BACKGROUND: Nightmares are common in posttraumatic stress disorder (PTSD), but they also frequently occur in idiopathic form. Findings associated with sleep disturbances in these two groups have been inconsistent, and sparse for idiopathic nightmares. The aim of the present study was to investigate whether sleep anomalies in PTSD sufferers with frequent nightmares (P-NM) differ from those observed in non-PTSD, idiopathic nightmare (I-NM) sufferers and healthy individuals.
METHODS: Sleep measures were obtained from nine P-NM sufferers, 11 I-NM sufferers, and 13 healthy control subjects. All pa

OBJECTIVE: To detect the endolymphatic hydrops by using the otolithic nature in patients with Meniere's disease and delayed hydrops.
METHODS: We developed a new technique for measuring horizontal eye movements, corresponding to lateral head drop in total darkness, approximately 1g environment. Normal subjects (n=16, mean age: 27.3 years old) and patients (seven for pre-operative state, four for furosemide test at pre-operative state; two for post-operative state) were employed in the test. The principle of the test is to drop the head in a lateral position separately from the body with a rapi

OBJECTIVES To report our experience over the past 12 years with the evaluation and follow-up of pilots with vestibular neuronitis and to discuss points relevant to flight safety and the resumption of flying duties. STUDY DESIGN A retrospective, consecutive case series.METHODS Eighteen military pilots with vestibular neuronitis were examined and followed up. A complete otoneurological workup was performed, including both physical examination and laboratory evaluation. The latter included electro-oculography (EOG) and a rotatory chair test using the smooth harmonic acceleration protocol. RESULTS

A case-control design was used to assess eye-head-trunk coordination for community-dwelling elders performing a stand-walk task. Eighteen elders with a high risk of falling were matched to 18 subjects with low risk on the basis of age, living status (living alone or with someone) and category of residence (independent living or assisted living facility). Standard electro-oculography was used in conjunction with an electromagnetic tracking device to measure eye and head-trunk motion, respectively. For the low-risk group, the mean slope of gaze-head and gaze-trunk relationships was significantly

The vestibulo-ocular reflex stabilizes gaze during head movements by producing compensatory eye movements. Retinal image velocity (RIV) is defined as the difference between the eye and head velocities. The RIV of 20 vestibular schwannoma (VS) patients and 17 healthy controls was measured with a head autorotation test. The head autorotation test had a sensitivity of 80% and a specificity of 88%. The mean RIV (degree/second) +/- 95% confidence intervals of the VS patients in the 5 frequency bands of 1 to 5 Hz was respectively 4.8 (4.2 to 5.5), 11.5 (8.6 to 14.4), 21.7 (15.5 to 27.9), 25.2 (17.1

OBJECTIVE: To investigate the morphology, scalp topography and temporal relationship with orbicularis oculi muscle contraction of bilateral blink related spikes (BRS) in a 7-year-old boy with chromosomopathy, mild mental retardation and left spontaneous centrotemporal spikes (SS).
METHODS: The patient underwent video-polygraphic recordings with off-line analysis of SS and BRS by means of spike-averaging and orbicularis oculi contraction-locked averaging techniques respectively. EEG activity related to reflex blinking (evoked by glabellar tapping) was also studied.
RESULTS: SS and BRS present

Three theories have been suggested as to the cause of space motion sickness: 1) eye and vestibular sensory mismatch, 2) abnormal shift of body fluids producing increased intracranial pressure and 3) pre-warning signals for unpleasant physical situations by self-produced neurotoxic substances released in the body. We are interested in the possible functional disabilities/incongruities of eye, head and body movements in 0-G. Space motion sickness might be explained from the viewpoint of lack of coordination of the movements of the eye and head. It is important to ascertain the significance of gr

