[Treatment of syringomyelia in patients with Chiari malformation and craniosynostosis. A case report and review of the literature].

INTRODUCTION: Patients with multisutural or single craniosynostosis, often suffer from Chiari malformation and syringomyelia. The surgical management of syringomyelia in these patients is controversial. CASE REPORT: A 3-year-old girl was referred with complex craniosynostosis that had not been corrected surgically. She was asymptomatic despite the cranial MRI showed a Chiari malformation and one year later she developed a cervico-dorso-lumbar syringomyelia. She underwent a decompressive suboccipital craniectomy but subsequently suffered a worsening of syringomyelia. The intracranial pressure

Anesthesia following overfilling of a root canal sealer into the mandibular canal: a case report.

The surgical treatment of a case of anesthesia that occurred with the extrusion of Endomethasone root canal sealer into the mandibular canal is presented. Endomethasone is a neurotoxic root canal sealer containing paraformaldehyde and eugenol. The literature indicates immediate surgical decompression on the extrusion of Endomethasone into the mandibular canal. In our case, the decompression surgery was done 3 weeks after the endodontic mishap. The nearly complete resolution of anesthesia 4 months following the decompression surgery suggests that the neurotoxic effects of Endomethasone are stil

A retrospective review of 26 cases of dysthyroid optic neuropathy.

Sixteen patients (14 women and two men) with dysthyroid optic neuropathy (26 involved eyes) were treated with either oral corticosteroids, orbital irradiation, surgical orbital decompression, combined corticosteroids and irradiation, or combined corticosteroids and surgical decompression. Thirteen of 16 eyes responded favorably to corticosteroid therapy but eight of the 13 relapsed upon discontinuation of treatment. Two of four eyes responded to irradiation initially but later relapsed. The response to orbital decompression was almost uniformly beneficial (eight of nine eyes responded) and las

Diagnosis and management of compartmental syndromes.

Patients at risk for compartmental syndromes challenge both the diagnostic and the therapeutic abilities of the physician. Suboptimum results may be due to delays in diagnosis and treatment, to incomplete surgical decompression, and to difficulties in the management of the limb after decompression. Although careful clinical assessment permits the diagnosis of a compartmental syndrome in most patients, we have found measurement of tissue pressure and direct nerve stimulation to be helpful for resolving ambiguous or equivocal cases. In our experience, the four-compartment parafibular approach to

Necessity of fusion following decompression surgery in patients with single-level lumbar stenosis: study protocol for an open-label multicentre non-inferiority randomized controlled clinical trial.

BACKGROUND: The necessity of spinal segment fusion after decompression is one of the most controversial and unresolved issues in single-level lumbar spinal stenosis surgery. To date, only one trial carried out 15 years ago focused on this problem. The key purpose of the current trial is to compare the long-term clinical results of the two surgical methods (decompression vs. decompression and fusion) in patients with single-level lumbar stenosis. METHODS: This study is focused on the non-inferior clinical results of decompression compared with the standard fusion procedure. In the decompressio

Median arcuate ligament compression of the mesenteric vasculature.

Compression of the celiac artery by fibrous bands of the diaphragmatic crura has been associated with gastrointestinal symptoms such as postprandial pain and delayed gastric emptying, a phenomenon known as median arcuate ligament syndrome. The hemodynamic effects of this compression have also been implicated in the development of aneurysms of the celiac artery or its visceral collaterals. Both open surgical decompression and laparoscopic decompression of the celiac artery have proven effective in the treatment of the compressive syndrome. Endovascular stent placement has largely supplanted ope

[Intraoperative brainstem auditory evoked potential in the microvascular decompression of the 5th and 7th cranial nerves].

Microvascular decompression techniques are rational surgical procedures for treating trigeminal neuralgia and facial hemispasm, with results ranging from good to excellent in 90% of cases. Among the most frequent complications recorded in the literature concerning these decompressions, impairment of the facial nerve and auditory nerve account for 5% to 10% due to mechanical traction and/or vascular injury. The peroperative use of early brain-stem evoked potentials (BAEP) was performed on 17 patients in a series of 325 decompressions of the trigeminal nerve, and on 17 patients in a series of 25

Robotic-Assisted Decompression, Decortication, and Instrumentation for Minimally Invasive Transforaminal Lumbar Interbody Fusion.

BACKGROUND: Robotic-assisted spine surgery has been reported to improve the accuracy and safety of pedicle screw placement and to reduce blood loss, hospital length of stay, and early postoperative pain1. Minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) is a procedure that is well suited to be improved by recent innovations in robotic-assisted spine surgery. Heretofore, the capability of robotic navigation and software in spine surgery has been limited to assistance with pedicle screw insertion. Surgical decompression and decortication of osseous anatomy in preparation for b

How i do it: endoscopic transposition technique for hemifacial spasm caused by AICA compression.

BACKGROUND: Anterior inferior cerebellar artery (AICA) compression is a common cause of hemifacial spasm (HFS). Endoscopic-assisted microvascular decompression (eMVD) offers several advantages, including minimal invasiveness, wide viewing angles, and clear visualization. METHODS: The surgical procedure was performed under a four-step endoscopic technique. Step 1: A 0° endoscope was used to dissect the cerebellopontine cistern and release cerebrospinal fluid for decompression. Step 2: A 30° endoscope was employed to dissect the arachnoid around the facial nerve and expose the root exit zone (R

Treatment of patients with spinal metastases from head and neck neoplasms.

BACKGROUND: Spinal metastases are uncommon in patients with advanced head and neck cancer. Treatment strategies in this patient group have not been defined. Although it has been established that neurologic dysfunction in patients with spinal metastases and cord compression constitutes an oncologic emergency, the role of surgical treatment remains controversial. OBJECTIVE: To clarify the treatment options in patients with head and neck cancer who develop spinal metastases. METHODS: The clinical course of patients seen at our institution with head and neck neoplasms and spinal metastases from
