Electroconvulsive therapy (ECT) is the most effective treatment for several particularly severe or pharmacotherapy-resistant psychiatric disorders, and a growing number of studies have investigated factors influencing the effectiveness of ECT. The objective of this article is to review the current evidence on clinical and biological markers potentially related to response to ECT and to outline perspectives for future research. In depressive disorders, the presence of clinical characteristics such as higher age, psychotic and psychomotor symptoms, and the absence of comorbid personality disorde…

OBJECTIVE: We aimed to synthesize the evidence on confusional states and neurocognitive changes associated with the concurrent use of lithium and electroconvulsive therapy (ECT).
METHODS: We conducted a systematic search of PubMed, EMBASE, Web of Science, Scopus, Cochrane Library, CINAHL, PsycArticles, and clinical trials registries up to September 2025. Eligible studies compared the incidence of confusional states or changes in neurocognitive test scores in patients receiving lithium and ECT (Li-ECT) and those receiving ECT without lithium. We performed separate meta-analyses and calculated …

Electroconvulsive therapy (ECT) is a well-established treatment for severe psychiatric disorders, and its role in modern-day psychiatry continues to expand with rising rates of complex mental illnesses. ECT has proven to be an effective treatment for conditions such as treatment-resistant depression, catatonia, bipolar disorder, schizophrenia, and other serious mental health disorders. Individuals who are resistant to or have contraindications against conventional pharmacologic therapies can particularly benefit from ECT. The procedure involves inducing a brief and controlled generalized seizu…

INTRODUCTION: Catatonia is a neuropsychiatric syndrome characterized by motor and behavioral abnormalities, often associated with severe psychiatric or medical conditions. The use of electroconvulsive therapy (ECT) is a well-established treatment for catatonia; however, paradoxical onset during treatment is rare.
CASE PRESENTATION: A 33-year-old woman with schizoaffective disorder, previously managed with antipsychotics, developed acute catatonia following the first ECT session. Her history included childhood epilepsy controlled with sodium valproate and long-term use of various antipsychotic…

The effective treatment of major depressive disorder remains one of the biggest public health challenges globally. For moderate to severe cases, pharmacotherapy often falls short, leading to treatment-resistant depression. Electroconvulsive therapy (ECT) has generally been considered the gold standard for severe cases of treatment-resistant depression. However, emerging evidence suggests that ketamine may serve as a promising alternative. Two relatively large noninferiority trials and three meta-analyses support the efficacy of both treatments but report contradictory findings regarding superi…

Electroconvulsive therapy (ECT) is a highly effective biological treatment in psychiatry with a history of nearly one hundred years. In contemporary clinical practice, its primary indications include treatment-resistant affec tive disorders, catatonia, and selected psychotic conditions. Although the overall use of ECT has declined internationally, with appropriate infrastructure and organization it remains a safe and effective intervention and can be delivered in an outpatient setting. The aim of this paper is to present the organizational, technical, and clinical changes implemented at the De…

Electroconvulsive therapy (ECT) is a recognized treatment option for patients with schizophrenia, especially when patients do not respond adequately to antipsychotic medication or when rapid response is needed in severe cases. The 3 common electrode placements, namely, bitemporal, bifrontal and right unilateral (RUL) modalities, have all been described by various studies to be efficacious in symptom reduction. However, the optimal electrode placement with the greatest success rate in the treatment of schizophrenia has yet to be ascertained. Furthermore, the benefit of switching ECT modalities …

Electroconvulsive therapy (ECT) is a neuromodulation procedure that uses short electrical pulses causing a brief generalized seizure to treat depression, mania, schizophrenia, and catatonia. Achieving sufficient seizure length is crucial for maintaining therapeutic efficacy, as it influences treatment response. However, over successive sessions, seizure length becomes shorter requiring an increased energy set to generate a sufficient seizure, potentially causing more side effects. Caffeine augmentation offers a promising tool by enhancing neuronal excitability via adenosine receptor modulation…

INTRODUCTION: Electroconvulsive therapy (ECT) is one the most effective interventions for treatment-resistant depression. There is a link between trauma history and the development of depression, and frequent comorbidity with post-traumatic stress disorder (PTSD) and personality disorders. The impact of comorbid trauma and personality disorders on the efficacy of ECT has attracted research interest, with the possibility raised for using ECT to target other core symptoms of these disorders.
AREAS COVERED: In this scoping review, the authors describe the available evidence on the use of ECT in …

