Irritant contact dermatitis is the most frequent cause of occupational hand eczema and results from undue environmental demands on the skin. Preventive measures to reduce the irritant stress to the skin is the best method of tackling this problem.

8 cases of allergic contact dermatitis to nickel in infants are reported. All showed a papular dermatitis matching the sites of contact. Patch testing was performed on 3 patients, 2 were tested to nickel sulfate in pet. at concentrations of 1.0%, 1.5%, 2.0%. 1 was tested to 2.5% alone. All developed ++ reactions at each concentration tested. We observed a strong association of nickel dermatitis with atopy; 7 of 8 patients had a family history of atopy and 5 of 8 had features of coexistent atopic dermatitis. We report the fourth case of coexisting dermatitis herpetiformis and vitiligo. This patient's HLA haplotype is HLA-B8 DR3, which is characteristic of dermatitis herpetiformis. The autoimmune features of both disorders suggest that their coexistence is not coincidental.

For a period of 12 years, 539 patients with shoe and dye contact dermatitis, were studied in 2 out-patient clinics in Lisbon. 75% of the patients had a bilateral recurrent dermatitis confined to the dorsa of the feet. PTBP resin and chromium proved to be the 2 principal allergens, but when lesions were localized to the soles, rubber additives, chiefly mercaptobenzothiazole, were the predominant ones. A study was carried out on 50 workers in a floriculture centre to evaluate the incidence of contact dermatitis to Alstroemeria. 3 subjects gave positive reactions to aqueous and ethanolic extracts of cut flowers, stems and leaves. By column chromatography, the allergen was isolated and its chemical structure identified as 6-tuliposide A by proton magnetic resonance and carbon-13 magnetic resonance. The role of cytology in the diagnosis and differentiation of contact dermatitis of two mojor types-allergic and irritant is evaluated. Study in 10 cases of allergic contact dermatitis and 5 cases of irritant dermatitis revealed that lymphocytes were the predominant cells in cytologic smears in the allergic contact dermatitis. But the polymorphs predominated in primary irritant dermatitis. It is concluded that cytology. Will be of help in differentiating the two types of contact dermatitis.

Redness and scaling of the feet in prepubertal children is likely to represent a skin disease other than tinea pedis. If the dermatitis involves the dorsum of the feet, allergic contact dermatitis is a likely cause. In this study, 8/19 (42%) children with involvement of the dorsum of the feet had clinically relevant positive patch tests related to shoe allergens. In a 3-year period, 551 patients were patch tested with chlorhexidine gluconate 1% in water. 14 patients showed a strong and obviously relevant reaction. A severe dermatitis developed during treatment with a chlorhexidine preparation in 10 patients with venous or traumatic ulcers of the leg, and in 4 patients with skin infection on the face and/or scalp. So far the sensitizing potential of chlorhexidine has probably been underestimated.

168 children, 14 years of age or younger, were patch tested with the Standard Series of the International Contact Dermatitis Research Group (ICDRG) over a 5-year period. 77 of the children had 1 or more positive reactions; relevant test results were found in 80% of them. 8 boys and 24 girls were allergic to nickel, which proved to be the most common allergen; 5 boys and 6 girls reacted to chromate, and 5 boys and 4 girls reacted to one or more of the rubber chemicals.

Primary hypothyroidism, dermatomyositis, dermatitis herpetiformis (DH), and Sjögren's syndrome developed sequentially, over a 20-year period, in a 67-year-old woman. The principal manifestations of her illness were periodic and simultaneous exacerbations of dermatomyositis and DH. Her histocompatability antigens were A1,B8, a haplotype associated with autoimmune diseases. We suggest that this unusual disease complex has a genetic basis.

Diaper area dermatitis is one of the most frequent dermatosis in infants. Knowledge of clinics, but specially of the management of this entity is important, since references to this respect are not up-to-date in spite of the importance of this disease. Newborns adn infants present this condition, due to the fact that vesical and anal sphincters are still beyond control and because the diaper is one of the most important clothing articles in babies and often in preschool or schoolage children complaining of some related pathological condition.
