[Multi-resistant pathogens - are they also resistant to disinfectants?].

Epidemiological studies show that the care of patients in rooms with a previous stay by a person with evidence of multi-resistant pathogens (MRP) is associated with an increased risk of these pathogens occurring. The question therefore regularly arises as to whether MRP also exhibit resistance to the disinfectants used. To date, there are no standardised definitions for "resistance" to disinfectants. However, disinfectants authorised on the market are also effective against multi-resistant pathogens and the failure of efficient disinfection is mainly caused by application errors (insufficient

Phenothiazines as anti-cancer agents: SAR overview and synthetic strategies.

Cancer is a leading cause of death worldwide and there are still limited options for cure. Chemotherapy is the most significant treatment for cancer which increased survival rates, despite this, it is associated with numerous side effects, as well as cancer relapsing due to drug resistance insurgence; consequently, it is still a challenging task to develop new potent and less toxic anti-cancer agents for patients' care. Phenothiazine moiety, which leads a class of well-known antipsychotic drugs, possesses a wide range of biological activities and has been also introduced in cancer chemotherapy

Nanozyme-natural enzymes cascade catalyze cholesterol consumption and reverse cancer multidrug resistance.

Multidrug resistance is still a major obstacle to cancer treatment. The most studies are to inhibit the activity of the drug transporter P-glycoprotein (P-gp), but the effect is not ideal. Herein, a nanosystem was built based on cascade catalytic consumption of cholesterol. Cholesterol oxidase (natural enzyme, COD) was immobilized on the carrier (NH2-MIL-88B, MOF) through amide reaction, COD catalyzed the consumption of cholesterol, the reaction product H2O2 was further produced by the MOF with its peroxidase-like activity to produce hydroxyl radicals (•OH) with killing effect. Due to the high

Efficient side-chain deacylation of polymyxin B1 in recombinant Streptomyces strains.

OBJECTIVES: Polymyxins are antibacterial polypeptides used as "last resort" therapy option for multidrug-resistant Gram-negative bacteria. The expansion of polymyxin-resistant infections has inspired development of novel polymyxin derivatives, and deacylation is one of the critical steps in generating those antibiotics. Deacylase from Actinoplanes utahensis hydrolyze the acyl moieties of echinocandins, and also efficiently deacylates daptomycin, ramoplanin and other important antibiotics. Here, deacylase was studied considering its potential usefulness in deacylating polymyxin B1. RESULTS: Al

Saponins and their derivatives: Potential candidates to alleviate anthracycline-induced cardiotoxicity and multidrug resistance.

Anthracyclines (ANTs) continue to play an irreplaceable role in oncology treatment. However, the clinical application of ANTs has been limited. In the first place, ANTs can cause dose-dependent cardiotoxicity such as arrhythmia, cardiomyopathy, and congestive heart failure. In the second place, the development of multidrug resistance (MDR) leads to their chemotherapeutic failure. Oncology cardiologists are urgently searching for agents that can both protect the heart and reverse MDR without compromising the antitumor effects of ANTs. Based on in vivo and in vitro data, we found that natural co

Draft genome sequence of carbapenem-resistant Elizabethkingia anophelis strain BP8467 clinical isolate from India.

OBJECTIVES: Elizabethkingia spp. are Gram-negative, glucose-non-fermenting bacilli that are ubiquitous in natural environments such as soil, plant and water sources. Besides environmental sources, the bacterium can be found in hospital environments, particularly medical equipment and reagents. Here we report the draft genome sequence of an Elizabethkingia anophelis isolate from a blood culture. METHODS: Genomic DNA of E. anophelis strain BP8467 was sequenced on an Ion Torrent PGM platform and the reads were assembled de novo using SPAdes v.5.0.0. The draft genome was annotated using the Proka

The use of carbapenems in the treatment of serious infections.

Inadequate initial antimicrobial treatment in serious infections leads to increased mortality. Achieving adequate treatment is increasingly difficult because of the increasing prevalence of multidrug-resistant (MDR) pathogens. The carbapenems are potent, broad-spectrum antibiotics that have been shown to be safe and efficacious therapies in the treatment of serious infections. This review is intended to compare the 4 major members of the carbapenem class, which include imipenem, meropenem, ertapenem, and doripenem, with other widely used antimicrobial agents in the intensive care unit (ICU). Assessment of safety of the major antimalarial drugs.

Antimalarial drugs remain the major intervention tool for the global malaria control efforts that save millions of lives. Nonetheless, emergence and spread of Plasmodium parasites resistant against chloroquine and other major antimalarial drugs has brought the urgency to develop a new generation of safe and effective drugs against malaria. In this article, the safety data for major antimalarial drugs is reviewed. Although an ample amount of clinical data regarding the safety and tolerability of several of these drugs in older children and adults is available, more critical safety and tolerabil

Drug-resistant salmonellosis.

