The effect of pronation and supination on the minimally displaced scaphoid fracture.

The amount of rotation that occurs at the scaphoid waist fracture site with pronation and supination of the forearm is studied in 10 upper extremities from cadavers. Two colinear metal markers were placed in the osteotomized scaphoid and a below-the-elbow cast was applied. Spiral volumetric computed tomography scanning of the scaphoid was done with multiplanar reformation to evaluate displacement of the metal markers. Four of the 10 specimens also were studied without any immobilization. The total magnitude of motion from pronation to supination averaged 0.2 mm in the specimens with a below-th

Anteromedial coronoid facet fractures.

The anteromedial coronoid facet and the medial lip of trochlea represent one of the most important stabilizing columns of the elbow to prevent posterior dislocation of the elbow. But on average, 58 % of the anteromedial facet extends from the proximal ulnar without sufficient support by the proximal ulnar metaphysic. Some important soft tissue structures insert on the coronoid process. The fracture of anteromedial coronoid facet was recognized recently in clinic as a distinct type of the coronoid fracture. The special injury mechanism is varus posteromedial rotational injury force. This mechan

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

Isolated avulsion fracture of the coronoid process requiring open reduction in a paediatric patient: a case report.

Coronoid process fractures are reported to occur from avulsion by the brachialis muscle or to be associated with elbow dislocations. We report a rare case of coronoid process fracture due to avulsion by the anterior bundle of the medial collateral ligament rendering the elbow unstable. In children, small fracture fragments of the coronoid process (types 1 & 2) are in reality often much larger but the actual size is not appreciated radiographically, as the coronoid process contains considerable amounts of cartilage. If the fragment is seen to be significantly displaced it may have resulted from

Natural history of unreduced Gartland type-II supracondylar fractures of the humerus in children: a two to thirteen-year follow-up study.

BACKGROUND: The preferred treatment of type-II supracondylar humeral fractures remains controversial. The purpose of this study was to evaluate the long-term clinical and radiographic outcome of type-II supracondylar humeral fractures in children treated with immobilization in a splint without reduction. METHODS: The medical records of forty-six consecutive patients who sustained a supracondylar Gartland type-II fracture of the humerus treated with immobilization in a splint were reviewed. Age at the time of fracture, sex, side involved, dominant extremity, duration of immobilization, and com

Injuries to the Coronoid Process of the Ulna with Involvement of the Lesser Sigmoid Notch.

INTRODUCTION: This study aimed to describe the involvement of the lesser sigmoid notch in fractures to the coronoid process. We hypothesized that injuries to the lateral aspect of the coronoid process regularly involve the annular ligament insertion at the anterior lesser sigmoid notch. MATERIAL AND METHODS: Patients treated for a coronoid process fracture at our institution between 06/2011 and 07/2018 were included. We excluded patients < 18 years, patients with arthritic changes or previous operative treatment to the elbow, and patients with concomitant injuries to the proximal ulna. In pat

Nonoperative manipulative reduction with Chinese herbs for the treatment of a displaced olecranon fracture: A case report.

RATIONALE: Displaced olecranon fracture is a common injury following a fall or direct trauma to the elbow. There have been no reports of patients with a displaced olecranon fracture who have only received nonoperative manipulative reduction with Chinese herbs. PATIENT CONCERNS: The patient was a 64-year-old woman with a complex elbow injury that occurred in a traffic accident. The patient complained of severe, painful limitation of motion on straightening or bending. DIAGNOSES: The patient was diagnosed with a displaced fracture of the left olecranon (type IIA olecranon fracture according to

Postoperative Contralateral Facet Cyst after Endoscopic Unilateral Laminectomy for Bilateral Decompression: A Case Report.

INTRODUCTION: The terrible triad of the elbow (TTE) is a well-described entity characterized by a triad of elbow dislocation, fractures of the radial head, and coronoid. Historically, these fractures gave poor clinical outcomes, and hence, the name "terrible triad." There are many variants of TTE that may lead to poor prognosis if not adequately dealt with. One such variant is the association of TTE with olecranon fracture, which has not been adequately described in the literature. There have been just three papers (one case series and two case reports) on this variant of TTE, and the prognosi

Correction of cubitus valgus and reconstruction of lateral humerus condylar defect using tricortical iliac graft in pediatric patients.

