Disseminated amyloidosis presenting with right proximal femur pathological fracture in a haemodialysis end-stage renal failure patient.

Osteoarticular amyloidosis can be one of the musculoskeletal system manifestations related to dialysis. We share our experience in dealing with a case of disseminated haemodialysis-associated amyloidosis in a 74-year-old end-stage renal failure patient. This patient suffered from a serious complication, namely an unprovoked pathological fracture at the basal neck region of the right femur. Polyarticular cystic and lytic lesions over the contralateral proximal femur, bilateral proximal humerus and the right lunate were noted on further imaging. In view of extensive amyloidotic infiltration into

Ipsilateral femoral neck and shaft fracture in children: a report of two cases and a literature review.

Concomitant ipsilateral fractures of the neck and shaft of the femur in children are rare. The most recent report in this context found a total of only nine reported cases (<12 years of age) following a search of the indexed English literature. These injuries occur in children due to high-velocity trauma, and there is no generally accepted method of treatment. We report three additional cases from the literature and two cases of our own. In our cases, one had a residual 10° varus deformity at the subtrochanteric level in the femur, but this did not affect hip function. Another patient exhibite

Do Patients Taking Warfarin Experience Delays to Theatre, Longer Hospital Stay, and Poorer Survival After Hip Fracture?

BACKGROUND: Patients sustaining a fractured neck of the femur are typically of advanced age with multiple comorbidities. As a consequence, the proportion of these patients receiving warfarin therapy is approximately 10%. There are currently few studies investigating outcomes in this subset of patients. QUESTIONS/PURPOSES: The purpose of this study was to assess the association between warfarin therapy and time to surgery, length of hospital stay, and survival in patients sustaining a fractured neck of the femur. METHODS: Data for 2036 patients admitted to our center between July 2009 and Jul

Femur osteoid osteoma in children: are there location-dependent differences in MRI findings?

BACKGROUND: Osteoid osteomas are most commonly found in the femur and preferentially affect the pediatric population. Magnetic resonance imaging (MRI) findings of femoral osteoid osteomas are not well described. OBJECTIVE: To systematically characterize pretreatment MRI findings of clinically confirmed femur osteoid osteomas in children and determine location-dependent differences. MATERIALS AND METHOD: The current study included children (< 18 years) with clinically confirmed femoral osteoid osteomas, who underwent pretreatment MRI in the last 14 years (2009-2023). Two radiologists retrospe

Bone mineral density assessment using radiofrequency echographic multispectrometry (REMS) in patients before and after total hip replacement.

UNLABELLED: REMS-BMD by radiofrequency echographic multispectrometry is primarily determined by a patient's BMI, age, and sex. Only about 2.8% of the changes in femoral neck REMS-BMD can be attributed to replacement of the total hip with metal implants. PURPOSE: This study aimed to evaluate how total hip replacement affects femoral neck REMS-BMD and to examine the contributions of a patient's sex, age, and BMI to the REMS-BMD assessment. METHODS: REMS-BMD was measured at the proximal femur using EchoStation (Echolight Spa, Lecce, Italy) in 27 women and 23 men with advanced hip osteoarthritis

Evaluation of outcome of proximal femur locking compression plate (PFLCP) in unstable proximal femur fractures.

INTRODUCTION: Pertrochanteric fractures are most frequent factures of the proximal femur, accounts for nearly 50% of all proximal femur fractures and are most devastating and also a major cause of disability in elderly. The aim and objective of this study was to evaluate the role of proximal femoral locking compression plate in unstable proximal femur fractures. MATERIAL AND METHOD: A total of 20 cases were included in our study (M:F - 16:4), who suffered proximal femur fracture due to various modalities and all of them were operated using proximal femoral locking compression plate. This prox

The effect of femoral rotation on the projected femoral neck-shaft angle.

Femoral neck-shaft angle (NSA) was measured in a series of anteroposterior (AP) hip radiographs of a cadaveric femur in varying degrees of rotation. A mathematical model was developed to predict NSA on an AP radiograph in varying degrees of femoral rotation. The predictions of the model were found to correlate well with the experimental data (correlation coefficient = 0.94). Based on the mathematical model, a wide range of patient positioning was found to result in a <10 degrees error in the measurement of femoral NSA. Coxa vara, increased femoral anteversion, cerebral palsy, and developmental

Bilateral femoral retroversion associated with acetabular dysplasia. A case report.

Retroversion of the proximal femur is associated with a number of acquired conditions but is unusual in a congenital form. It is even more unusual to be associated with acetabular dysplasia. A 38-year-old woman with bilateral hip pain had roentgenographic evidence of acetabular dysplasia with valgus neck-shaft angles. Physical findings were consistent with femoral retroversion, and computed tomography demonstrated 28 degrees of retroversion on both sides. Symptomatic relief was obtained with bilateral varus internal rotation osteotomies of the proximal femur. To date, a case of retroverted fem

Damage to the capital femoral epiphysis due to Frejka pillow treatment.

Seven cases of damage to the capital femoral epiphysis in seven infants with congenital dysplasis, subluxation, or dislocation of the hip treated with a Frejka pillow splint are described. The pillow was first applied shortly after birth (up to four months of age) and was continued for three to six months. The roentgenographic changes in the femoral head and neck were consistent with either avascular necrosis of the femoral head or a growth disturbance causing alteration of the normal angle of the neck and shortening of the affected femur.

