Laparoscopic esophagojejunostomy using the EndoStitch and a circular stapler under a direct view created by the ENDOCAMELEON.

Laparoscopic esophagojejunostomy using a circular stapler is associated with technical difficulties in the purse-string sutures used to insert the anvil head and in obtaining an adequate visual field to prevent rolling the mesentery and the wall of the jejunum on the mesenteric side into the anastomosis. To overcome these difficulties, we used the EndoStitch to create the purse-string suture and the ENDOCAMELEON to create the visual field to stretch the jejunum. After resecting the esophagus, purse-string sutures were placed using the EndoStitch. A total of five to six needle deliveries were p

A novel technique to relieve a closed-loop obstruction secondary to a competent ileocecal valve and an unresectable mid-colon tumor.

A competent ileocecal valve complicates the surgical treatment of an unresectable obstructing mid-colon tumor. Specifically, it may not be feasible to bypass with a colocolotomy, especially when the sigmoid colon has limited mobility or if the ascending colon is severely distended and edematous. A technique is described in which the closed loop obstruction is relieved at its proximal extent by an ileocecal valvuloplasty. A circular stapling device is fired across the ileocecal valve. Once the ileocecal valve is rendered competent, a loop ileostomy or a colocolotomy can be constructed, providin

[Use of present-day suturing instruments in gastrointestinal surgery].

Since 1993, 272 patients underwent surgery on gastrointestinal tract: 92--for gastric ulcer, 79--for duodenal ulcer, 29--for cancer of the stomach, 67--for cancer of the colon, 5--for other diseases. The main operations were resection of the stomach (195), hemicolectomy (23), abdominal-anal and anterior resection of the rectum (44). In 135 patients group 1 all stages of surgery on gastrointestinal tract were performed with "Auto Suture" instruments (USA). In 137 patients group 2 anastomosis was created by two-layer nodal suture with Russian auto-suture instruments (UKL-60, UO-40) during some s

Open versus minimally invasive small bowel resection for Crohn's disease: a NSQIP retrospective review and analysis.

INTRODUCTION: Many patients with Crohn's Disease will require surgical resection. While many studies have described outcomes following ileocecectomy, few have evaluated surgical resection of other portions of small bowel. We sought to compare open and minimally invasive surgery (MIS) approaches for small bowel resection excluding ileocecectomy of patients with Crohn's Disease using the National Surgical Quality Improvement Program (NSQIP) database. METHODS: The NSQIP database was queried for patients with Crohn's disease or complications related to Crohn's disease who underwent segmental smal

[THE FREQUENCY OF FACTORS CONTRIBUTING TO THE FORMATION OF ADHESIONS IN THE ABDOMINAL CAVITY IN WOMEN].

To determine the major factor that contributed to the formation of adhesions in the abdominal cavity in women with a history of surgical interventions, examined 86 women with adhesive disease (main group) in the past have suffered various surgeries. The average age of patients was 35,7±5,6 years. Clinical examination of patients included a collection of complaints, anamnesis of disease and life, physical examination. The degree of adhesion process in the abdominal cavity was evaluated according to the classification of the American Fertility Society (R-AFS, 1985) and the macroscopic scale prop

One-stage transanal surgical technique for Hirschsprung's disease: is the age of a patient a limiting factor?

Hirschsprung's disease in the adolescent is a rare and often misdiagnosed cause of lifelong refractory constipation. In this particular case, a male patient spent 13 years with an unrecognized illness that was eventually diagnosed as Hirschsprung's disease. The age of the patient is considered a limiting factor due to over-distention and hypertrophy of the rectosigmoid colon that has to be pulled through a muscular cylinder in a limited operative field. This report of a successfully treated teenage boy using a one-stage transanal endorectal approach without conversion to laparotomy, his uneven

Abnormal advanced hemorrhoidal disease: the need of a staged resection.

We present an unusual case of advanced hemorrhoidal disease in an 57-year-old man presenting with anemia, persistent bleeding, anal protrusion. Conventional hemorrhoidectomy was not feasible because of the risk of a massive hemorrhage caused by hypertrophic protruded vascular anal cushions. A two-stage resection was then undertaken. The aim of the first procedure was to reduce the vascularization of the protruded mucosa by a circular series of interrupted mass stitches. This approach was successful and an important reduction of vascularization was obtained. The operation was then completed two

Staging rectourinary fistulas to guide surgical treatment.

PURPOSE: Surgical management for rectourinary fistulas remains a reconstructive challenge. There are few guidelines to direct the surgeon to the most successful and least morbid technique. We developed a rectourinary fistula staging system that allows selection of the most appropriate technique for the patient. We present the details of the staging system and surgical outcomes. MATERIALS AND METHODS: From July 1999 to July 2005 we treated 14 male patients with rectourinary fistula. Mean patient age was 68 years (range 62 to 73). Etiology was rectal injury during open radical prostatectomy in

Laparoscopic transvesical urethrorectal fistula repair: a new technique.

INTRODUCTION: Urethrorectal fistulas are rare, and several open operative approaches and techniques have been used in treatment. We report our preliminary experience with laparoscopic transvesical urethrorectal fistula repair after iatrogenic urethrorectal fistula. A 65-year-old man with insulin-dependent diabetes mellitus and peripheral vascular disease was hospitalized 6 months after transurethral resection of the prostate for benign prostatic hyperplasia with a recurrent urinary tract infection, fecaluria, and urine leakage from the rectum during voiding. Urethrocystoscopy revealed a urethr

Laparoscopically assisted anorectal pull-through for high imperforate anus--a new technique.

BACKGROUND/PURPOSE: This report describes a new technique of laparoscopically assisted anorectal pull-through (LAARP) for repair of high imperforate anus. The procedure utilizes minimal perineal dissection, preservation of the distal rectum, and accurate placement of the rectum within the levator ani and external anal sphincter muscle complex. METHODS: Sharp dissection and cautery was used laparoscopically to expose the rectal pouch down to the urethral or vaginal fistula, which was clipped distally and divided. The pelvic floor musculature was then assessed and the levator sling identified.
