A 56-year-old female, who underwent aortocoronary bypass graft for occlusion of anterior descending artery is reported, who postoperatively developed a harsh systolic murmur, mitral regurgitation, and intraventricular systolic pressure gradient suggestive of hypertrophic subaortic stenosis. The above findings were due to the administration of dobutamine hydrochloride for hypotension in association with afterload reduction (intra-aortic balloon pumping) and disappeared almost immediately after left ventriculography (volume load).

We report the case of an 80-year-old man with a preexistent right bundle branch block (RBBB) who underwent dobutamine stress echocardiography. As the sinus cycle length gradually decreased during the stress test, he first developed a 2:1 RBBB pattern alternating with beats showing normal conduction and then an alternating 2:1 less complete and more complete RBBB pattern. An elucidation of the potential mechanism of the latter very rare phenomenon is provided.

Middle aortic syndrome (MAS) is a rare disorder characterized by narrowing of the aorta. Depending on the location and extent of aortic narrowing, surgical management may be necessary to prevent long-term sequelae, such as stroke and congestive heart failure. We report a case of MAS and discuss the unique intraoperative anesthetic considerations including hemodynamic monitoring proximal and distal to the aortic narrowing, minimizing the risk of spinal cord ischemia, and utilization of intraoperative dobutamine stress testing to guide surgical management.

Concomitant occurrence of pulmonary embolism and right ventricular infarction is rare. It poses important diagnostic and therapeutic implications. A case of pulmonary embolism with isolated right ventricular anterior wall infarction presented with ventricular tachycardia. One pathology could have led to the other. Two-dimensional echocardiography was useful in documenting pulmonary artery hypertension as well as regional wall motion abnormality of the right ventricle. Thrombolytic therapy and dobutamine infusion were useful. Nitrates, fluid infusion and diuretics should be used cautiously.

We report perioperative management of two patients with severe dilated cardiomyopathy belonging to the group IV of classification of Inoh (ejection fraction 18% and 30%).
