Biliopancreatic diversion with duodenal switch.

The biliopancreatic diversion with a duodenal switch (BPD-DS) is a less commonly performed but very effective bariatric procedure that has been in existence for more than 20 years. It is particularly effective for the resolution of diabetes and is associated with the highest weight loss among other bariatric operations. Typically, the BPD-DS is not associated with postgastrectomy symptoms, such as dumping and marginal ulceration. Because of its complexity, it has usually been performed by laparotomy in the past; but, more recently, minimally invasive techniques are being used with acceptable r

Minilaparotomy for perforated duodenal ulcer.

The usefulness of the minilaparotomy approach for perforated duodenal ulcer repair was retrospectively evaluated in 37 patients (26 men; mean age, 56.5 years). Simple closure with an omental patch by minilaparotomy (skin incision, < or = 7 cm) was successful in 86.5% of the cases, with an operative mortality of 2.7%. Compared with the results in historic control patients who underwent conventional open surgery (n = 27), a shorter operative time (P < 0.01), lower frequency of analgesic use (P = 0.03), earlier passage of flatus (P < 0.01), and shorter hospital stay (P = 0.04) were obtained in th

Duodenal gastrointestinal stromal tumor.

BACKGROUND: Gastrointestinal stromal tumors (GISTs) are low-grade malignant tumors that may arise anywhere in the alimentary tract, and in the past, most of them were diagnosed as intestinal leiomyoma or leiomyosarcoma. GISTs of the duodenum make up only 4.5% of all GISTs. MATERIALS AND METHODS: We report a patient presented with upper GI bleeding who exhibited radiological findings of a tumor in the third portion of the duodenum. The patient underwent a segmental duodenectomy. Pathological examination concluded in a GIST. RESULTS: The patient is free of recurrence for one year after surgery

A case of gastric and duodenal strongyloidiasis.

Strongyloides stercoralis is a nematode parasite which causes a protracted asymptomatic intestinal infection. It is considered a life threatening condition in immunocompromised patients when hyperinfection is associated with disseminated disease. The diagnosis by routine stool examination is very limited since the larval output in stools is very low. We present the case of a 52 year-old Omani man from Salalah, in the southern region of Oman, with a 15-year history of type 2 diabetes mellitus and recently discovered to have hairy cell leukaemia, who complained of nausea, abdominal pain, loss of

Duodenal heterotopic pancreas in a child.

BACKGROUND: Heterotopic pancreas is characterized by pancreatic tissue outside the pancreatic bed. However, duodenal heterotopic pancreas in children is rarely reported so far. We describe herein duodenal heterotopic pancreas in a child who suffered from chronic abdominal pain. METHODS: An 8-year-old boy presented with upper abdominal pain and intermittent vomiting, without a history of melena, hematochezia, hematemesis, clay-colored stools, jaundice, hepatitis or dyscrasia. No specific medication or change in position relieved the pain. Based on the elevated serum amylase levels, and the fin

An unusual bleeding duodenal ulcer.

Childhood socioeconomic status (SES) often is associated with physical health even decades later. However, parent-child emotional bonds during childhood may modify the importance of childhood SES to emergent health inequalities across the life course. Drawing on national data on middle-aged adults (1995 and 2005 National Survey of Midlife Development in the United States; MIDUS; Ns = 2,746 and 1,632), I find that compromised parent-child bonds eliminate the association between childhood SES and midlife disease. Longitudinal models of incident disease across one decade show that childhood abuse

Duodenal angiomyolipoma: a case report.

The authors present a case of duodenal angiomyolipoma. Angiomyolipoma is a benign neoplasm, the most common site being the kidney. Extrarenal angiomyolipomas are rare. Only one case of duodenal angiomyolipoma has been reported. A 66 year old man presented with GI bleeding for two years. Endoscopy revealed a duodenal polyp which on histopathology revealed features of angiomyolipoma. On immunohistochemistry, smooth muscle component was positive for Smooth Muscle Actin. HMB-45 showed mild focal positivity. Patient had no history of tuberous sclerosis. Surgical excision of angiomyolipoma is usuall

[Oral cholangiography and duodenal atresia].

A newborn male patient with trisomy-21 presented with bilious hemesis. The patient was icteric with slight hepatomegaly. Simple abdominal X-ray and upper gastrointestinal series with barium showed a dilated duodenal loop and inflammatory changes involving the duodenal mucosa. This image known as "double bubble" is characteristic of congenital duodenal obstruction. Simultaneously the gallbladder and choledochus were visualized. The former X-ray finding is very unusual. An uneventful Kimura procedure was performed.

Histological value of duodenal biopsies.

This study was performed to see the value of histopathological diagnosis in management of patients with duodenal biopsies; to look for correlation of histology and serology in suspected cases of coeliac disease; the reasons for taking duodenal biopsies and whether proper adequate histories are provided on the forms sent with request for histopathological view on duodenal biopsies. Here are the observations of the study followed by the discussion.

Strongyloidiasis presenting as duodenal obstruction.

Small bowel obstruction due to Strongyloides stercoralis is rare especially in immunocompetent individuals. We report a case of a 45-year-old man who presented with a history of severe abdominal pain, intermittent low-grade fever and vomiting. An upper GI endoscopy revealed a diffusely edematous second portion of duodenum and narrowing in the third part of duodenum with food residue in the stomach. Barium contrast upper-GI radiography revealed partial small-bowel obstruction. Duodenal biopsy specimens revealed Strongyloides stercoralis in the submucosa with inflammatory infiltrate. The patient
