Long-term sequelae of epilepsy.

The long-term prognosis for epileptics vary from an entirely normal life to early death or complete incapacity. Studies to date have not provided clear answers on the long-term sequelae of epilepsy. This is because there are many epileptic syndromes with different outcomes. Most studies include a heterogeneous group of patients who may not be representative of all the epileptics in the community. The prognosis for permanent remission of seizure varies with the seizure type. In childhood epilepsies, overall there is at least 50% chance of permanent remission. The more seizures the patient has h…

Clorazepate therapy for intractable epilepsy.

Thirty-one epileptic patients with seizures refractory to conventional anticonvulsants were treated by adding clorazepate dipotassium to their regimen. Twelve cases showed improvement in seizure frequency, three of whom attained a seizure free state. Response to clorazepate was not related to the type of epilepsy, but patients with secondary generalized epilepsy tended to be less responsive than those with partial epilepsy. Among the various seizure types, generalized tonic-clonic seizures and simple partial seizures showed, although not significant, a tendency to be more responsive to cloraze…

Disappearing CT lesions in epilepsy.

A striking but reversible computed tomographic (CT) lesion corresponding to seizure activity is reported in a series of 46 epileptic patients. None of these patients had evidence of tuberculosis or cysticercosis, and all were treated with antiepileptic drugs only. Maximal radiological changes occurred in the area of maximal epileptic discharge; however, multiple lesions were seen in three cases. The CT lesion reappeared with recurrence of the ictus in four cases during follow-up, and this, too, disappeared after complete arrest of the seizures. The clinical, CT scan, and other investigatory fi

Surgical options for uncontrolled epilepsy.

Surgical management of refractory epilepsy has undergone many modifications since it was instituted for the treatment of post-traumatic seizures many decades ago. Localization of onset of all seizures to a single resectable area of brain by various noninvasive and invasive recording methods is a complicated and controversial area, but guidelines are available. Success is correlated with accurate location and complete resection of the focus. When this is accomplished, resective surgery for epilepsy can provide cure or over 95 per cent reduction in seizure frequency in 60 to 90 per cent of patie…

Late epilepsy after aneurysm operations.

The risk of late epilepsy was analyzed in a consecutive series of 177 patients operated on for supratentorial aneurysms. Late seizures occurred in 25 patients (14%); the seizures were recurrent in 21 patients (12%). Most seizures were partial, secondary generalized, or generalized tonic-clonic (72%). The mean latency between the operation and seizures was 8.4 months (range, 1 to 24 months), and in only 2 patients was the interval more than 12 months. The most important risk factors were preoperative and postoperative complications. Only 2.5% of the 81 Grade I patients developed epilepsy, compa…

[Tick-borne encephalitis and epilepsy].

In patients suffering from tick-borne encephalitis development of epileptic seizures, Kozhevnikov's epilepsy, Jackson's epilepsy, myoclonus-epilepsy, chorea-epilepsy Hunt's myoclonic dyssynergy and various hyperkineses without general convulsions was observed. It was found that a particular place in the structure of the convulsive syndrome was occupied by Kozhevnikov's epilepsy which was not so frequently encountered in other diseases. This is, probably, due to a particular vulnerability of the motor structures, mainly, at the spinal level, at which the final motor response is formed.

Psychiatric aspects of psychomotor epilepsy.

Psychomotor or temporal lobe epilepsy is a frequently missed diagnosis. It is often confused with grand mal and petit mal epilepsy. At times it is the first symptom of an organic neurological disease. It is often masked as a psychiatric disorder or is associated with a mental illness without clinically detectable seizures. These psychic manifestations simulate all of the neuroses and major psychiatric states. Excitement states with amnesia may lead to violent antisocial behavior. All these manifestations may be aggravated by alcohol. Thalamic epilepsy shows itself in similar psychiatric manife…
