We report a case of duodenal carcinoma with continuous bleeding that was successfully treated with transcatheter arterial embolization using gelatin sponge particles. The case was a woman in her 70's who had a curative surgical resection for sigmoid colon cancer with liver and lung metastases, hepatic arterial infusion chemotherapy and radiofrequency ablation for liver metastasis in the past. We report a case of chronic gastric volvulus in which ultrasonography (US) was useful. An 81-year-old woman was hospitalized due to vomiting, and upper gastroduodenoscopy revealed that the stomach was spirally twisted and constricted. BACKGROUND: Penetration into the liver is a rare complication of peptic ulcer. Usually the diagnosis is made by operation or autopsy. We found only 13 reports, in which hepatic penetration was confirmed by endoscopy and histopathological examination of the gastroscopic biopsy. Large ulcers with a pseudotumoral mass protruding from the ulcer bed and the presence of liver tissue in the biopsy specimen were the main findings.

Therapeutic endoscopy is followed by complications in less than 5% of cases. We report a case of an intramural duodenal hematoma after local endoscopic injection of 28 mL of adrenaline 1/10,000 for a bleeding duodenal peptic ulcer. This hematoma was associated with acute pancreatitis and was revealed by a hemoperitoneum.

A 31-year-old man presented with abdominal pain and iron deficiency anaemia due to gastrointestinal blood loss. Endoscopic retrograde cholangiopancreatography (ERCP) revealed a choledochocele, located between the ampullary sphincter and the sphincters of the common bile duct and pancreatic duct. The choledochocele was removed surgically and appeared to be covered with duodenal mucosa. Gastrointestinal blood loss is explained by the extensive erosions found in the duodenal mucosa of the choledochocele. Choledochoceles should be treated by radical resection.

It has become increasingly recognized that nonsteroidal anti-inflammatory drug (NSAID) use is associated with serious gastroduodenal mucosal injury that can eventuate in upper gastrointestinal bleeding or perforation. Upper endoscopic and gastric acid output studies were done in 51 patients with homozygous sickle cell (SS) disease and recurrent epigastric pain. Twenty (39%) had abnormalities in the upper gastrointestinal tract including 18 (35%) with peptic ulcers. Primary carcinoma of the duodenum is an uncommon tumor of the gastrointestinal tract. We reviewed the histories of ten patients seen between 1976 and 1986. Some of the patients with tumors in the second part of the duodenum presented with clinically evident jaundice. The prevalence of hyperamylasemia 2 hr and 24 hr after upper gastrointestinal endoscopy without attempts to cannulate the ampulla of Vater was prospectively studied in 50 consecutive patients. In the 2-hr sample, hyperamylasemia was observed in nine patients (18%) (serum amylase range 59-191 units/liter with a mean value of 102 units/liter; reference range: 12-46 units/liter).
