Hemophilia A (HA) patients under emicizumab prophylaxis may require the concomitant use of procoagulant factors for breakthrough bleedings or immune tolerance induction (ITI). The aim of this study is to evaluate the ex vivo procoagulant effect of plasma-derived FVIII concentrates containing von Willebrand factor (pdFVIII/VWF) in samples from patients with severe HA without inhibitors on emicizumab prophylaxis. Samples from healthy controls (HC) and HA patients were drawn in sodium citrate plus corn trypsin inhibitor tubes and spiked with increasing concentrations of pdFVIII/VWF concentrates (

In patients with hemophilia A with inhibitor (PwHA-I), emicizumab drastically reduces bleeding events. However, few studies have investigated the behavior and effects of factor X (FX) in patients who require intensive treatment with factor VIII-bypassing agents (BPA) and emicizumab. A 59-year-old man with HA-I receiving emicizumab prophylaxis was admitted to our hospital because of acute gangrenous cholecystitis. He received percutaneous transhepatic gallbladder drainage and laparoscopic cholecystectomy along with repeated administration of recombinant activated factor VII (rFVIIa). On day 10,…

Several rare European von Willebrand disease missense variants of VWF (including p.Arg2185Gln and p.His817Gln) were recently reported to be common in apparently healthy African Americans (AAs). Using data from the NHLBI Exome Sequencing Project, we assessed the association of these and other VWF coding variants with von Willebrand factor (VWF) and factor VIII (FVIII) levels in 4468 AAs. Of 30 nonsynonymous VWF variants, 6 were significantly and independently associated (P < .001) with levels of VWF and/or FVIII. Each additional copy of the common VWF variants encoding p.Thr789Ala or p.Asp1472H…

In type 2N von Willebrand disease (vWD), von Willebrand factor (vWF) is characterized by normal multimeric pattern, normal platelet-dependent function, but a markedly decreased affinity for factor VIII (FVIII). In this report, we describe the case of a vWD patient who has an abnormal vWF multimers distribution associated with a markedly decreased vWF ability to bind FVIII. Sequencing analysis of patient's vWF gene showed, at heterozygous state, a G-->A transition resulting in the substitution of Asn for Asp at position 116 of the mature vWF subunit and a C-->T transition, changing the codon fo…

Congenital combined deficiency of factor V (FV) and factor VIII (FVIII; F5F8D, OMIM 227300) is a rare hereditary coagulopathy and accounts for approximately 3% of cases of rare coagulation disorders. The prevalence of this disease in the general population is estimated to be 1:1,000,000 and is significantly higher in regions where consanguineous marriages are permitted, such as the Mideast and South Asia. The disease has an autosomal recessive mode of inheritance and therefore occurs with an equal incidence among males and females. Heterozygous mutation carriers usually do not have clinical ma…

Three hundred and thirty-nine consecutive bleeds into the knees, elbows and ankles of severe haemophiliacs were graded I if more than 50% of normal movement was present, and graded 2 if less than 50% of normal movement was present. Nursing staff then administered randomly allocated doses of 7,14 or 28 units of factor VIII per kg. Medical staff unaware of initial doses assessed progress thereafter at 12-hourly intervals. Grade I bleeds into "normal" joints showed no difference in response to the three dosage regimes, but grade 2 bleeds did worse on low doses. There was no significant difference…

The detection rate of carriers of haemophilia was evaluated using the ratio of factor VIII procoagulant activity (VIIIc) to factor VIII antigen (VIIIag). In normals the corelation coefficient of VIIIc to VIIIag was 0.82. In 15 obligatory carriers of haemophilia whose VIIIc and VIIIag levels were studied in the authors' labotatory there was no correlation between VIIIc and VIIIag and the ratio of VIIIc to VIIIag was below the lowest normal value in 12 (80%). In all five obligatory carriers whose VIIIc levels were estimated in the referring institution and VIIIag levels in the authors' laborary …

The course and treatment of a life-threatening hemorrhagic episode in a patient with hemophilia A whose plasma contained a high concentration of an inhibitor of factor VIII activity is presented. The inhibitor of factor VIII was localized to the most anodal fractions of immunoglobulin G on electrophoresis, and was thus presumed to be an antibody directed against factor VIII. No therapeutic benefit occurred with infusions of massive amounts of fresh blood and factor VIII concentrates, or with a brief course of immunosuppressive therapy. Administration of standard and activated prothrombin compl…

Acquired hemophilia A is a rare autoimmune bleeding disorder caused by neutralizing autoantibodies against factor VIII. It typically affects older adults and may present with spontaneous mucocutaneous bleeding, extensive ecchymoses, soft-tissue hematomas, and isolated prolongation of the activated partial thromboplastin time. We report the case of an 84-year-old woman with hypertension, hyperlipidemia, and anxiety who presented with several weeks of spontaneous nontraumatic bruising, fatigue, and exertional dyspnea. Initial evaluation revealed severe normocytic anemia, isolated aPTT prolongati…

