OBJECTIVE: In a previous publication, the authors documented the prevalence and characteristics of head-shaking nystagmus (HSN) in over 1300 consecutive dizzy patients during formal electronystagmography (ENG) at the combined Otologic Function Unit (OFU) of the Toronto and Mount Sinai Hospitals. This companion study was done to prospectively analyze a subgroup of 300 patients with known clinical diagnoses belonging to the senior author (J.R.).
METHODS: The prevalence of HSN in this series of patients with a known diagnosis is prospectively compared to the presence of HSN in a series of normal

ObjectiveWe introduce two novel scales to measure two important aspects of motion sickness, dizziness and eye strain.BackgroundThe Fast Motion Sickness Scale (FMS) has become a valuable tool to assess motion sickness whenever quick unobtrusive measures need to be taken. It is well-anchored and provides scalable, quantitative data. However, it does not differentiate among symptoms of motion sickness.MethodWe discuss the benefits and shortcomings of the FMS as well as other, symptom-based rating scales. We present a taxonomy to assess symptom clusters and illustrate the trade-off between fast gl…

BACKGROUND AND OBJECTIVES: Persistent postural-perceptual dizziness (PPPD) is characterized by maladaptive central sensory processing and frequent psychiatric comorbidity. Prospective outcome data, particularly from low- and middle-income settings, remain limited. This study evaluated clinical characteristics, psychiatric comorbidity, objective balance findings, and treatment response in patients with PPPD, with emphasis on neurologist-delivered diagnostic explanation within a multimodal treatment framework.
METHODS: Seventy-five consecutive patients fulfilling Bárány Society criteria for PPP…

Persistent postural-perceptual dizziness (PPPD) is a disabling functional vestibular disorder characterized by chronic dizziness and visually and motion-induced unsteadiness that markedly impairs daily activities, yet it lacks objective neurobiological markers. We acquired resting-state functional MRI (rs-fMRI) in 52 patients with PPPD and 50 age- and sex-matched healthy controls (HCs) and analyzed the data using a three-tier approach: (i) functional network connectivity (FNC) of independent component analysis (ICA-FNC), (ii) voxel-wise measures of spontaneous amplitude of low-frequency fluctu…

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a chronic functional vestibular disorder characterized by non-spinning dizziness, postural instability, and hypersensitivity to visual and motion stimuli. While Bárány Society criteria enable diagnosis, the prognostic value of neuro-otological examinations remains unclear. To assess the relationship between neuro-otological test results and short-term clinical improvement in PPPD, clarifying their clinical significance.
METHODS: A retrospective review was conducted of 24 patients with PPPD identified from 103 patients with intract…

BACKGROUND: Distinguishing ischemic stroke in patients presenting with isolated dizziness or vertigo (IDV) from more benign causes remains a significant clinical challenge. Current understanding of the specific clinical and imaging characteristics that differentiate IDV strokes from strokes with non-isolated symptoms (NIDV) is incomplete.
OBJECTIVE: This study aimed to systematically compare the clinical characteristics, with a specific focus on infarction topography and lipid profiles, between patients with acute cerebral infarction presenting with IDV and those with NIDV.
METHODS: In this …

RATIONALE: Spinocerebellar ataxia type 3 (SCA3) is the most common autosomal dominant inherited spinocerebellar disorder. The disease can easily be misdiagnosed because the symptoms of SCA3 are diverse and not obvious in the early stages.
PATIENT CONCERNS: A 55-year-old woman had experienced dizziness and an unstable gait since 2017. She was misdiagnosed with persistent postural-perceptual dizziness at another hospital because of normal brain magnetic resonance imaging and emotional abnormalities. In our hospital, there were no abnormalities in routine laboratory tests and brain magnetic reso…

BackgroundPatients with persistent postural-perceptual dizziness (PPPD) suffer from symptoms of chronic dizziness that exacerbate in a moving environment. However, not much is known regarding gaze behavior in a static condition. The present study investigated gaze stability under static visual stimuli in patients with PPPD.MethodsThree groups of participants were recruited; 22 PPPD, 9 Ménière's disease (MD) and 19 healthy participants (HC). Participants were asked to undergo a free viewing test of emotionally neutral images through a virtual reality headset with a gaze-recording system. Bivari…

Apart from the legs, restless legs syndrome (RLS) also affects the arms, head, neck, face, oral cavity, genital area, abdomen, intestines and bladder. RLS is also linked to several comorbid diseases, including headache disorders. Its association with dizziness has never been explored. We are reporting on two patients with RLS who also had a history of chronic dizziness. The treatment with levodopa or dopamine agonists completely alleviated both RLS and dizziness. We propose that RLS-like symptoms in the head may be experienced as dizziness and that dizziness may be part of the symptom complex …

