Infective endocarditis in hypertrophic cardiomyopathy--mural and aortic valve vegetations: a case report.

Hypertrophic cardiomyopathy (HCM) is infrequently complicated by infective endocarditis (IE). The authors report the case of a 46-year-old woman developing IE in asymptomatic HCM. Blood cultures were positive for Streptococcus viridans. Echocardiography demonstrated: 1) a mobile (1.2 x 1 cm2) vegetation attached to the septal endocardium at the site of contact between the mitral valve leaflet and the hypertrophic septum; 2) two large (2.7 x 1.7 cm2 and 1.6 x 1.1 cm2) vegetations at NCC and RCC respectively of aortic valve, causing moderate valve regurgitation, and, 3) a mural (1 x 0.8 cm2) veg…

[Papillary fibroelastoma. A rare etiology of strokes in young patients].

BACKGROUND: The papillary fibroelastomas are cardiac lesions, which typically occur on the cardiac valves, but rarely on the endocardium. The incidence of these benign primitive tumors varies from 0.002 to 0.33% and increases with advancing age. METHODS: We report two cases of stroke, one in a 31-year-old man and the other in a 48-year-old woman, both admitted to the same stroke center. RESULTS: The diagnostic studies were normal in these two patients, except for the echocardiography. The first showed an echogenic mass on the mitral valve on transthoracic echocardiography (TTE), confirmed by…

Surgical repair of postinfarction ventricular septal defect by infarct exclusion.

Repair of postinfarction ventricular septal defect (VSD) by infarct exclusion has been used for a decade in our unit. It involves securing a glutaraldehyde-preserved bovine pericardial patch to the endocardium of the left ventricle all around the necrotic myocardium to exclude the VSD and the infarct from the left ventricular cavity. Fifty-two patients with postinfarction VSD underwent this type of repair from 1987 to 1996. Thirty-four patients were in shock when undergoing operation; 26 had anterior VSD and 26 had posterior. Ten patients died perioperatively, for surgical mortality of 19%. Th…

[Surgical treatment for tetralogy of Fallot with pulmonary atresia right ventricular outflow tract reconstruction with autologous pulmonary tissue and concomitant extensive pulmonary angioplasty].

Eight consecutive patients with tetralogy of Fallot associated with pulmonary atresia successfully underwent corrective surgery without using an extracardiac conduit at a mean age of 2 years 4 months. Each of these patients had undergone a classical or modified Blalock-Taussig shunt at a mean age of 55 days. In 6 cases, central PA was fully mobilized and pulled forward. An autologous flap was made from the rudimentary PA trunk or right PA, then it was anastomosed to the endocardium of RV infundibulum as a posterior wall of the outflow tract. In 7 cases, concomitant extensive pulmonary angiopla…

[Healing of operative cardiac wounds].

Healing process of operative cardiac wounds was studied histopathologically in 14 patients who died 28 hours to 5 years after the operation … Granulation tissue rarely developed at the septum. Adhesion between sutured edges of atrial septal endocardium could not be observed and the dead space there remained over a period of 7 months after the operation. These findings indicated the continuous presence of unexpected weakness of the sutured septum until sufficient thickening of neointima had develop …

Flexible Percutaneous Portal Becomes Firm to Facilitate Endocardial and Intravascular Surgical Procedures.

OBJECTIVE: Percutaneous endocardial cardiac surgical procedures are hindered by limited stability and control within the heart. A novel technology, which enables transcatheter devices to shift between flexible and firm states, creates a stable endovascular base to precisely access and intervene upon high-value cardiac structures. METHODS: A uniquely designed endocardial system provides distinctly stable and precise access to the heart chambers, valves, and great vessels. This system enters the heart in a flexible, relaxed state, conforming to the intracardiac anatomy with minimal cardiac dist

Endocardial versus epicardial pacing in pacemaker-dependent patients after device extraction: a meta-analysis.

INTRODUCTION: Pacemaker-dependent (PM) patients with cardiac implantable electronic device (CIED) infection require implantation of a temporary-pacemaker (TP) and delayed endocardial reimplantation or implantation of an epicardial-pacing-system (EPI) before device extraction. Our aim was to compare the TP and EPI-strategy after CIED extraction through a meta-analysis. METHODS: We searched electronic databases up to 25 March 2022, for observational studies that reported clinical outcomes of PM-dependent patients implanted with TP or EPI-strategy after device extraction. RESULTS: 3 studies wer

Epicardial mapping and ablation of ventricular tachycardia from the coronary venous system in post-coronary bypass patients.

BACKGROUND: Ventricular tachycardia (VT) ablation of mid- or epicardial substrate is difficult and requires a creative approach in patients with a history of coronary bypass that precludes percutaneous epicardial catheter manipulation. The coronary venous system (CVS) provides limited access to the epicardial surface of the heart. The objective of this study is to assess the feasibility, safety, and efficacy of epicardial mapping and ablation of VT substrates from the CVS in patients with history of coronary bypass. METHODS: Patients undergoing VT ablation at our institution were retrospectiv

Experimental study of cryofreezing energy applications on the ventricular myocardium using sheep hearts.

