Glucocorticoids stimulate endolymphatic water reabsorption in inner ear through aquaporin 3 regulation.

Menière's disease, clinically characterized by fluctuating, recurrent, and invalidating vertigo, hearing loss, and tinnitus, is linked to an increase in endolymph volume, the so-called endolymphatic hydrops. Since dysregulation of water transport could account for the generation of this hydrops, we investigated the role of aquaporin 3 (AQP3) in water transport into endolymph, the K-rich, hyperosmotic fluid that bathes the apical ciliated membrane of sensory cells, and we studied the regulatory effect of dexamethasone upon AQP3 expression and water fluxes. The different AQP subtypes were identi…

The vestibular aqueduct in patients with Meniere's disease. A temporal bone histopathological investigation.

Meniere's disease (idiopathic endolymphatic hydrops) was studied in human temporal bone histology sections. Measurements were made of the area, length, width, angle, position, and external aperture of the vestibular aqueduct in 27 temporal bones of individuals with this disease. These measurements were compared with measurements of the same parameters in 88 normal temporal bones. It was found that small vestibular aqueducts were more often observed in the temporal bones of patients with Meniere's disease than in temporal bones from individuals without this disorder. The difference in size of t…

Meniere's syndrome and otitis media.

We here present a clinical study of 37 patients with Meniere's syndrome. Meniere's syndrome can occur subsequent to and in some cases simultaneously with chronic otitis media. When otitis media which has occurred many years earlier in childhood becomes inactive, leading to sequelae of Meniere's later in life, full-blown Meniere's symptom-complex with vertigo tends to occur; whereas when active chronic otitis media accompanies Meniere's, cochlear Meniere's syndrome tends to predominate. Endolymphatic hydrops is described in pathological cases of labyrinthitis and in 11 human temporal bone cases…

Comparison of Endolymphatic Duct Dimensions and Jugular Bulb Abnormalities Between Meniere Disease and a Normal Population.

BACKGROUND: The pathogenesis of Meniere disease (MD) has not been fully understood. According to the widely accepted theory, imbalances due to overproduction and/or impaired absorption of endolymph may cause endolymphatic hydrops, which is the hallmark pathological finding in MD. Some developmental temporal bone abnormalities may impair endolymph circulation and absorption, and these abnormalities could be a part of MD pathophysiology. However, structural features of the temporal bone cannot explain MD pathophysiology definitively. The authors aimed to determine the length and width of the end…

Meniere's disease: Pathogenesis, treatments, and emerging approaches for an idiopathic bioenvironmental disorder.

Meniere's disease (MD) is a severe inner ear condition known by debilitating symptoms, including spontaneous vertigo, fluctuating and progressive hearing loss, tinnitus, and aural fullness or pressure within the affected ear. Prosper Meniere first described the origins of MD in the 1860s, but its underlying mechanisms remain largely elusive today. Nevertheless, researchers have identified a key histopathological feature called Endolymphatic Hydrops (ELH), which refers to the excessive buildup of endolymph fluid in the membranous labyrinth of the inner ear. The exact root of ELH is not fully un…

The Effect of Collagen Viscoelastoplasiticty on Reissner's Membrane Displacement: A Graphic Analysis.

BACKGROUND: The aim of this study is to analyze the effect of collagen viscoelastoplasticity on the bulge displacement of Reissner's membrane that is observed in endolymphatic hydrops and Meniere's disease. METHODS: Viscoelastoplastic load-deformation characteristics for Reissner's membrane were based on a reported collagen polymer model of the cochleo-saccular membranes. The projected bulge displacements of Reissner's model membrane at key distention points were quantified trigonometrically and plotted graphically. RESULTS: Initial deformation is characterized by a membrane laxity with subs…

Long-term treatment with chlorthalidone reduces experimental hydrops but does not prevent the hearing loss.

Long-term treatment of guinea pigs with the diuretics chlorthalidone and acetazolamide, following the experimental obstruction of the endolymphatic duct, was assessed using chronically implanted round window cochlear electrodes. The diuretic chlorthalidone appeared to curb the progressive low-frequency sensitivity loss during the first 4 weeks following surgery, as compared with animals receiving the diuretic acetazolamide or no treatment. However, this apparent beneficial effect decreased after 4 weeks and was not apparent at 14 weeks post-induction. On the other hand, morphological control a

Lomerizine therapy for the treatment of benign paroxysmal vertigo of childhood transitioning into atypical basilar migraine: A case report.

We report a rare case of benign paroxysmal vertigo (BPV) of childhood transitioning into basilar migraine (BM) that was effectively treated … revealed profound right-side sensorineural hearing loss. Among the differential diagnoses, delayed endolymphatic hydrops, epilepsy and BM were considered. Delayed endolymphatic hydrops was considered unlikely since no nystagmus occurred during the vertigo attacks and there was no change in hearing; electroencephalography revealed no epileptic seizure waves. The attack …

Amplitude and area ratios of summating potential/action potential (SP/AP) in Meniere's disease.

CONCLUSIONS: Our results suggest that summating potential/action potential (SP/AP) area ratio may not necessarily have higher sensitivity in the diagnosis of endolymphatic hydrops of Meniere's disease (MD) than SP/AP amplitude ratio in transtympanic electrocochleography (ECochG). OBJECTIVE: Recent studies suggested that SP/AP area curve ratio was more sensitive to endolymphatic hydrops in comparison with SP/AP amplitude ratio in extratympanic ECochG. The purpose of the present study was to evaluate the utility of the SP/AP area curve ratio in transtympanic ECochG for the diagnosis of MD. PAT…