[Osteoarthritic changes in hip joint in patients with fractures of femoral neck].

BACKGROUND: Fractures of proximal femur are common among elderly people. They are associated with considerable morbidity and mortality. Identification of etiopathogenetic factors associated with fractures might facilitate prevention. Osteoporosis is commonly present in the heads of femurs. The prevalence of osteoarthritic changes in hip joints is controversial. Some authorities report low prevalence and even speculate on the protective effect of osteoarthritis against fractures. OBJECTIVE: The goal of the study was to examine the association between osteoarthritic changes (radiologic and hist

Mechanical strength of a femoral reconstruction in paediatric oncology: a finite element study.

In 1997 the proximal femur of a four-year-old child affected by a Ewing sarcoma was reconstructed using a massive bone allograft in conjunction with a vascularized fibula autograft. During the first three years of follow-up the reconstruction underwent important morphological changes. The aim of the present study was to evaluate the risk of fracture of the reconstructed proximal femur, once the physiological loads are restored, associated with a short, slow but unprotected level walk. Subject-specific finite element models of the operated femur, and of the intact contralateral one, were genera

Antegrade insertion of a retrograde intramedullary femoral nail for treatment of a subtrochanteric pathological femoral shaft fracture in a patient with pre-existing coxa vara.

Objective: We report the unorthodox use of a retrograde intramedullary nail via antegrade technique for the treatment of a subtrochanteric pathological femur fracture in the presence of significant coxa vara. No similar use has been documented previously in literature. Methods: A standard approach to the proximal right femur was performed and standard antegrade technique used to introduce a guide wire into the proximal femur after which the femoral shaft was reamed. A 38-cm long 10 mm diameter retrograde nail was attached to the retrograde insertion and targeting device as if to perform a left

Principles of Valgus Intertrochanteric Osteotomy (VITO) after Femoral Neck Nonunion.

Nonunion is a relatively rare, yet challenging problem after fracture of the femoral neck. Risk factors include verticality of the fracture line and presence of comminution of the posteromedial calcar, as well as quality of reduction. Treatment options consist of valgus intertrochanteric osteotomy versus arthroplasty. Treatment should be tailored to the individual patient, taking into account patient age and activity demands. This review outlines the principles and technical considerations for valgus osteotomy of the proximal femur in the setting of femoral neck nonunion.

Smaller femoral neck anteversion in varus knees than in healthy and valgus knees.

It is important to investigate anatomical differences of the femur and tibia three-dimensionally between varus and valgus knees to enhance surgical approaches and better understand structural factors related to specific patterns of osteoarthritis progression. Three-dimensional femoral and tibial bone models were reconstructed from transverse computed-tomography scans in varus osteoarthritis (43 knees), valgus osteoarthritis (40 knees), and healthy (32 knees) groups. Different coordinate systems were defined in each femoral bone model ("Knee" and "Hip" Coordinate System). Femoral neck inclinati

Angle estimation of human femora in a three-dimensional virtual environment.

The estimation of human femur morphology and angulation provide useful information for assisted surgery, follow-up evaluation and prosthesis design, cerebral palsy management, congenital dislocation of the hip and fractures of the femur. Conventional methods that estimate femoral neck anteversion employ planar projections because accurate 3D estimations require complex reconstruction routines. In a recent work, we proposed a cylinder fitting method to estimate bifurcation angles in coronary arteries and we thought to test it in the estimation of femoral neck anteversion, valgus and shaft-neck

Dynamic parallel traction theoretical model for the application and validation in femoral neck fractures - a finite element analysis.

STUDY OBJECTIVE: This study aims to validate the application effects of a novel theoretical model of dynamic parallel traction in the treatment of femoral neck fractures through three-dimensional finite element analysis. By simulating the femoral neck fracture model, we explore the promotional effect of dynamic parallel traction on fracture healing. METHOD: A digital 3D femur model was constructed using high-resolution computed tomography data of the lower limbs of a 70-year-old elderly subject. An axial compression of 500N was applied at different traction angles (0°, 10°, 20°, 30°, 40°, 50°

Long-term bisphosphonate use and femoral morphological changes in osteoporosis patients: a retrospective cohort study.

OBJECTIVES: We examined the association between long-term bisphosphonates use and morphological change in the femur. METHODS: 140 patients (97.1% female, 70.8 ± 8.7 years) with over 5-year BP use (8.1 ± 2.4 years) were matched 1:1 for age (± 3), same BMD status with patients as a control group. The primary outcome was femoral lateral bowing (FLB). The hip-knee-shaft angle (HKSA) and femoral neck shaft angle (FNSA) were secondary outcomes. RESULTS: Compared with the Control group, the femoral lateral bowing and hip-knee-shaft angle (5.9 ± 4.3° vs. 3.7 ± 1.7°; 6.6 ± 2.6° vs. 5.1 ± 1.5°, respec

Osteosynthesis with the hook-pin in slipped capital femoral epiphysis.

Osteosynthetic materials, such as metallic nails, screws, pins, and bone pegs, used in the surgical treatment of slipped capital femoral epiphysis, have caused peroperative problems because of increased displacement of the femoral head and postoperative problems because of bone resorption and growth of the femoral neck, resulting in loosening of the osteosynthetic material and reslipping of the femoral head. Premature closure of the growth plate and shortening of the femoral neck have also been registered. In order to avoid these problems, a hook-pin was developed. This device has now been in