BACKGROUND: Neglected non-united lateral humeral condyle fractures in pediatrics are a probable cause of cubitus valgus deformity which is a disabling complication. The ideal management for this condition is still debatable. This study aimed to evaluate the reconstruction of a non-united lateral humerus condylar fracture complicated by cubitus valgus using a tricortical iliac crest graft in pediatric patients. PATIENTS AND METHODS: Twenty children suffering from cubitus valgus as a complication after a non-united fracture of the lateral humeral condyle were included in this study. They were m

Type II Supracondylar Humerus Fractures: To Fix or Not To Fix?

Extension-type supracondylar humerus (SCH) fractures are among the most common pediatric elbow fractures. Treatment algorithms for Gartland type I and type III fractures have been well established, yet managing type II SCH fractures has remained a topic of debate in the pediatric orthopaedic community. It is important to explore the controversies surrounding both the classification and treatment of type II SCH fractures. Radiographically, these fractures are difficult to reliably classify to guide decision making. In addition, the decision to pursue closed reduction and casting versus closed (…

Osteomyelitis variolosa: a report of two cases.

We present 2 patients with osteomyelitis variolosa in both elbow joints--sequelae of smallpox. The condyles were elongated and the central portions of the distal humeri were absorbed and the elbow joints were unstable. One of the patients sustained a closed fracture of the distal humerus. The fracture united uneventfully following stabilisation and bone grafting. At the 13-year follow-up, the patient had satisfactory elbow function.

Results of operative treatment for ulnar neuropathy in patients with a history of fracture of the lateral part of the humeral condyle in childhood.

BACKGROUND: Fractures of the lateral part of the humeral condyle in childhood very often go undetected and are treated incorrectly, giving rise to numerous late complications. The study evaluated the outcomes of treatment of ulnar neuropathy secondary to a fracture of the lateral part of the humeral condyle in childhood. MATERIAL AND METHODS: The study enrolled 12 patients aged 28-58 who had suffered fractures of the elbow at between 2 and 14 years of age. Arthrosis and a valgus deformity of the joint, contractures in flexion and extension of the elbow as well as ulnar neuropathy were observe

Olecranon fractures in adults: factors influencing outcome.

In a retrospective, non-randomized study, we reviewed our experience with the operative treatment of olecranon fractures to find out which factors correlate with subjective complaints and functional outcome. A total of 95 consecutive olecranon fractures were operatively treated in our level I trauma institution between 1 March 1992 and 1 March 2000. Medical records and X-rays of all patients were evaluated. Three patients died, 23 were lost to follow-up. Fifty-eight of 69 available patients (84.0%) were reviewed clinically and/or radiologically after a mean time of 36 months. Fracture type, fr

Arthroscopically-assisted reduction and pinning of a radial neck fracture in a child: a case report and review of the literature.

BACKGROUND: Displaced radial neck fractures in children are challenging to treat. The age of the patient and the degree of angulation are the main criteria to consider when managing these fractures in children. Various surgical techniques have been described in the literature for both indirect and direct reduction and for fixation. However, the best treatment is still debated. CASE PRESENTATION: The case presented is of a 6-year-old Caucasian boy with an impacted and displaced radial neck fracture. With the patient in lateral position, under general anesthesia, elbow arthroscopy was performed

Traumatic Elbow Instability: How to Fix It and When to Phone a Friend.

Traumatic elbow instability is a difficult condition to manage. Many surgeons consider the elbow a perplexing black box and evaluating damaged structures in the setting of pain and swelling a diagnostic challenge. Injury most commonly occurs from a fall onto an outstretched hand but also results from elbow dislocation, direct trauma, and sporting activities. The injury can initiate on the medial or lateral side, depending on forearm position at the time of injury, and usually follows a predictable pattern, with progressive instability caused by soft-tissue disruption and fractures of the media
