The effects of V2 antagonist (OPC-31260) on endolymphatic hydrops.

In the present study, two experiments were performed to investigate the influence of OPC-31260 on experimentally induced endolymphatic hydro … of OPC-31260 resulted in a significant decrease in MAQP2/Mbeta-actin both in the cochlea and in the endolymphatic sac (t-test, P<0.001). These results indicate that water homeostasis in the inner ear is regulated via the vasopressin-AQP2 system, and that the vasopressin type-2 antagonist OPC-31260 is a promising drug in the treatment of Meniere's disease.

The effects of pressure on cochlear microphonics in experimentally induced hydropic ears in the guinea pig.

The effect of perilymphatic pressure on the cochlear microphonics (CM) threshold was examined in guinea pigs with experimentally induced hydrops. When a pressure of 300 mm H2O was applied to the scala tympani of the basal turn, the 500 Hz CM threshold shifts in ears during the 1st week following obliteration of the endolymphatic sac and duct were significantly smaller when compared with shifts in normal ears. Larger threshold shifts were then regained at 4-12 weeks after surgery. When acetazolamide was administered to hydropic animals, the effect of the increase of perilymphatic pressure on th…

Hearing preservation in vestibular neurectomy.

The goal of vestibular neurectomy is to control disabling vertigo while preserving hearing in patients with nonhydropic intractable peripheral vertigo or in patients with Meniere's disease in which an endolymphatic sac procedure has failed. Labyrinthectomy continues to be used to treat patients with intractable vertigo and serviceable hearing. We feel that a labyrinthectomy is contraindicated when any useful hearing remains. Vestibular neurectomy affords the surgeon a means to eliminate the abnormal vestibular input without sacrificing hearing. Two approaches have been used to section the vest…

Alternobaric oxygen therapy in long-term treatment of Ménière's disease.

Hyperbaric oxygen therapy (HBQ) has been used for several years as a treatment for Ménière's disease, particularly in Sweden. In this study continuous variations in pressure (from 1.7 to 2.2 ATA; alternobaric oxygen therapy: ABOT) were used to decrease endolymphatic hydrops, the typical histopathological substrate of Ménière's disease by increasing hydrostatic pressure and mechanical stimulation of the endolymphatic flow toward the duct and the endolymphatic sac, which produces a consequent increase in the dissolved O2 content in the labyrinth liquid, which should contribute to recovering cell…

Von Hippel-Lindau Syndrome: An Updated Narrative Review for the First-Contact Clinician.

Von Hippel-Lindau (VHL) syndrome is an autosomal dominant (AD) condition that increases the risk of developing CNS and retinal hemangioblastomas (R-Hb), as well as clear cell renal cell carcinoma (ccRCC), pheochromocytomas, pancreatic neuroendocrine tumors (pNETs), and endolymphatic sac tumors. CNS hemangioblastomas and renal cell carcinomas are among the leading causes of mortality in affected individuals. Recent therapeutic advances, particularly hypoxia-inducible factor 2 alpha (HIF-2α) inhibitors and vascular endothelial growth factor (VEGF)-targeted therapies, have yielded promising resul…

[Surgery for vertigo].

Spontaneous recovery or central compensation makes surgical procedures rare in patients with vertigo. Surgery for vertigo proposed after pharmacological or physical therapy fails to eliminate Ménière's disease and some very rare cases of paroxystic positional vertigo. The main target in treating Ménière's disease is to promote vestibular compensation, which is possible only with a nonprogressive and stable deficit leading to readjustment of vestibular reflexes. Surgical procedures can be classified as nondestructive (endolymphatic sac decompression, vestibular nerve decompression, patching of …

Middle ear adenocarcinoma with intracranial extension. Case report.

Middle ear adenocarcinoma is a very rare, locally invasive neoplasm assumed to arise from the middle ear mucosa. Although endolymphatic sac tumor (aggressive papillary middle ear tumor) and jugulotympanic paraganglioma may show brain invasion, intracranial extension of histologically confirmed middle ear adenocarcinoma has not been previously reported. The authors describe a 53-year-old man who suffered from otalgia and tinnitus for more than 10 years and from neurological deficits for 1 year due to a large temporal bone tumor that invaded the temporal lobe. A combined neurosurgical and otolar…

Evaluation of the monothermal caloric test.

In 1970, Torok reported that patients with labyrinthine disease showed vestibular recruitment and that patients with retrolabyrinthine disea … showed recruitment and one showed asymmetry. Two patients with Meniere's disease who had undergone endolymphatic sac operations showed normal responses. These findings support Torok's claim that the monothermal caloric test discriminates between labyrinthine and retrolabyrinthine vestibular pathology.

All symptoms were eliminated within 8 days after EDSS. There was no significant difference in the duration of any vestibular symptoms between bilateral (n = 8) and unilateral cases (n = 42). Temporal bone histopathologic findings of Waardenburg's syndrome: a case report.

A histopathological study of the temporal bones of a 3-year-old black girl who had bilateral deafness associated with Waardenburg's syndrome type II showed a similar pattern of pathology in both ears. The most striking findings were an absence of pigmentation in the inner ear and cochleosaccular abnormality. This is, to our knowledge, only the third report on human temporal bone histopathology in Waardenburg's syndrome and the first report of such a case with absence of pigment (melanin) in the inner ear. A possible association of hearing loss with absence of inner ear pigment in this case is

Lermoyez's syndrome: histopathologic report of a case.

This paper reports findings in a pair of temporal bones from a patient with clinical evidence of Lermoyez's syndrome, a rare variant of Meniere's disease. Endolymphatic hydrops is limited to the basal turn of the cochlea and saccule in Lermoyez's syndrome, but more generalized in Meniere's disease.

Medical and surgical management of Meniere's disease.

Meniere's disease has been the focus of much controversy concerning its etiology and pathophysiology, not to mention treatment. A brief review is presented of the major proposed causative factors and therapy, both medical and surgical, founded upon these theories. We also present the diagnostic evaluation and therapeutic modalities employed by The Otology Group, P.C., as well as long-term, follow-up data on our patients.

Use of polytomography in Meniere's disease.

In a long-term study of the treatment of Meniere's disease, 105 cases treated from 1965 through 1973 were divided into subgroups by virtue of the institution of routine polytomographic examination of the vestibular aqueduct (1968). A comparison of results of treatment in the two groups (empiric indication of endolymphatic shunt vs x-ray film evidence) yielded the following conclusions: (1) Success or failure of the endolymphatic shunt operation is significantly correlated with patency or obliteration of the vestibular aqueduct as seen on roentgenographic examination. (2) To be effective, medic

Ossicular fixation caused by bone dust after saccus decompression surgery.

HYPOTHESIS: To highlight the clinical and histologic features of ossicular fixation produced by bone dust during mastoid surgery. BACKGROUND: Bone dust deposition in the middle ear may occur during temporal bone surgery, but the ossicular fixation that may result from it remains underreported. METHODS: A case is reported of delayed conductive hearing loss after saccus decompression surgery resulting from ossicular fixation by bone dust deposition. RESULTS: Six months after the initial surgery, the patient experienced a 45 dB conductive hearing loss. The findings on second-look tympanotomy w

Ménière's disease (MD) is a chronic vestibular disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss (SNHL), t … nt a secondary manifestation rather than the primary driver of symptoms, potentially resulting from endolymphatic sac hyperplasia. Given this complexity, no single unifying mechanism has been confirmed, and etiologic contributors may vary among patients.
