Early detection of myocardial fibrosis in patients with coronary microvascular dysfunction in the absence of late enhancement: role of T1 mapping and extracellular volume analysis.

BACKGROUND: Coronary microvascular dysfunction (CMD) can exacerbate myocardial fibrosis, which may further impair microvascular function. However, the relationship between CMD and the extracellular volume fraction (ECV) in patients with ischemia with no obstructive coronary artery disease (INOCA) who are late gadolinium enhancement (LGE)-negative remains undetermined. We used T1 mapping and ECV to assess diffuse myocardial tissue abnormalities and extracellular compartment expansion in LGE-negative patients with CMD. We then investigated how these markers correlate with global myocardial flow

Age-related changes in the atrial muscarinic type 2 receptor and their effects on atrial fibrillation vulnerability in rabbits.

Aging plays an important role in increased vulnerability to atrial fibrillation (AF). Mediated by activity at the muscarinic type 2 receptor (M2R), the parasympathetic nerve contributes to the onset of AF. The purpose of this study was to investigate whether aging changes the distribution of M2R in the atrial myocardium and to determine the impact of these changes on AF vulnerability. Expression of M2R in the atrial myocardium was evaluated by immunostaining and Western blot in three groups-young (3 months old), mature (8 months old) and senescent (36-48 months old) rabbits. AF inducibility wa

Electrocardiographic imaging of patients with heart failure with left bundle branch block and response to cardiac resynchronization therapy.

BACKGROUND: Cardiac resynchronization therapy (CRT) for patients with left bundle branch block (LBBB) and left ventricular (LV) dysfunction has variable responses. We hypothesized that conduction delays in intrinsic rhythm and responses to pacing may be influenced by electrophysiologic substrate. To determine this, we used electrocardiographic imaging (ECGI), which maps epicardial ventricular excitation noninvasively. METHODS AND RESULTS: Electrocardiographic imaging was conducted in 8 patients undergoing CRT. During intrinsic conduction with LBBB (6 patients), ventricular activation was hete

[Clinical importance of evaluation of circulating miRNA expression and epicardial adipose tissue thickness as predictors of cardiovascular pathology in young patients with type 1 diabetes mellitus].

UNLABELLED: Cardiovascular disease (CVD) in type 1 diabetes mellitus (T1DM) is preceded by asymptomatic changes in the geometry of the heart. The only symptoms of the beginning of cardiac remodeling and concomitant predictors of an unfavorable cardiovascular prognosis are: thickening of epicardial fat (EAT), secreting a number of adipokines, and cardiospecific miRNAs. To improve the effectiveness of prevention of CVD in young patients with DM1, a search was made for structural-functional and epigenetic markers. AIM: To assess the state of the cardiovascular system according to MRI-heart with

Analysis of the expression of genes encoding the endothelin signaling pathway in fetal, healthy and diseased adult human heart using single-cell transcriptomics.

This study maps the cellular landscape of the principal genes (EDN1, EDN2, EDN3, EDNRA, EDNRB, ECE1, and ECE2) in the endothelin signaling pathway using published single cell/nucleus RNA sequencing data, comparing cellular expression in healthy fetal, healthy adult, or diseased adult human hearts. EDN1 was the most abundantly expressed gene, principally localized to endothelial cells from all three conditions. EDN2 and EDN3 expression was only detectable in epicardial mesothelial cells forming the outer layer of the heart, important for cardiac development. EDNRA was expressed in contractile c

Effects of left anterior fascicular block on the depolarization process as depicted by total body surface mapping.

To examine the effects of left anterio fascicular block (LAFB) on the depolarization process as manifested on the body surface, 142 lead maps were recorded in 25 subjects with LAFB. Three abnormalities were detected: (1) In the early and mid portion of QRS, twenty of 25 subjects showed abnormal anterior superior positivity, starting in the precordial area and proceeding toward the left subclavicular area. The explanation was thought to be the relatively delayed, dysynchronous, and superiorly directed altered sequence of depolarization of the anterior left ventricle. (2) All the subjects showed

Association of regional myocardial conduction velocity with the distribution of hypoattenuation on contrast-enhanced perfusion computed tomography in patients with postinfarct ventricular tachycardia.

