BACKGROUND: Echis coloratus (Burton Carpet viper), a highly venomous snake belonging to the family Viperidae, is responsible for a large proportion of the venomous snakebites in Israel and Palestinian Authority. Procoagulant enzymes are present in Echis coloratus venom and significant coagulopathy is typical after envenomation. Here we report a case of envenomation by Echis coloratus that involved severe coagulopathy as the main systemic manifestation with severe bleeding after fasciotomy.
CASE DETAILS: A 3-year-old boy was admitted to our pediatric critical care unit after a snakebite to his

Disseminated intravascular coagulation (DIC) may complicate a variety of disorders that contribute to mortality, particularly those related to bleeding. It is therefore very difficult to manage DIC in patients with known bleeding disorders. We treated a 62-year-old woman with idiopathic thrombocytopenic purpura (ITP) complicated with sepsis-induced DIC. She had been diagnosed with ITP 8 months prior to admission. Laboratory tests showed an elevation of d-dimer and endotoxin, while pyelonephritis was shown by abdominal computed tomography. Escherichia coli was detected by blood culture. Based o…

A 24-years old female patient with acute renal failure and disseminated intravascular coagulation (DIC) accompanied by acute promielocytic leukemia (APL) is described. The typical for APL features of DIC was dominated by signs of shock, acute renal failure and acute respiratory failure. The absence of blasts in peripheral blood was the reason of diagnostic difficulties and delayed treatment of leukaemia.

BACKGROUND: Sepsis-induced coagulopathy (SIC) was introduced to identify early coagulation abnormalities in sepsis and to predict progression to overt disseminated intravascular coagulation (DIC). However, inclusion of the Sequential Organ Failure Assessment (SOFA) may conflate coagulation-specific biology with overall illness severity.
OBJECTIVES: This study aimed to evaluate whether removal of the SOFA component preserves the diagnostic and prognostic performance of SIC and to determine whether a simplified coagulation-centered construct (SIC-2) maintains clinical utility.
METHODS: In a re…

In patients with sepsis, anticoagulant therapy is expected to have maximal efficacy when administered before the development of sepsis-induced overt disseminated intravascular coagulation (DIC). This therapeutic strategy requires a valid method for real-time, early prediction of sepsis-induced DIC, which is likely to progress to an overt stage. We aimed to develop machine learning prediction models for overt DIC, based on the International Society on Thrombosis and Haemostasis criteria, using a large-scale electronic medical record database. This multi-center, retrospective, observational study…

BackgroundDisseminated intravascular coagulation (DIC) is a complex hemostatic disorder characterized by simultaneous thrombosis and bleeding and is frequently observed in sepsis. Traditional coagulation assays such as prothrombin time (PT) and activated partial thromboplastin time (aPTT) primarily assess the initiation of clot formation but fail to capture the dynamic balance between procoagulant and anticoagulant forces. The thrombin generation (TG) assay provides a more comprehensive evaluation of coagulation, incorporating both propagation and decay phases, and may offer additional insight…

ObjectiveTo evaluate the association between alactic base excess (ABE) and mortality outcomes in patients with sepsis-associated disseminated intravascular coagulation (DIC) within intensive care unit (ICU).MethodsPatients with sepsis-associated DIC were identified using Sepsis-3 criteria and the Japanese Association for Acute Medicine 2 DIC criteria. Multivariable Cox models and restricted cubic splines (RCS) were used to assess the relationship between ABE and 30-day and 90-day ICU mortality, with inflection points derived from a recursive algorithm. The predictive value of adding ABE to the…

Disseminated intravascular coagulation (DIC) is a severe condition characterized by systemic, persistent activation of coagulation in the presence of an underlying disease, leading to the formation of microthrombi in small blood vessels. In DIC, fibrinolysis is also activated alongside coagulation, but the extent of fibrinolysis varies significantly depending on the underlying condition. The classification of DIC types is crucial not only for understanding the pathophysiology involved, but also for selecting appropriate treatment strategies. Internationally, DIC is often associated with sepsis…

BACKGROUND: Disseminated intravascular coagulation (DIC) occurs in 30-50% of septic patients and contributes to high mortality in the intensive care unit (ICU). However, there are few proven interventions for coagulation disorder management in sepsis. Experimental and clinical data have demonstrated that sepsis could benefit from unfractionated heparin (UFH) treatment. To date, there are no large multicenter trials to determine the safety and efficacy of UFH in septic patients with suspected DIC.
METHODS: A multicenter, double-blinded, placebo-controlled randomized trial is designed to recrui…

Disseminated intravascular coagulation (DIC) poses a high mortality risk, yet its exact impact remains contentious. This study investigates DIC's association with mortality in individuals with sepsis, emphasizing multiple organ function. Using data from the Peking University People's Hospital Investigation on Sepsis-Induced Coagulopathy database, we categorized patients into DIC and non-DIC groups based on DIC scores within 24 h of ICU admission (< 5 cutoff). ICU mortality was the main outcome. Initial data comparison preceded logistic regression analysis of mortality factors post-propensity s…

BACKGROUND: Both immune-mediated thrombotic thrombocytopenic purpura (iTTP) and septic disseminated intravascular coagulation (DIC) are life-threatening disorders developed by platelet-consuming microvascular thrombi and necessitate immediate therapeutic interventions. Although severe deficiencies of plasma haptoglobin in iTTP and factor XIII (FXIII) activity in septic DIC have been reported, few studies have focused on the possibility of using these markers to distinguish between iTTP and septic DIC.
OBJECTIVES: We investigated whether the plasma levels of haptoglobin and FXIII activity coul…

Hematologic/Oncologic emergencies are rarely seen in the critical care transport environment but must be recognized and treated without delay. We report such a patient transported from a referring hospital to a tertiary care center by the LifePACT team, a 52-year-old male with a history of acute myeloid leukemia (AML). The patient presented to the referring hospital with known laboratory test abnormalities, suffered cardiac arrest, was resuscitated, accepted for transfer to a tertiary care center, and LifePACT was requested to perform the transport.

BACKGROUND: Coagulation abnormalities can accompany acute congestive heart failure (CHF). However, disseminated intravascular coagulation (DIC) is rarely documented in such patients. DIC is characterized by generalized excessive activation of coagulation pathways followed by their depletion with secondary activation of anticoagulation and fibrinolysis. Treatment of the cause is an integral part of management of DIC; thus, recognition of the cause is critical.
CASE REPORT: A 55-year-old previously healthy man presented with breathlessness, swelling of both legs, and left leg pain. His physical…

A 62-year-old man had a history of acute aortic dissection (Stanford type A) and had been diagnosed with polycystic kidney disease three years earlier, and then developed end-stage renal failure. He was referred with chief complaints of difficult hemostasis and consecutive hemorrhagic episodes at the puncture site of the shunt soon after dialysis introduction. We suspected chronic disseminated intravascular coagulation (DIC) due to mild thrombocytopenia and a fibrinolytic system abnormality. Plasma factor XIII activity was decreased, but no inhibitor was detected. In addition, contrast-enhance…