BACKGROUND: Previous cohort studies have suggested an association between cerebral small vessel disease (cSVD) and "unexplained dizziness". The causality of this link remains uncertain, but it would be of significant clinical importance, considering the substantial number of patients presenting with unexplained dizziness is large. We aimed to investigate the causal effect of cSVD-related phenotypes on unexplained dizziness using a Mendelian randomization approach.
METHODS: Genetic instruments for each cSVD-related phenotype - white matter hyperintensity (WMH) volume, lacunar stroke (LS), peri…

Acute dizziness and vertigo are common emergency department presentations (≈4% of annual visits) and sometimes, a life-threatening diagnosis like stroke is missed. Recent literature reviews the challenges in evaluation of these symptoms and offers guidelines for diagnostic approaches. Strong evidence indicates that when well-trained providers perform a high-quality bedside neurovestibular examination, accurate diagnosis of peripheral vestibular disorders and stroke increases. However, it is less clear who can and should be performing these assessments on a routine basis. This article offers a …

INTRODUCTION: Postimplantation dizziness is common, affecting approximately 50% of patients. Theories for dizziness include utricular inflammation, endolymphatic hydrops, and loss of perilymph. Four-point impedance (4PI) is a novel impedance measurement in cochlear implantation that shows potential to predict hearing loss, inflammation, and fibrotic tissue response. Here, we associate 4PI with dizziness after implantation and explore the link with utricular function.
METHODS: Subjective visual vertical (SVV) as a measure of utricular function was recorded preoperatively as a baseline. 4PI was…

One in three older people (>60 years) complain of dizziness which often remains unexplained despite specialist assessment. We investigated if dizziness was associated with vascular injury to white matter tracts relevant to balance or vestibular self-motion perception in sporadic cerebral small vessel disease (age-related microangiopathy). We prospectively recruited 38 vestibular clinic patients with idiopathic (unexplained) dizziness and 36 age-matched asymptomatic controls who underwent clinical, cognitive, balance, gait and vestibular assessments, and structural and diffusion brain MRI. Pati…

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a functional disorder, typically preceded by acute vestibular disorders. It is characterized by a shift in processing spatial orientation information, to favor visual over vestibular and somatosensory inputs, and a failure of higher cortical mechanisms. To date, no therapies for PPPD have been approved. Kampo medicine hangebyakujutsutemmato (HBT) has been reported to alleviate disturbances of equilibrium. We hypothesized that HBT would be a beneficial treatment for PPPD.
AIM: To examine the efficacy of HBT for the treatment of PPP…

Persistent postural perceptual dizziness (PPPD) is a common chronic vestibular disorder characterized by persistent vestibular symptoms, including postural instability and non-spinning vertigo, which is aggravated by motion, upright posture and moving or complex visual stimuli. In our review of 198 cases seen over a 5 year period, we have confirmed a number of common precipitating conditions for PPPD, including anxiety disorders and vestibular migraine. Vestibular abnormalities, including a unilateral loss of vestibular hypofunction and isolated otolith abnormalities, were found on investigati…

Sporadic endolymphatic sac tumor is a very rare neoplasm. It is low malignant, locally destructive and expansive, but non-metastasizing. The tumor is very rare in the sporadic form, but more often associated with Von Hippel-Lindau disease. A 65-year old man with left sided tinnitus and hearing loss for several months. Audiometry showed an asymmetrical sensory neural hearing loss on the left side up to 60 dB. The speech discrimination score was 46% and stapedial reflexes were absent. Several years earlier, he had suffered from periods of dizziness. Magnetic resonance imaging (MRI) showed a dest…

The clinical syndrome of supine hypertension associated with orthostatic hypotension (OH) in given individuals is recognized by specialists, but is underdiagnosed in the community. The objective of this study was to assess supine hypertension associated with hypotensive reactions on head-up tilt (SH-HRT) among patients evaluated for nonspecific dizziness. Consecutive patients with nonspecific dizziness were studied with a 10-min supine 30-min head-up tilt test. Supine hypertension (SH) was diagnosed when supine systolic blood pressure (SBP) was > or = 140 mmHg and/or supine diastolic blood pre…

When a patient complains of dizziness, the term may cover several types of sensations. When the physician carefully takes the history, he or she will most often be able to differentiate between vertigo and other types of dizziness, including a feeling of lightness or fainting. Based on this differentiation, the Dutch College of General Practitioners (NHG) designed a practice guideline for its members. The most important causes of vertigo are paroxysmal benign positional vertigo, vestibular neuritis, Ménière's disease and TIA or stroke in the vertebrobasilar system. Differentiation between vest…