BACKGROUND: Cardiac magnetic resonance imaging has been shown to be beneficial for identification of the ventricular tachycardia (VT) substrate before catheter ablation. Contrast-enhanced perfusion multidetector computed tomography (CEP-MDCT) is more generalizable to clinical practice, and wall thickness and regional hypoenhancement on CEP-MDCT can identify potential substrate sites, albeit with decreased specificity. OBJECTIVE: The purpose of this study was to evaluate the association between wall thickness and attenuation on CEP-MDCT with local conduction velocity (CV) and electrogram abnor

Endogenous contrast T1rho cardiac magnetic resonance for myocardial fibrosis in hypertrophic cardiomyopathy patients.

BACKGROUND: Late gadolinium enhancement (LGE) is a standard method to evaluate myocardial fibrosis, but restricted due to contrast agent contraindications. Non-contrast T1rho can generate endogenous contrast, and detect fibrosis in chronic myocardial infarction. However, T1rho for hypertrophic cardiomyopathy (HCM) patients is still unreported. The present study aimed to investigate T1rho for fibrotic assessment and the clinical implication in HCM patients. METHODS: 18 HCM patients and 8 controls underwent T1rho, cine, and LGE cardiac magnetic resonance (CMR). T1rho relaxation time maps were c

Transcriptomic Analysis of Adult Mouse Cardiac Stromal Cells Using Single-Cell qRT-PCR.

Fate-mapping studies have challenged the longstanding view of the adult mammalian heart as a post-mitotic organ, suggesting limited cardiomyocyte renewal. This has spurred efforts to determine whether selected cardiac stromal cells have regenerative potential; however, their contribution to cardiac regeneration has been found to be minimal compared with that of cardiomyocyte proliferation. Despite this, transplantation of some cardiac stromal cell populations has shown therapeutic potential through paracrine signalling. The identity of the paracrine-active stromal cell populations remains uncl

Advanced Cardiac Rhythm Management by Applying Optogenetic Multi-Site Photostimulation in Murine Hearts.

Ventricular tachyarrhythmias are a major cause of mortality and morbidity worldwide. Electrical defibrillation using high-energy electric shocks is currently the only treatment for life-threatening ventricular fibrillation. However, defibrillation may have side-effects, including intolerable pain, tissue damage, and worsening of prognosis, indicating a significant medical need for the development of more gentle cardiac rhythm management strategies. Besides energy-reducing electrical approaches, cardiac optogenetics was introduced as a powerful tool to influence cardiac activity using light-sen

Doxycycline Improves Fibrosis-Induced Abnormalities in Atrial Conduction and Vulnerability to Atrial Fibrillation in Chronic Intermittent Hypoxia Rats.

BACKGROUND The structural remodeling of atrial architecture, especially increased amounts of fibrosis, is a critical substrate to atrial fibrillation (AF). Doxycycline (Doxy) has recently been shown to exert protective effects against fibrogenic response. This study investigated whether doxycycline (Doxy) can sufficiently ameliorate the fibrosis-induced changes of atrial conduction and AF vulnerability in a chronic intermittent hypoxia (CIH) rat model. MATERIAL AND METHODS Sixty rats were randomized into 3 groups: Control, CIH, and CIH with Doxy treatment (DOXY) group. CIH rats were exposed to

Validation of an animal model of right ventricular dysfunction and right bundle branch block to create close physiology to postoperative tetralogy of Fallot.

BACKGROUND: In the past 5 years a few number of studies and case reports have come out focusing on biventricular (BiV) stimulation for treatment of congenital heart disease related ventricular dysfunction. The few available studies include a diverse group of pathophysiological entities ranging from a previously repaired tetralogy of Fallot (TOF) to a functional single ventricle anatomy. Patient's status is too heterogeneous to build important prospective study. To well understand the implication of prolonged electromechanical dyssynchrony we performed a chronic animal model that mimics essenti
