cryptococcosis presenting as a lung mass mimicking malignancy in an immunocompetent patient. In this case, the patient presented to us with left-sided pleural effusion and lung mass on computed tomography (CT) of the chest. Bronchoscopy and endobronchial ultrasound (EBUS)-guided fine needle aspiration cytology (FNAC) was performed, which showed cryptococcal organisms…

Primary Cutaneous Cryptococcosis: An Unusual Injection Site Infection.

Primary cutaneous cryptococcosis (PCC) is an uncommonly reported entity. We describe an unusual case of PCC in an injection site of an immunocompromised patient. The specific case demonstrates a challenging treating dilemma with different alternative treatment choices. In the presented clinical setting, each choice concealed its risks and benefits. We highlight the importance of patient education for taking the appropriate measures for the disinfection of subcutaneous injection sites.

Disseminated Cryptococcosis After Liver Transplant: A Case Report.

Cryptococcosis is an opportunistic infection caused by the Basidiomycota Cryptococcus neoformans (Cryptococcus gattii), which affects immunosuppressed patients and less frequently immunocompetent patients. Solid-organ transplant recipients are a particularly high-risk group, depending on the net state of immunosuppression. In these patients, the infection usually appears after the first year after transplant, although it may occur earlier in liver transplant recipients. In most cases, the infection is secondary to the reactivation of a latent infection, although it may be due to an unidentifie…

18F-FDG PET/CT Imaging in Adrenal Cryptococcosis.

Cryptococcosis is a fungal infection, relatively rare in the absence of impaired immunity. Lung or brain may be involved. Cryptococcal dissemination is rare in absence of impaired immunity, and documentation of alone adrenal involvement without central nervous system involvement is rare. Here, we present a case of an immunocompetent patient presented with bilateral adrenal cryptococcosis with adrenal insufficiency, without central nervous system involvement and F-FDG PET/CT correctly identified the residual disease.

Etiological factors of cryptococcosis - what makes them pathogens ?

A significant participation in the growing prevalence of fungal infections have basidiomycete yeasts of the genus of Cryptococcus. Among about seventy actually known species belonging to Cryptococcus genus there are only two: Cryptococcus neoformans and Cryptococcus gattii, which as anamorphs in parasitic stage of its life fully evolved the mechanisms predisposing them to be a human and animal pathogens. Specific attributes important in the pathogenesis of cryptococcosis called virulence factors allow them to colonize the mammalian tissues. Undoubtedly, the most important role in the pathogene…

Opportunistic yeast infections: candidiasis, cryptococcosis, trichosporonosis and geotrichosis.

Opportunistic yeast infections are diseases caused by fungi which normally are saprophytic and do not cause disease in humans or animals. The prevalence of these diseases has been increasing due to immunosuppressive, corticosteroid, and long-term antibiotic treatment following organ transplantation or after serious metabolic, hematological, or immunological diseases. We review epidemiological, clinical, diagnostic, and therapeutic aspects of the four "big" opportunistic yeast infections: candidiasis, cryptococcosis, trichosporonosis, and geotrichosis.

How I treat cryptococcosis in organ transplant recipients.

Cryptococcosis is a significant opportunistic mycoses in organ transplant recipients. Topical developments in the field in the past few years have highlighted important issues and at the same time raised new questions regarding the management of this yeast. These include, for example, management of pretransplant cryptococcosis during transplant candidacy and timing of transplant in these instances; potential for donor transmission of cryptococcosis in light of recent fatal transmissions; and prevention and treatment of Cryptococcus-associated immune reconstitution syndrome. Discussed herein ar…

Primary cutaneous cryptococcosis and Cryptococcus neoformans serotype D.

We report a healthy, 73-year-old Japanese woman who presented with primary cryptococcosis on the skin of both cheeks. She had initially developed an erythematous, partly ulcerated lesion on the right cheek 2 weeks earlier following an injury. There was no regional lymphadenopathy, and chest X-rays were normal. Histopathological findings showed granulomatous cell infiltration. Periodic acid Schiff staining revealed spores that were identified by the indirect immunoperoxidase staining method as Cryptococcus neoformans. The isolate was identified as C. neoformans var. neoformans serotype D. The s…

Cryptococcosis in AIDS patients: observations concerning CNS involvement.

The clinical course and response to therapy of seven patients with cryptococcosis and AIDS were reviewed. One patient was still in the primary stage of cryptococcosis in AIDS, i.e. the stage that is characterized by the sole cultural detection of Cryptococcus neoformans in the respiratory tract. The other six patients were in the secondary stage, where C. neoformans can be detected from the cerebrospinal fluid (CSF), blood, urine, faeces and other body sites. The main presenting features (headache, fever, nausea) were due to central nervous system involvement, although meningism and mental cha…

[Iatrogenic colitis during flucytosine treatment for neuromeningeal cryptococcosis].

The authors report a case of colitis, with anatomo-clinical confrontation, during a treatment with flucytosine. In this fatal colitis, there were neither ulcerations nor pseudo-membranes. The histological data, with significant cytonuclear lesions, were consistent with a toxic-induced disorder of protein synthesis.

Cryptococcosis after renal transplantation: report of ten cases.

Ten cases of cryptococcosis have been identified in a 13 year experience with more than 650 renal transplants. Eight patients had meningitis, one patient had a cerebral granuloma, and in one patient the infection appeared to be limited to the lungs. The central nervous system infection often masqueraded as brain tumor and was not suspected initially. The most useful diagnostic test was cerebrospinal fluid examination including India ink preparation. Various ther apeutic regimens with amphotericin B and 5-fluorocytosine were effective in suppressing the infection. A combination of low doses of …
