Brain and inner-ear fluid homeostasis, cochleovestibular-type tinnitus, and secondary endolymphatic hydrops.

Secondary endolymphatic hydrops (SEH) has clinically been found to have a significant incidence of occurrence in patients with subjective idiopathic tinnitus (SIT) of a severe disabling type. The diagnosis is made clinically and has been established by integration in a medical audiological tinnitus patient protocol of the clinical history with results of electrodiagnostic cochleovestibular testing that fulfill the diagnostic criteria of inner-ear disease consistent with Ménière's disease. SEH is hypothesized to be a factor, not an etiology, influencing the clinical course of SIT. Alterations o

Inner ear drug delivery via a reciprocating perfusion system in the guinea pig.

Rapid progress in understanding the molecular mechanisms associated with cochlear and auditory nerve degenerative processes offers hope for the development of gene-transfer and molecular approaches to treat these diseases in patients. For therapies based on these discoveries to become clinically useful, it will be necessary to develop safe and reliable mechanisms for the delivery of drugs into the inner ear, bypassing the blood-labyrinthine barrier. Toward the goal of developing an inner ear perfusion device for human use, a reciprocating microfluidic system that allows perfusion of drugs into

Hush puppy: a new mouse mutant with pinna, ossicle, and inner ear defects.

OBJECTIVES/HYPOTHESIS: Deafness can be associated with abnormalities of the pinna, ossicles, and cochlea. The authors studied a newly generated mouse mutant with pinna defects and asked whether these defects are associated with peripheral auditory or facial skeletal abnormalities, or both. Furthermore, the authors investigated where the mutation responsible for these defects was located in the mouse genome. METHODS: The hearing of hush puppy mutants was assessed by Preyer reflex and electrophysiological measurement. The morphological features of their middle and inner ears were investigated b

Conductive hearing loss: investigation of possible inner ear origin in three cases studies.

This is a report of three cases of mixed hearing loss that resulted from inner ear disorders. Two cases were unilateral and the third was bilateral. The diagnosis was based on the findings of normal middle and external ears in association with the absence of round window reflexes. The contralateral stapedial reflex was present in the two unilateral cases. This is the first documentation of conductive deafness due to inner ear abnormality. This diagnosis should be considered in cases of conductive hearing loss if the middle and external ears are normal. More studies are needed to establish the

Monitoring inner ear pressure changes in normal guinea pigs induced by the Meniett20.

The inner ear fluid pressure of guinea pigs was measured during a series of complex oscillating middle ear pressure changes induced by the Meniett20 (Pascal Medical, Sweden), a possible therapeutic pressure generator to be used by patients with Meniere's disease. Middle ear pressure changes were transferred instantly to the inner ear, although inner ear pressure declined while middle ear pressure stayed relatively stable. An average undershoot of -1.0 cm H2O with respect to the steady-state pressure was seen after application of a pressure pulse, which was released in a few seconds. The result

Bacterial meningitis in adults: demonstration of inner ear involvement using high-resolution MRI.

OBJECTIVE: To visualize the sites involved in audiovestibular dysfunction during bacterial meningitis in adults and to relate these findings to the extent of hearing impairment and vestibular dysfunction. BACKGROUND: Hearing impairment is among the most frequent complications of bacterial meningitis. METHODS: High-resolution MRI (HR-MRI) of the inner ear was performed in seven adult patients with hearing loss as a complication of bacterial meningitis. RESULTS: Five patients had unilateral (n = 1) or bilateral (n = 4) contrast enhancement of vestibulocochlear structures. The structures most

Fistula of stapes footplate caused by pulsatile cerebrospinal fluid in inner ear malformation.

Congenital malformations of the inner ear are well described, though the combination with cerebrospinal fluid (CSF) leaks remains controversial. In this paper a case of a bilateral Mondini malformation with a CSF otorrhea on one side is reported. The malformed stapes contains a perforation in the middle of the footplate and associated thinning analogous to a pothole in a mountain stream. The histological findings support the hypothesis of pulsatile flow of CSF as origin of the perforation of the footplate.

Visualization of inner ear structures by three-dimensional high-resolution magnetic resonance imaging.

High-resolution computed tomography (CT) has long been the method of choice in the visualization of the petrous bone, the internal auditory canal, and the cerebellopontine angle. The introduction of magnetic resonance imaging (MRI), especially of the three-dimensional Fourier transformation constructive interference in steady state (3DFT-CISS), has proved to be superior in the detection of soft-tissue lesions in the inner ear. The aim of this study was to visualize small anatomic structures of the inner ear and cerebellopontine angle. The examinations were performed with a standard head coil o

Endolymphatic system shunting: a long-term profile of the Denver Inner Ear Shunt.

Endolymphatic system surgery for Meniere's disease, particularly endolymphatic shunting, remains controversial. In 1988, we presented our findings on the efficacy of the Denver Inner Ear Shunt in 100 patients. These data were accumulated in the short term. The purpose of this article is to review the long-term results of our endolymphatic shunt procedure, highlighting the population of Denver Inner Ear Shunt recipients. Results were analyzed according to both 1972 and 1985 AAO-HNS criteria. We seek corroboration or refutation of our preliminary conclusion that (a) endolymphatic shunt surgery h

[Sudden vertigo--differential diagnosis and therapy in peripheral vestibular (inner ear-induced) forms].