After the development of plasmid-mediated resistance against first-line drugs, the emergence of chromosomally-mediated quinolone resistance in typhoid fever has become a major concern. Quinolone insensitivity and treatment failures with ciprofloxacin are being increasingly reported. However, third generation cephalosporins have come as a saviour. To overcome fluoroquinolone insensitivity, higher doses with longer duration of treatment is recommended. Moreover, return of chloramphenicol and furazolidone sensitivity shown by different studies is a welcome observation.

Shigella species from Addis Ababa: frequency of isolation and in vitro drug sensitivity.

One hundred and five shigella isolates from Addis Ababa were studied to determine serogroup frequency and in vitro antibacterial drug sensitivity. About 70% of the isolates were Shigella flexneri followed by Sh. dysenteriae (15%), Sh. boydii (10%) and Sh. sonnei (5%). All or most of the strains were susceptible to cephalothin, gentamicin, kanamycin, polymyxin B and trimethoprim-sulphamethoxazole. Frequencies of susceptibility to ampicillin, carbenicillin and chloramphenicol were, respectively, 79, 80 and 75%. Only 37, 23 and 58% were susceptible to streptomycin, sulphadiazine and tetracycline,

Ultrasound-Driven Coassembly of Anticancer Drugs into Carrier-Free Particles.

The evolution of drug resistance in tumor malignancies has necessitated advancements in anticancer drug therapy. Drug combination therapy, which can burden cancer progression at multiple target sites, has been used to address drug resistance and includes the coencapsulation of synergistic drugs within nanoparticle carriers. However, the use of organic and inorganic carriers can lead to additional material-induced safety concerns, including inflammation and antibody formation. Herein, we report an ultrasound-driven approach to combine synergistic anticancer drugs into carrier-free particles. Ve

Optimal management of chronic hepatitis B patients with treatment failure and antiviral drug resistance.

The management of treatment failure in patients with chronic hepatitis B, remains a clinical concern. Incomplete viral suppression and the emergence of drug resistance are key determinants of treatment failure. The correct choice of a potent first-line therapy to achieve sustained long-term suppression of viral replication provides the best chance of preventing treatment failure and drug resistance. Clinical studies have demonstrated that drugs with a high barrier to resistance have significantly lower rates of resistance compared with those with a low barrier to resistance. Management of trea

The Concept of an Ideal Antibiotic: Implications for Drug Design.

The emergence and spread of antibiotic-resistant pathogens is a major public health issue, which requires global action of an intersectoral nature. Multidrug-resistant (MDR) pathogens-especially "ESKAPE" bacteria-can withstand lethal doses of antibiotics with various chemical structures and mechanisms of action. Pharmaceutical companies are increasingly turning away from participating in the development of new antibiotics, due to the regulatory environment and the financial risks. There is an urgent need for innovation in antibiotic research, as classical discovery platforms (e.g., mining soil

Antifolate drug resistance: Novel mutations and haplotype distribution in dhps and dhfr from Northeast India.

Malaria is a major public health concern in Northeast India with a preponderance of drug-resistant strains. Until recently the partner drug for artemisinin combination therapy (ACT) was sulphadoxine pyrimethamine (SP). Antifolate drug resistance has been associated with the mutations at dihydropteroate synthase (dhps) and dihydrofolatereductase (dhfr) genes. This study investigated antifolate drug resistance at the molecular level. A total of 249 fever cases from Arunachal Pradesh, NE India, were screened for malaria, and of these, 75 were found to be positive for Plasmodium falciparum. Sample

XDR-TB, what is it; how is it treated; and why is therapeutic failure so high?

Drug resistance is a major hurdle in the global battle against tuberculosis (TB). In tackling this problem it is important to understand both how resistance emerges as well as the ways in which multi-drug and extensively drug-resistant TB (M/XDR-TB) may be successfully treated. The biggest problem with treatment is the lack of effective drugs. Exciting developments have been made in this area over recent years with useful drugs being found from older agents as well as new discoveries. Patent applications are being made for novel agents and are also possible as new use for older agents such as

Phenothiazines as anti-multi-drug resistant tubercular agents.

Pulmonary tuberculosis (TB) has again become a global problem: it infects 2.2 billion people world-wide, caused the deaths of over 3 million last year and will produce over 8 million new cases of TB this coming year. Although effective therapy is widely available for antibiotic susceptible strains of Mycobacterium tuberculosis, current drugs are relatively useless against multi-drug resistant infections (MDRTB). Mortality is almost complete within two years regardless of therapy, and in the case of co-infection with HIV/AIDS, mortality is 100% within a few months of diagnosis especially the M.