Hemophilia A presents significant perioperative challenges, particularly for neuraxial anesthesia due to the risk of spinal or epidural hematoma. Extended half-life recombinant factor VIII products (EHL), such as efanesoctocog alfa, provide sustained FVIII activity. We report a 52-year-old man with severe hemophilia A on weekly EHL prophylaxis undergoing total knee arthroplasty under spinal anesthesia with an adductor canal block. FVIII levels were maintained with a combination of EHL and standard factor VIII replacement. The patient had no neurologic sequelae. Safe neuraxial anesthesia can be…

BACKGROUND: Development of neutralizing anti-factor [F]VIII antibodies (inhibitors) follows recognition by CD4+ T cells of epitopes that are presented on the individual's human leukocyte antigen (HLA) class II. Limited blood volumes have presented a major challenge in mapping T-cell epitopes in FVIII, especially as these immune responses typically develop in early childhood.
OBJECTIVES: To determine whether CD4+ T cells from individuals with and without hemophilia A (HA) respond to the same T-cell epitopes.
METHODS: We used interferon-γ enzyme-linked immunospot (ELISPOT) assays with added co

The therapeutic clotting factor VIII (FVIII) is known for its particular immunogenicity, with nearly 30% of hemophilic patients developing neutralizing antibodies against the infused protein. The root cause of this immunogenicity is still not well understood, but intrinsic factors, such as FVIII byproducts, have been linked to the immunological response elicited. Bioengineering of the FVIII molecule has been improving its recombinant (rhFVIII) production in many aspects, mainly enhancing its expression and stability. Assessment of immunogenicity for novel recombinant isoforms is crucial for fu…

Tolerance to self-proteins involves multiple mechanisms, including conventional CD4+ T-cell (Tconv) deletion in the thymus and the recruitment of natural regulatory T cells (nTregs). The significant incidence of autoantibodies specific for the blood coagulation factor VIII (FVIII) in healthy donors illustrates that tolerance to self-proteins is not always complete. In contrast to FVIII-specific Tconvs, FVIII-specific nTregs have never been revealed and characterized. To determine the frequency of FVIII-specific Tregs in human peripheral blood, we assessed the specificity of in vitro expanded T…

BACKROUND: Patients with hemophilia A can develop inhibitors to factor concentrates. Emicizumab, a nonfactor-based therapy, has efficacy despite inhibitors. FVIII activity assessment on emicizumab treatment requires a bovine chromogenic reagent such as TriniCHROM FVIII:C.
METHODS: FVIII levels were measured in 15 patients with and 35 without hemophilia and 10 patients on emicizumab therapy with a time-to-clot and the TriniCHROM FVIII:C reagents. FVIII inhibitor levels were also determined with both reagents.
RESULTS: Acceptable agreement of FVIII and FVIII inhibitor levels were obtained with

BACKGROUND: Hemophilia A (HA) is an X-linked monogenic disorder caused by deficiency of the factor VIII (FVIII) gene in the intrinsic coagulation cascade. The current protein replacement therapy (PRT) of HA has many limitations including short term effectiveness, high cost, and life-time treatment requirement. Gene therapy has become a promising treatment for HA. Orthotopic functional FVIII biosynthesis is critical to its coagulation activities.
METHODS: To investigate targeted FVIII expression, we developed a series of advanced lentiviral vectors (LVs) carrying either a universal promoter (E…

BACKGROUND: Deficiency in blood coagulation factor VIII (FVIII) results in life-threating bleeding (hemophilia A) treated by infusions of FVIII concentrates. To improve disease treatment, FVIII has been modified to increase its plasma half-life, which requires understanding mechanisms of FVIII catabolism. An important catabolic actor is hepatic low density lipoprotein receptor-related protein 1 (LRP1), which also regulates many other clinically significant processes. Previous studies showed complexity of FVIII site for binding LRP1.
OBJECTIVES: To characterize binding sites between FVIII and …

Myocardial infarction is a life-threatening emergency with a high mortality rate. A high plasma level of factor VIII is an established risk for both arterial and venous thrombotic events. In this mini-review, we report the case of a 41-year-old woman without cardiovascular risk factors or a previous history of thrombotic events, admitted for ST-elevation myocardial infarction, in whom coronary angiography showed a thrombotic occlusion in the left anterior descending artery. The patient underwent primary percutaneous coronary intervention (PCI), with GPIIB-IIIA antagonist, then, a pre-dilation …