The most important forms of inner ear disorders manifesting as sudden vertigo may be distinguished by the duration of the vertiginous spells: sudden vertigo, diminishing during days = acute peripheral vestibulopathy, recurrent spells lasting for hours = Ménière's disease, recurrent vertigo lasting for minutes = "vascular', recurrent positioning-induced vertigo lasting for seconds = benign paroxysmal positioning vertigo (BPPV). In the last years, major progress was made in the treatment of BPPV. Based on new ideas on the underlying pathomechanism, this most common form of vestibular vertigo now

Inner ear defect similar to Alport's syndrome in the glomerulosclerosis mouse model Mpv17.

The Mpv17 mouse strain is a recessive transgenic mouse mutant that develops glomerulosclerosis and nephrotic syndrome at a young age. The phenotype results from a loss of function of a gene coding for a hydrophobic peroxisomal protein of 176 amino acids of 20 kDa following its destruction by retroviral integration. To investigate a potential effect of the missing Mpv17 function on the inner ear light and electron microscopic investigations were performed on the inner ears of Mpv17 mice and controls. These revealed degeneration of the stria vascularis and spiral ligament, loss of cochlear neuro

Anatomical correlates of functional recovery in the avian inner ear following aminoglycoside ototoxicity.

Tucci and Rubel have demonstrated functional recovery of the chick cochlea following aminoglycoside ototoxicity. The cochleae of these same animals were examined by scanning electron microscopy (SEM) in order to further understand this recovery process. Hatchling chicks were given daily doses of gentamicin for 10 days. Auditory-evoked potential measurements and examination of the cochlea by scanning electron microscopy were performed after survival periods of 5 days to 20 weeks. After 5 days of gentamicin exposure, there was near complete basal hair cell loss associated with a high-frequency h

[The development of endogenous inner ear hearing loss due to occupational noise exposure].

Pure tone audiograms of 80 workmen with preexisting endogenous cochlear hearing loss were investigated over an average of 16 years (range 10 to 26) of occupational exposure to noise. Further hearing deterioration was moderate in the majority of cases more severe hearing losses more often affected the low tone range. The pure tone threshold 0.25 to 8 kHz remained unaltered in at least one ear in 24% of cases, in the high tone range 3 to 6 kHz in 51%. The average hearing loss was least at 3 kHz, followed by 4 kHz. The average hearing loss was somewhat greater in the initially better hearing ear.

Applications of scanning electron microscopy and X-ray microanalysis in inner ear pathology.

Surface pathology of inner ear structures so far described in detail concern cochlear and vestibular hair cells and the stria vascularis. In man, surgical intervention into the inner ear is very uncommon and when performed is in general with the primary objective of destroying the diseased peripheral end organs. The vast majority of inner ear tissue available for use with scanning electron microscopy (SEM) is therefore obtained from animals. The present paper reviews the progression of surface pathology caused by aminoglycoside antibiotics, acoustic overstimulation and in a guinea pig strain w

[Treatment of acute inner ear deafness (sudden deafness and acoustic trauma) (author's transl)].

The principal facts concerning therapeutic measures in cases of sudden deafness and acoustic trauma are outlined. Experiment data concerning the pharmacology of inner ear circulation are presented. There is sufficient evidence for a critical evaluation of a few treatments whose effect seems to be proven (experimental increase of inner ear circulation in normal animals) or at least possible (in normal animals no or dubious increase; effective under pathological conditions only?). Experimental and clinical experiences concerning the dextran treatment of sudden deafness and acoustic trauma are re

[Expert consensus on diagnosis and treatment of refractory inner ear disease (2026 edition)].

Refractory inner ear disease (RIED) encompasses a group of diseases in which pathologic lesions involve both the cochlea and vestibule, disrupting auditory-vestibular conduction pathways and their central projections. Clinically, RIED typically manifests as varying combinations of hearing loss, tinnitus, and vertigo. Despite adherence to standardized treatment protocols recommended by current clinical guidelines, a subset of inner ear diseases remain resistant to therapy and fail to achieve satisfactory outcomes. Recent epidemiological studies from both domestic and international sources indic

The inner ear immune microenvironment in sensorineural hearing loss: mechanisms and therapeutic perspectives.

Sensorineural hearing loss (SNHL) affects over 1.5 billion individuals worldwide and remains the most prevalent sensory deficit in humans. Although classical pathological models have emphasized hair cell death from oxidative stress and mitochondrial dysfunction, accumulating evidence over the past decade has redefined the cochlea as an immunologically active tissue rather than an immune-privileged organ. Resident macrophages populate the spiral ligament, stria vascularis, spiral ganglion, and modiolus, and they orchestrate both protective and damaging responses to acoustic, ototoxic, metabolic

Inner Ear Organoids: A Hydrogel-Based Platform for Drug Screening and Deafness Modeling.

This review highlights advances in inner ear organoids (IEOs) as a novel platform for drug screening and disease modeling, particularly for hearing loss. IEOs, derived from embryonic stem cells, induced pluripotent stem cells, or tissue-specific progenitors, provide a physiologically relevant alternative to traditional animal models. Significant progress has been made in utilizing various cell sources, extracellular matrix materials such as Matrigel and hydrogels, and methods for controlling microenvironments through biochemical and biophysical signals. Applications of IEOs in drug screening,
