Pediatric invasive fungal rhinosinusitis in immunocompromised children with cancer.

OBJECTIVES: 1) To determine the factors that predispose towards invasive fungal rhinosinusitis (FS) in immunocompromised children with cancer, and 2) to propose practice guidelines for management of pediatric FS in immunocompromised patients. METHODS: Retrospective chart review of 9 patients who developed invasive FS compared to 8 patients who did not develop invasive FS during the study period. Presenting signs and symptoms, nasal endoscopic findings, radiographs, laboratory studies, histologic and microscopic samples, and outcomes were compared. RESULTS: Seventeen consecutive pediatric imm

Total-length invasion of the facial nerve by parotid carcinoma ex pleomorphic adenoma.

A case of parotid carcinoma extending along the facial nerve up to the internal auditory canal is presented. Total parotid resection with neck dissection was performed, as well as resection of the ear canal, eardrum, ossicles, and transmastoid and translabyrinthine facial nerve, and obliteration using the fascia lata and fatty tissue. This was followed by adjuvant radiotherapy. The facial nerve showed continuous swelling along its length with lump formation at the site of the geniculate ganglion and the internal acoustic canal. Pathological examination revealed the salivary duct carcinoma subt

Retrograde Parotidectomy Allowing Diagnosis of Parotid Lymphoma and Concomitant Facial Reanimation.

We describe a rare case involving a 52-year-old woman presenting with a large facial mass and facial palsy in whom five biopsies were non-diagnostic. In the setting of a growing facial mass that evades traditional diagnostic methods, we describe a strategy of concurrent facial reanimation surgical approach and parotidectomy to safeguard the facial nerve, obtain a diagnostic tissue sample, and simultaneously reinnervate midfacial function with a CN V-VII nerve transfer.

Surgical management of symptomatic hemangioma of the geniculate ganglion: fascicular-sparing resection or grafting?

Geniculate ganglion hemangioma (GGH) is rarely presented in the neurosurgical literature. It extends extradurally on the middle fossa floor and displaces the intratemporal part of the facial nerve. Surgical treatment is advisable at early symptoms. Proposed techniques include fascicular-sparing resection or nerve interruption with grafting. No definitive conclusions exist about the superiority of a certain technique in preserving facial nerve integrity and function. Through the description of a surgically managed symptomatic GGH, we herein discuss literature data about the surgical results of

Tumor localization is the important factor for recovery time of postoperative facial nerve paralysis in benign parotid surgery.

OBJECTIVE: Facial nerve paralysis is the most problematic complication of surgery for parotid tumors. This study aimed to examine the progress of recovery from postoperative transient facial nerve paralysis (POFNP). METHODS: Participants were 203 patients who developed POFNP after benign parotid surgery. A Kaplan-Meier showed the progress of recovery from paralysis. Factors involved in recovery were examined. For factors for which a significant difference was found, recovery from paralysis was examined over time. RESULTS: Rates of recovery from paralysis were as follows: 28.6% of patients at

Facial Nerve Schwannoma Resection and Nerve Anastomosis in 1 Stage by Translabyrinthine Approach.

Facial nerve schwannomas (FNS) are rare benign tumors that can develop in any segment of the facial nerve. Patients often experience facial palsy and hearing loss after FNS removal or even before surgery.1-3 Double-stage treatment is usually necessary for tumor resection and reconstruction of facial nerve function.4,5Video 1 describes the translabyrinthine approach for resection of the middle-posterior cranial fossa dumbbell FNS and end-to-side hypoglossal-facial nerve anastomosis in 1 stage. The case presented is of a 28-year-old female patient who suffered from right-side hearing loss for 1.

Contemporary Concepts of Primary Dynamic Facial Nerve Reconstruction in the Oncologic Patient.

Transection of the facial nerve and its branches during extensive ablative procedures in the oncologic patient causes loss of control of facial mimetic muscles with severe functional and aesthetic sequelae. In such patients with advanced tumorous disease, copious comorbidities, and poor prognosis, rehabilitation of the facial nerve has long been considered of secondary priority. However, recent advances in primary facial nerve reconstruction after extensive resection demonstrated encouraging results focusing on rapid and reliable restoration of facial functions. The authors summarize 3 innovat

Granuloma faciale.