Electroconvulsive therapy (ECT) is an established treatment for various mental disorders. However, ECT can induce significant haemodynamic fluctuations, raising safety concerns. This systematic review and meta-analysis aimed to evaluate the effects of adding beta-blockers to ECT on maximum heart rate after stimulation and the occurrence of adverse events. Only randomised controlled trials (RCTs) that compared the addition of beta-blockers to ECT with treatment as usual (TAU), such as placebo added to ECT or ECT without antihypertensive or antiarrhythmic medications, were included. The primary …

Pseudocholinesterase deficiency (PD) is a genetic or acquired condition that impairs the metabolism of succinylcholine, leading to unpredictable and potentially prolonged paralysis. Its recognition during electroconvulsive therapy (ECT) is particularly difficult because postictal physiology, psychotropic polypharmacy, and the frequent absence of quantitative neuromuscular monitoring may make residual paralysis difficult to detect. We report a 34-year-old woman with pharmacotherapy-resistant schizophrenia who developed consistently prolonged paralysis after succinylcholine during ECT. In the fi…

Electroconvulsive therapy (ECT) is an effective treatment for various psychiatric disorders such as depression, bipolar disorder, and schizophrenia but is associated with certain side effects. Epistaxis following ECT appeared to be rare, with few reports in the medical literature. We reported two cases of epistaxis following ECT. In one case, we observed a sudden transient increase in blood pressure during ECT, followed by epistaxis. In the other case, the patient developed epistaxis after ECT, with subsequent ECT sessions leading to increasingly difficult-to-control nosebleeds. Nasal endoscop…

OBJECTIVE: Electroconvulsive therapy (ECT) is one of the most studied and validated available treatments for severe or treatment-resistant depression. However, little is known about the neural mechanisms underlying ECT. This systematic review aims to critically review all structural magnetic resonance imaging studies investigating longitudinal cortical thickness (CT) changes after ECT in patients with unipolar or bipolar depression.
METHODS: We performed a search on PubMed, Medline, and Embase to identify all available studies published before April 20, 2023. A total of 10 studies were includ…

This review aimed to investigate the effects of electroconvulsive therapy (ECT) on akathisia. We systematically searched MEDLINE and Embase and included case reports on akathisia caused by or treated with ECT. We identified and included 9 articles. Seven articles, representing 10 cases, reported that akathisia improved after ECT. Most cases (7 cases) were diagnosed with a mood disorder, and all 10 cases were treated with antipsychotics. Akathisia was regarded as tardive (3 cases) and severe (4 cases). Although treatments for akathisia, such as adjusting antipsychotics and adding medication for…

BACKGROUND: Electroconvulsive therapy (ECT) is highly efficacious in catatonia yet remains underutilized in pediatric patients. Practice guidelines recommend bilateral placement in cases with urgent need for response such as catatonia. Because of significantly lower cognitive burden and efficacy (compared to bilateral), right unilateral placement (RUL) is preferred for major depression. Increasing literature shows RUL is effective for catatonia in adults, but its use in catatonic youth is largely unknown.
OBJECTIVES: The aims of the study are to describe naturalistic outcomes of ECT in pediat…

Electroconvulsive therapy is a type of therapy frequently used in psychiatric clinical practice. Although it is generally used for the treatment of affective disorders or severe and/or resistant psychotic disorders, it has also demonstrated its usefulness in many other neuropsychiatric conditions. We present the case of a 26-year-old woman, previously diagnosed with a functional neurological disorder-with fixed dystonia phenotype in the right upper limb-, admitted to our hospital for a severe depressive episode. After noting the absence of clinical improvement with psychopharmacological treatm…

BACKGROUND: Schizophrenia is a kind of intractable brain disorder. Electroconvulsive therapy (ECT) has been used to rapidly improve the clinical symptoms of patients with schizophrenia, but the effect of ECT on topological attributes of brain functional network in patients with schizophrenia has not been clear. The purpose of this study was to investigate the brain functional network mechanism of ECT against schizophrenia.
METHODS: Thirty-one patients with schizophrenia and fifty healthy controls matching age, gender, and years of education were included. All participants underwent general da…
