BACKGROUND: A right sided aortic arch (RAA) and an aortic diverticulum (AD) are a rare congenital anomaly associated with an aortic rupture and dissection. Recently, various methods for the surgical management have been described to treat RAAs and ADs.
CASE REPORT: We describe a case of an RAA with a mirror image branching (RAMI) and AD in a teenager who complained of chest pain. We planned a total arch replacement with the frozen elephant trunk technique. Further, we needed to devise a way to reconnect the cerebral vessels because of his small arch anatomy and limited space in the mediastinu

OBJECTIVES: The aim of this study was to evaluate the outcomes of the frozen elephant trunk (FET) technique, using the J Graft FROZENIX for Stanford type A acute aortic dissection, in comparison with the unfrozen elephant trunk technique.
METHODS: Between January 2010 and August 2018, we performed total arch replacement for Stanford type A acute aortic dissection in our hospital. Thirty patients were treated by the elephant trunk procedure (ET group), and 20 patients were treated by the FET procedure (FET group). To evaluate aortic remodelling, we measured the area of the aorta, the true lume

BACKGROUND: Retrograde type A dissection (RTAD) is a serious complication after ascending aorta involved endovascular repair (AAIER). We here report our surgical approach to this serious complication.
METHODS: From July 2011 to July 2014, 8 RTADs after AAIER patients received surgical repair in our institution. Data of these RTAD patients were retrospectively collected for further analysis. All patients received urgent surgical repair based on the stented elephant trunk technique. We took 2 different ways to handle the previous stent during operation. In patients who had a prior hybrid aortic

Novel endovascular techniques hope to offer patients aortic arch repair with reduced morbidity compared to conventional arch surgery; however, current endovascular strategies remain challenged by the proximal seal zone, higher stroke rates, long-term durability and select anatomy. Hybrid arch repair offers patients a less invasive alternative that can treat more distal aorta than conventional arch repair yet still be performed via standard sternotomy. Areas covered: This review will discuss the current evidence and future development of hybrid aortic arch and frozen elephant trunk reconstructi

PURPOSE: The purpose of this study was to evaluate the long-term results of partial arch repair using the frozen elephant trunk (FET) technique for distal arch aortic aneurysm and to examine the late complications.
METHODS: Thirty-eight patients with true distal arch aortic aneurysms were repaired with FET introduced through an incision in the proximal arch aorta. Follow-up computed tomography was performed every 6 or 12 months in 36 surviving patients. The maximum dimension of the excluded aneurysmal space was measured to determine whether the aneurysmal space had decreased or disappeared. L

BACKGROUND: Conventional open total arch replacement is the treatment of choice for surgical aortic arch pathologies. However, it is a two-stage procedure related to high cumulative and interval mortality rates. Hybrid type III aortic arch reconstruction, the so-called "frozen elephant trunk" is a one-stage alternative approach.
METHODS: A meta-analysis and detailed review of the literature published from January 2013 until December 2016, concerning frozen elephant trunk hybrid approach was conducted and data for morbidity and mortality rates were extracted.
RESULTS: Among 989 patients inclu

Total aortic arch replacement using the frozen elephant trunk (FET) procedure has widely spread all over the world with more sophisticated devices. However, spinal cord injury is one of the most important complications, which limits the use of FET. In our hospital, 80 patients treated with FET had no spinal cord injury. There are three key points to avoid spinal cord injury, (1) The distal end of stent graft should be positioned up to the eighth thoracic vertebrae level; (2) the time for selective left subclavian artery perfusion and distal body circulatory arrest should be shortened within 60

OBJECTIVE: The use of the frozen elephant trunk technique for type A aortic dissection in Marfan syndrome is limited by the lack of imaging evidence for long-term aortic remodeling. We seek to evaluate the changes of the distal aorta and late outcomes after frozen elephant trunk and total arch replacement for type A aortic dissection in patients with Marfan syndrome.
METHODS: Between 2003 and 2015, we performed frozen elephant trunk + total arch replacement for 172 patients with Marfan syndrome suffering from type A aortic dissection (94 acute; 78 chronic). Mean age was 34.6 ± 9.3 years, and

