Ferric derisomaltose and Outcomes in the Recovery of Gynecologic oncology: ERAS (Enhanced Recovery After Surgery) (FORGE) - a protocol for a pilot randomised double-blinded parallel-group placebo-controlled study of the feasibility and efficacy of intravenous ferric derisomaltose to correct preoperative iron-deficiency anaemia in patients undergoing gynaecological oncology surgery.

INTRODUCTION: Iron-deficiency anaemia is common in gynaecological oncology patients. Blood transfusions are immunosuppressive and carry immediate and long-term risks. Oral iron replacement remains the standard of care but requires prolonged treatment courses associated with gastrointestinal side effects, poor compliance and variable absorption in cancer patients. Intravenous iron has been shown to decrease the need for allogeneic blood transfusion in gynaecological oncology patients undergoing chemotherapy, but the efficacy of this treatment in the preoperative period is unknown. The goal of t

Evaluation of the Feasibility, Safety, and Effectiveness of a Tailored Enhanced Recovery After Surgery Protocol for Gastroepiploic Vascularized Lymph Node Transfer in Breast Cancer-Related Upper Extremity Lymphedema: A Preliminary Retrospective Comparative Study.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols are multimodal perioperative pathways designed to reduce surgical stress, accelerate recovery, and improve resource utilization. Although ERAS principles have been increasingly adopted in microsurgery, their application in vascularized lymph node transfer (VLNT), particularly using gastroepiploic flaps, remains poorly defined. The aim of this study was to evaluate the feasibility, safety, and effectiveness of a tailored ERAS protocol in patients undergoing gastroepiploic VLNT for breast cancer-related upper extremity lymphedema. PAT…

Integrated narrative and cognitive therapy enhances psychological and functional recovery after ultra-low sphincter-preserving surgery for low rectal cancer: A randomized controlled trial with exploratory oncologic outcomes.

BACKGROUND: Ultra-low sphincter-preserving surgery (ULSPS) for low rectal cancer often results in persistent bowel and sexual dysfunction alongside significant psychological distress, which may adversely affect recovery and may be associated with longer-term oncologic outcomes. Evidence on integrated psychosocial interventions in this context remains limited. OBJECTIVE: To evaluate whether a structured six-month psychosocial intervention integrating narrative therapy and cognitive behavioral therapy improves psychological and functional recovery after ultra-low sphincter-preserving surgery fo

Indicators for Monitoring Recovery From Surgery to Discharge Using Accelerometer in Patients With Esophageal Cancer.

Esophageal cancer is a highly invasive malignancy necessitating esophagectomy, which is associated with considerable postoperative morbidity and prolonged hospitalization. Enhanced Recovery After Surgery (ERAS) protocols recommend early mobilization to facilitate recovery; however, objectively assessing physical activity during hospitalization remains challenging. Traditional methods, such as the 6-min walk test or patient-reported questionnaires can be burdensome, or may not accurately reflect recovery. Accelerometer-based measurement provides a low-burden, objective approach, but previous st…

Impact of ERAS compliance on the short-term outcomes for distal radius surgery: a single-center retrospective study.

BACKGROUND: Distal radius fractures (DRF) account for one in five bony injuries in both primary and secondary trauma care. Enhanced recovery after surgery (ERAS) has been adopted successfully to improve clinical outcomes in multiple surgical disciplines; however, no study has investigated the effect of different degrees of compliance with ERAS protocol on short-term outcomes following distal radius surgery. We aimed to analyze whether different degrees of compliance with the ERAS pathway are associated with clinical improvement following surgery for DRF. METHODS: We retrospectively analyzed a…

Updates in Enhanced Recovery Pathways for Total Knee Arthroplasty.

Over 800,000 total knee arthroplasties (TKAs) are performed annually in the United States, with costs exceeding $18 billion. As the prevalence of knee osteoarthritis rises, enhanced recovery after surgery (ERAS) protocols have become vital to improve outcomes and reduce costs. ERAS in TKA focuses on optimizing pain control, minimizing hospital stays, and reducing complications. Key preoperative interventions include correcting anemia, improving nutritional status, encouraging smoking cessation, and cautiously evaluating carbohydrate loading. These measures have been shown to lower transfusion …

From feasibility to biological recovery: Reframing enhanced recovery pathways for elderly gastric cancer patients.

Enhanced recovery after surgery (ERAS) has reshaped perioperative care in gastrointestinal oncology, but its application in elderly gastric cancer patients requires a shift from feasibility toward a biologically grounded understanding of recovery. Aging is characterized by frailty, sarcopenia, multimorbidity, chronic inflammation, and circadian vulnerability, which collectively influence postoperative resilience. Contemporary evidence shows that ERAS pathways remain feasible and safe in older adults, yet responsiveness varies widely due to physiological heterogeneity rather than chronological …

Ambulatory colectomy: a pathway for advancing the enhanced recovery protocol.

Enhanced recovery after surgery (ERAS) protocols employ multiple factors to decrease surgical stress and improve recovery (Lyon et al., World J Gastroenterol 18(40):5661-5663, 2012). These protocols use multimodal approaches to improve outcomes, including length of stay and morbidities (Lyon et al., World J Gastroenterol 18(40):5661-5663, 2012; Carmichael et al., Dis Colon Rectum 60:761-784, 2017). The ERAS guidelines have evolved since development; however, the question is posed of how to improve next (Lyon et al., World J Gastroenterol 18(40):5661-5663, 2012). With the success of ERAS, in co…