BACKGROUND: Cryofreezing energy has been utilized to abolish arrhythmogenic substrates of various kinds of tachyarrhythmias. However, systematic electrophysiological and histological investigations of cryothermia have never been performed. The aim of this study was to clarify those aspects using sheep beating hearts. METHODS: A total of eight adult sheep were utilized under total anesthesia, and a pericardial cradle was made by opening the pericardium. Eight epicardial plaque electrodes were sutured on the epicardial surface of the left ventricle 2∼3 cm apart from each other. A cryoprobe was

Endocardiosis and other heart disease in greyhounds.

Gross and microscopic lesions of heart disease in 230 greyhounds were recorded. The most common lesion was endcardiosis, which occurred in 24 (10.4%) of the greyhounds. Other heart lesions included endocardial fibrosis, myocardial necrosis, ossification of the aortic fibrous ring, and abscesses of the right atrium. Although females weighed less and had lighter hearts, body weights, heart weights and heart:body weight ratios did not differ between greyhounds with or without endocardiosis.

Endocardial function in pacing-induced heart failure in the dog.

Endocardial endothelium has been shown to modulate the contractile characteristics and alpha-1-adrenergic responsiveness of its adjacent myocardium. This study was designed to evaluate whether this was also the case in the pacing-overdrive model of heart failure in the dog, a model in which changes in contractile characteristics and changes in alpha-1-adrenergic responsiveness similar to those caused by endocardial endothelial removal, occur prior to endocardial endothelial removal. Pacing-overdrive heart failure caused a decrease in total tension (TT) (5.1 +/- 0.5 g/mm2 in controls to 2.2 +/-

Endocardial mapping of reentry around an anatomical barrier in the canine right atrium: observations during the action of the Class IC agent, flecainide.

INTRODUCTION: Flecainide is effective in terminating stable atrial flutter in the conscious dog with a Y-shaped right atrial lesion. In this model, flutter is due to circus movement of the impulse around a fixed anatomical barrier. METHODS AND RESULTS: To investigate the mechanism of flecainide-induced termination of this type of reentry, we determined the pattern of endocardial activation of the right and left atria before and during administration of flecainide by recording simultaneously from 192 electrode pairs in the isolated blood perfused heart. At least five consecutive flutter beats

Controversial issues in restrictive cardiomyopathy.

The topic of restrictive cardiomyopathy remains controversial for many reasons. The term cardiomyopathy is unfortunately sometimes used interchangeably with diastolic heart failure. Furthermore, diastolic heart failure is sometimes linked with other causes of diastolic dysfunction such as hypertrophic cardiomyopathy and mitral valve disease. Restrictive cardiomyopathy is a clinical entity of primary or secondary myocardial disease presenting a picture that closely simulates that of constrictive pericarditis. In the majority of cases the correct diagnosis can be arrived at following a careful p

Active fixation of endocardial pacing leads: the preferred method of pediatric pacing.

Pacing system failure due to lead related problems may necessitate repositioning or explantation of the problem lead. Pediatric patients with permanent pacemakers have additional considerations that necessitate revision or explantation of pacing leads. Active fixation type leads appear to offer the physician advantages over passive fixation leads that may make them the lead of choice for use in children. We reviewed our experience with active fixation type leads to determine whether the ease with which these leads could be revised or explanted justified recommending their use in our patients.

Chronic supraventricular tachycardia causes ventricular dysfunction and subendocardial injury in swine.

Chronic supraventricular tachycardia has been associated with ventricular dysfunction in humans and animals. However, this ventricular failure is poorly characterized, and the ultrastructural consequences of supraventricular tachycardia are unknown. We serially examined right and left ventricular function, endomyocardial ultrastructure, and creatine kinase activity in eight pigs at base line and again at 1, 2, and 3 wk following rapid atrial pacing. Left and right ventricular ejection fractions fell significantly from base line after 1 wk of chronic tachycardia. Three weeks of chronic pacing r

[Obstructive-restrictive cardiomyopathy. On the clinical aspects and pathology of Löffler endocarditis].

Löffler's endocarditis parietalis fibroplastica represents a special entity among the cardiomyopathies which is seldom to be seen. The disease can be diagnosed by clinical findings including a myocardial biopsy. Prognosis is bad because no adequate therapy exists. The mean survival rate is 18 months and congestive heart failure is cause of death. Etiology of endomyocardial fibrosis is unknown. Included is a case report of Löffler's endocarditis in which the patient had an unusually long survival time of more than three years. Characteristic for the disease were an initial right heart failure f

Pathology of the heart and the cardiac conduction system in Wegener's granulomatosis.

Two cases of Wegener's granulomatosis occurred in two women aged 53 and 67 years. The clinical course in both was of rapid deterioration resulting from renal failure and leading to death. At necropsy both cases showed severe and disseminated necrotising granulomatosis in the lungs, kidneys, and spleen but with variable degrees of cardiac and systemic vasculitis. One patient, with appreciable cardiac damage, developed a P wave abnormality due to atrial disease. In patients with Wegener's granulomatosis damage to the heart should be suspected and signs of conduction system abnormalities looked f