BACKGROUND: In patients with type A dissection, residual dissection and new distal entry tears following the frozen elephant trunk (FET) procedure adversely affect long-term prognosis. Management include open and endovascular repair, while clinical experience is limited. We evaluate the efficacy of thoracic endovascular aortic repair (TEVAR) in management of residual intimal tear or distal new entry tear following FET in patients with type A aortic dissection (TAAD).
METHODS: Between May 2003 and April 2013, we performed FET and total arch replacement for 1,003 patients with TAAD. Among these

Pinnipeds are a diverse clade of semi-aquatic mammals, which act as key indicators of ecosystem health. Their transition from land to marine environments provides a complex microbial milieu, making them vulnerable to both aquatic and terrestrial pathogens, thereby contributing to pinniped population decline. Indeed, viral pathogens such as influenza A virus and phocine distemper virus (PDV) have been identified as the cause of several of these mass mortality events. Furthermore, bacterial infection with mammalian Brucella sp. and methicillin-resistant Staphylococcus aureus strains have also be

OBJECTIVE: Complete or partial supra-aortic debranching, after thoracic endovascular aortic repair, is appealing treatment for complicated acute type B dissection (ABAD) with aortic arch involvement (AAI) because of reduced surgical trauma. However, unsatisfactory outcomes have been reported. We retrospectively reviewed our experience of left subclavian artery (LSCA) transposition with stented elephant trunk (SET) implantation for complicated ABAD with distal AAI.
METHODS: From April 2011 to December 2014, 19 patients (all men; mean age: 44 years) who had complicated ABAD and distal AAI under

Interspecies transmission of influenza A is an important factor in the evolution and ecology of influenza viruses. Marine mammals are in contact with a number of influenza reservoirs, including aquatic birds and humans, and this may facilitate transmission among avian and mammalian hosts. Virus isolation, whole genome sequencing, and hemagluttination inhibition assay confirmed that exposure to pandemic H1N1 influenza virus occurred among free-ranging Northern Elephant Seals (Mirounga angustirostris) in 2010. Nasal swabs were collected from 42 adult female seals in April 2010, just after the an

Optimal management of the aortic arch during repair of acute type A aortic dissection remains debated. Hemiarch repair can achieve acceptable early survival, but persistent false lumen perfusion and residual distal dissection may contribute to later distal aortic enlargement or reintervention in a subset of patients. The frozen elephant trunk (FET) technique aims to improve distal aortic remodeling by covering intimal tears, expanding the true lumen, and promoting false lumen thrombosis, but it adds operative complexity because it requires more extensive arch reconstruction and supra-aortic ve

BACKGROUND: In acute type A aortic dissection (AADA) patients, total arch replacement (TAR) using the frozen elephant trunk (FET) technique has been adopted in emergencies to promote distal false lumen (FL) thrombosis. However, TAR is technically demanding and is associated with prolonged operative time and increased intraoperative blood transfusion. We previously reported the branched stented anastomosis frozen elephant trunk repair (B-SAFER) technique. Comparative data versus conventional TAR (cTAR) adjusted for disease severity remain limited. Here we compared the early surgical outcomes of

BACKGROUND: The frozen elephant trunk (FET) procedure has emerged as an effective single-stage treatment for complex aortic pathologies. However, it carries a risk of distal stent graft-induced new entry (dSINE) in patients with chronic aortic dissection (CAD). This study investigated risk factors associated with dSINE development.
METHODS: Between 2009 and 2021, 160 FET procedures were performed, including 48 cases of CAD (mean time from onset: 5.6 ± 3.8 years). After excluding five patients due to incomplete 6-month postoperative computed tomography (CT) data, 43 patients were included. A m

BACKGROUND: The favorable long-term outcomes have been reported for total arch replacement using the frozen elephant trunk technique for acute aortic dissection. However, the data on chronic degenerative aortic aneurysms treated with this technique are limited compared with those for acute aortic dissection, and the long-term outcomes are not well understood. Here, we report early and long-term outcomes of total arch replacement using the frozen elephant trunk technique in patients with chronic aortic aneurysms.
METHODS: We included 37 patients with chronic degenerative aortic aneurysms who u
