Management of food allergies.

The mainstay of successful management of food allergy is the identification of the offending food(s), as verified by the elimination-challenge test. Dietary elimination is the most effective and inexpensive method, but its success depends on several factors. Initially, dietary elimination may need to be strict, but later certain patients may discover that they can tolerate limited quantities or certain preparations of the offending food without significant symptoms. In certain cases, provision of a substitute or a nutritional supplement may be needed. Food allergy in infants who are exclusivel

[Diagnosis and treatment of acute purulent pleurisy in children].

Under consideration is the diagnosis and treatment of different forms of staphylococcic destruction of the lung in children. The immediate and late results of treatment are studied. Application of the intensive therapy in association with different surgical interventions enabled the authors to obtain good immediate and late results in children with staphylococcic destruction of the lung.

Prognostic criteria in macular histoplasmic choroiditis.

1. Fifty percent of 22 patients "desensitized" to histoplasmin developed subsequent activity, as compared to 44% of 23 patients without "desensitization." 2. Prednisone-induced remission of subclinical activity in patients monitoring their own central fields appears to be the most rewarding therapy for these desperate patients.

Allergen immunotherapy for IgE-mediated food allergy: a systematic review and meta-analysis.

BACKGROUND: The European Academy of Allergy and Clinical Immunology (EAACI) is developing Guidelines for Allergen Immunotherapy (AIT) for IgE-mediated Food Allergy. To inform the development of clinical recommendations, we sought to critically assess evidence on the effectiveness, safety and cost-effectiveness of AIT in the management of food allergy. METHODS: We undertook a systematic review and meta-analysis that involved searching nine international electronic databases for randomized controlled trials (RCTs) and nonrandomized studies (NRS). Eligible studies were independently assessed by

Risk-adapted HLA delisting and imlifidase-enabled deceased-donor kidney transplantation in highly sensitized kidney transplant candidates: a German expert consensus report.

Kidney transplantation is the preferred treatment for suitable patients with end-stage renal disease; however, access to transplantation declines dramatically with increasing HLA sensitization. While the acceptable mismatch (AM) program by Eurotransplant improves transplantability for highly sensitized candidates, a clinically relevant subgroup with extremely low donor frequency or ineligible for AM remains disadvantaged. In carefully selected cases, the controlled delisting of unacceptable HLA antigens and the use of peri-transplant desensitization (e.g., imlifidase) may enable transplantatio

Probable loop diuretic-induced pancreatitis in a sulfonamide-allergic patient.

OBJECTIVE: To report the case of a patient with a prior allergy to a sulfonamide antibiotic who subsequently developed the same reaction when administered various loop diuretics. CASE SUMMARY: A 57-year-old female with cardiomyopathy and "sulfa" (trimethoprim/sulfamethoxazole) allergy documented as pancreatitis presented with symptoms consistent with pancreatitis after use of furosemide. She subsequently developed similar symptoms after multiple rechallenges with various loop diuretics including furosemide, bumetanide, and torsemide. The patient was placed on ethacrynic acid until she was des

ABO-Incompatible Kidney Transplantation as a Strategic Expansion Tool in Low- and Middle-Income Countries.

The persistent disparity between organ supply and demand remains the principal limitation to kidney transplantation worldwide. ABO-incompatible kidney transplant has progressively evolved from an experimental procedure to an established strategy for expanding the living donor pool. This review critically evaluated whether ABO-incompatible kidney transplant represents a justified, effective, and feasible solution in low- and middle-income countries. We conducted a structured review of international registry data, meta-analyses, and major cohort studies to assess epidemiology, historical evoluti

Donor-specific antibodies in kidney transplantation: the University of Wisconsin experience.

PURPOSE OF REVIEW: Antibody-mediated rejection (ABMR) is the leading cause of allograft failure in kidney transplant recipients. This article provides a summary of the clinical research relating to donor-specific antibodies (DSA) and ABMR in kidney transplant recipients at the University of Wisconsin-Madison Transplant Center. RECENT FINDINGS: Over 40% of the kidney transplant candidates on the UNOS waitlist are sensitized, and both preformed and de novo DSA are associated with increased risk of rejection and graft loss. We have developed graded induction-desensitization treatment and monitor

Imatinib-induced drug reaction with eosinophilia and systemic symptoms in solid tumors: a patient with dermatofibrosarcoma protuberans and successful desensitization management.

Drug reaction with eosinophilia and systemic symptoms (DRESS) is a severe, potentially life-threatening drug-induced hypersensitivity reaction, characterized by cutaneous eruptions, fever, diffuse lymphadenopathy, along with hypereosinophilia, and elevated liver function tests, which in severe cases may lead to fulminant hepatic failure and death. Although DRESS syndrome has been associated with over 50 different drugs including imatinib, it has never been reported in association with imatinib treatment in solid tumors. We recently treated a patient with metastatic dermatofibrosarcoma protuber

Long-term effect of monophosphoryl lipid A adjuvanted specific immunotherapy in patients with grass pollen allergy.

BACKGROUND: Ultra-short course pollen immunotherapy adjuvanted with monophosphoryl lipid A (MPL) is attractive to conventional allergen-specific immunotherapy (AIT). Long term efficacy of MPL-AIT has not been evaluated. METHODS: 68 patients (age 16.75 ± 5.3 years) with allergic rhinitis to grass pollen were investigated. Group 1: 21 controls; Group 2: 19 after complete AIT, and Group 3: 28 with AIT and treatment cessation: 4 years range 3-6 years ago. RESULTS: The clinical symptoms (running nose, sneezing, conjunctivitis and the weekly overall score) were significantly reduced in patients gr

Successful oral desensitization to i.v. para-aminosalicylic acid (PAS) using encapsulated PAS dry substance.

INTRODUCTION: para-Aminosalicylic acid (PAS) is commonly used in the treatment of drug-resistant tuberculosis, including multidrug-resistant tuberculosis. Since its first use in the 1940s, hypersensitivity reactions frequently limit its use in clinical practice. Cases of successful desensitization against PAS using orally administered ascending doses are described in the literature. CASE REPORT: A 25-year-old patient with severe pulmonary multidrug-resistant tuberculosis developed drug fever with rash, acral cyanosis, and shivering immediately after the intravenous application of PAS. Hard ge