Granuloma faciale is a rare, benign skin condition that usually occurs on the face. Using an exemplary case of granuloma faciale, we will present the clinical and histological characteristics of this dermatosis. A 49-year-old man presented with a 6-month history of a 10 mm-diameter asymptomatic papulo-nodular red-brown lesion of the nose. A biopsy specimen led to the diagnosis of granuloma faciale. The patient received a session of pulsed-dye laser therapy, which led to significant improvement. This benign and usually isolated dermatosis can more rarely be extrafacial. It may often be mistaken

Facial nerve schwannoma of the internal auditory canal.

BACKGROUND: Facial nerve schwannoma of the internal auditory canal is a very rare tumour. AIM: While Bell's palsy is the commonest cause of a facial paresis, more serious causes should be excluded if recovery is delayed. METHOD: A case report of a young man who presented with a long standing facial palsy. CONCLUSION: Any facial palsy that does not show evidence of recovery within six weeks should be investigated radiologically to exclude a tumour.

Revision facial nerve surgery.

Transection of the facial nerve can result from blunt or penetrating trauma to the face or temporal bone. It can also occur accidentally during surgery, or as a planned surgical procedure carried out in the interest of eradicating disease. If transection is recognized at surgery, direct anastomosis or cable grafting is the procedure of choice. This article presents two cases with neither clinical nor electrical evidence of recovery. The authors review current understanding of the changes that occur in the neuron, axon, and muscle after injury to the nerve and the underlying pathology that led

The bony crescent sign--a new sign of facial nerve schwannoma.

Schwannomas of the facial nerve are rare slowly growing lesions that have a predilection for the geniculate ganglion. Radiological evaluation is important in their diagnosis and in the assessment of their extent. In our series of 4 cases the facial nerve schwannoma was seen on high resolution CT as a soft tissue mass bounded anteriorly by a thin rim of bone. This bony crescent sign is a previously undescribed sign of facial nerve schwannoma which appears on the basis of this small series to be strongly indicative of the presence of this tumour. Schwannomas are relatively uncommon intracranial

[Facial nerve neurinoma masked by otosclerosis (author's transl)].

A case of operation for presumed otosclerosis. Discovery of a neurinoma between the branches of the stapes probably originating from the geniculate. Facial paralysis was not present either before or after the operation which, in fact, was only partial (a more extensive operation was refused later on). A curious F.P. "to cold", 30 years beforehand, recovered without sequelae.

Angiofibromata mimicking viral warts.

A case is presented of an otherwise healthy woman with a relatively normal childhood and adult life and no evidence of any congenital anomaly, who at the age of twenty-seven, suddenly, almost explosively, had development of verruca-like lesions of the face. It is well to remember that the clinically obvious is not always the obvious, things are not always what they seem, and verrucae are not always viral warts.

[Diagnostic studies of facial nerve function in bell's palsy (author's transl)].

In early Bell's Palsy the nerve excitability test (NET) still proves to be a very simple and reliable method for determining whether or not a decompression operation is necessary. However, two weeks after the onset of the paralysis, pathological findings in the NET may require verification by electromyography.

Natural history of a fibrous cephalic plaque and sustained eight decade follow-up in an 80 year old with tuberous sclerosis complex type 2.

INTRODUCTION: Fibrous cephalic plaques (FCP) are a characteristic manifestation of tuberous sclerosis complex (TSC) and occur in one third of cases. Their natural history and long term course is unknown, as is the outcome of long term follow-up of TSC cases in old age. PHENOTYPE AND METHODS: We describe an 80 year old with TSC due to a c.2784dupC TSC2 mutation, who was diagnosed in infancy with an FCP and was regularly followed up at the TSC clinic over 8 decades with regular epilepsy treatment and renal monitoring. RESULTS: Regular clinical photography and clinical records document the plaq

Combined isolated trigeminal and facial neuropathies from perineural invasion by squamous cell carcinoma: A case series and review of the literature.

Perineural invasion is a targeted cellular proliferation guided by neurotrophins, rather than a simple diffusion of tumor in a path of least resistance. Invasion of cranial nerves by squamous cell carcinoma can represent an important diagnostic dilemma. It commonly presents as a distinct clinical neurological syndrome of combined isolated trigeminal and facial neuropathies. The focal cancer source may have been overlooked or remain occult. This case series illustrates diverse clinical presentations and neuroimaging challenges in four patients with squamous cell carcinoma of the cranial nerves.