The P300 and regional cerebral blood flow were measured before and after intravenous injection of L-dopa in 10 patients with idiopathic Parkinson's disease and 10 patients with vascular parkinsonism. The P300 was measured with an evoked potential recorder using an oddball paradigm and the regional cerebral blood flow was measured using the stable xenon computed tomography method. The P300 latency was significantly longer and the regional cerebral blood flow in the cerebral cortex and basal ganglia was significantly lower in the Parkinson's disease group and the vascular parkinsonism group than

We describe a 49-year-old man with chronic cluster headache unresponsive to all medications. Following investigation in the sleep lab he was found to have obstructive sleep apnea (OSA) with associated oxygen desaturations during rapid eye movement (REM) sleep. He awakened during one of these episodes with a typical headache. Treatment with nasal CPAP abolished his OSA and desaturations, and largely abolished his headaches. He then developed central apneas during REM sleep. Further treatment with BiPAP, with a set backup rate, abolished both the apneas and the headaches. We conclude that there

Two middle-aged patients with motor neuron disease showed common eye movement disorders and intellectual impairment in the later stage of the illness. Eye movement disorders were characterized by slow saccades and vertical glaze palsy, which seemed to be supranuclear ophthalmoplegia. Neuropathological examination of one patient revealed degeneration of the substantia nigra without pathological changes in the ocular motor nuclei, in addition to findings compatible with motor neuron disease. These cases appear to raise a possibility of a distinct group of multiple system degenerations.

400 sleep-apnoeic patients with an intact autonomic nervous system underwent twenty-four-hour electrocardiography with simultaneous polygraphy recording at night. At onset of sleep apnoea all showed progressive bradycardia, followed by abrupt tachycardia on resumption of breathing. The electrocardiographic pattern, which is identifiable by computer analysis, can be used as a screening tool for sleep apnoea; it was not seen in controls without sleep apnoea syndrome. A subgroup of patients with sleep apnoea and impairment of autonomic nervous control of the heart (heart transplants, autonomic ne

During the period from January 1973 to June 1975, 60 cases with definite Meniere's disease and 30 cases with sudden deafness were listed according to such criteria for the diagnosis of both diseases as determined by the Research Committee of Japan. A neurootological assessment for both diseases were performed in our series. In patients with definite Meniere's disease, hearing impairment was characterized by deficiency in detecting low- and middle-pitched tones and fluctuation of auditory threshold in low frequency range. In sudden deafness, highly impaired hearing loss, i.e., total deafness, w

The writers analyze 29 nyctohemeral recordings in 20 patients with sleep apneas. In addition to the standard polygraphy, use is made of pneumotachography and capnography. The results (central apnea with open or secondarily closed glottis; obstructive apnea with closed glottis or with expiratory valve phenomenon) raise the hypothesis of a central physiopathological mechanism common to the various types of apneas through a more or less dissociated inhibition of the activity of the respiratory, laryngeal, intercostal, and diaphragmatic muscles:--in central apnea: inhibition of the thoracoabdomina

Two non-obese patients suffering from hypersomnia with periodic breathing, who presented predominantly obstructive apnoea persisting throughout sleep, underwent tracheotomy with placement of a permanent tracheal cannula. Before tracheostomy there was a serious state of alveolar hypoventilation with a marked rise in pulmonary arterial pressure which appeared during sleep. After tracheostomy breathing during sleep became regular, pulmonary arterial pressure became normal, and daytime drowsiness disappeared. These observations suggest that the obstruction of the upper air tracts during sleep repr

BACKGROUND: Panic disorder (PD) has a strong genetic component showing high heritability rates and familial aggregation. Moreover, there is evidence for associations between parental PD and patterns of psychopathology. So far, little is known about possible endophenotypes representing premorbid vulnerability markers in high-risk subjects for PD. In the present study, we investigated saccadic eye movement (SEM) as an index of CNS inhibitory function in subjects at high risk for PD.
METHODS: 132 healthy children at high and low familial risk for PD were included in the study. Basal SEM paramete
