Adults with Fetal Alcohol Spectrum Disorder: Factors Associated with Positive Outcomes and Contact with the Criminal Justice System.

BACKGROUND: Adults with FASD are at increased risk for contact with the criminal justice system (CJS). To date, there has been limited research devoted to development of supports for adults with FASD and it is unclear what supports are required to improve outcomes and reduce CJS contact. OBJECTIVES: To examine the services and supports experienced by a small group of adults with FASD living in both rural and urban locations in Ontario, and their contact with the CJS. METHODS: A sample of 14 individuals with FASD and 11 support persons participated in the study. Semi-structured interviews wer

Modeling Fetal Alcohol Spectrum Disorder: Validating an Ex Vivo Primary Hippocampal Cell Culture System.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is the leading nongenetic cause of mental retardation. There are no treatments for FASD to date. Preclinical in vivo and in vitro studies could help in identifying novel drug targets as for other diseases. Here, we describe an ex vivo model that combines the physiological advantages of prenatal ethanol (EtOH) exposure in vivo with the uniformity of primary fetal hippocampal culture to characterize the effects of prenatal EtOH. The insulin signaling pathways are known to be involved in hippocampal functions. Therefore, we compared the expressio

An economic evaluation of the parent-child assistance program for preventing fetal alcohol spectrum disorder in Alberta, Canada.

Parent-Child Assistance Program (P-CAP) is a 3-year home visitation/harm reduction intervention to prevent alcohol exposed births, thereby births with fetal alcohol spectrum disorder, among high-risk women. This article used a decision analytic modeling technique to estimate the incremental cost-effectiveness ratio and the net monetary benefit of the P-CAP within the Alberta Fetal Alcohol Spectrum Disorder Service Networks in Canada. The results indicate that the P-CAP is cost-effective and support placing a high priority not only on reducing alcohol use during pregnancy, but also on providing

Using telehealth for assessment of fetal alcohol spectrum disorder: the experience of two Canadian rural and remote communities.

INTRODUCTION: Telehealth has been used for fetal alcohol spectrum disorder (FASD) diagnostic assessment in select Manitoban communities since 2000. OBJECTIVE: The purpose of this study was to evaluate the FASD telehealth program within two rural and remote Northern Manitoban communities by comparing community practices from the perspective of professionals working with the FASD diagnostic clinics in these communities. Recommendations for the further development of FASD assessment by telehealth were made to further improve current implementation and guide expansion of the FASD telehealth progr

Diagnosis of fetal alcohol spectrum disorder (FASD): fatty acid ethyl esters and neonatal hair analysis.

Measuring levels of fatty acid ethyl esters (FAEE) in hair has been recently shown to discriminate between adult heavy and non-drinkers. Here, we review the potential of neonatal FAEE measurement in detecting infants exposed to alcohol in utero by outlining current progress in the development of a neonatal hair test for the diagnosis of fetal alcohol spectrum disorder (FASD). Developing a reproducible, accurate and predictable hair test for FAEE measurements in neonatal hair may prove to be a powerful tool in the detection of in utero alcohol exposure which is needed for the diagnosis of FASD.

Neural speech encoding in fetal alcohol spectrum disorder: an exploratory study using frequency-following responses.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is associated with neurodevelopmental impairments, including listening difficulties not always explained by peripheral hearing loss, suggesting alterations at the level of neural auditory processing. The frequency-following response (FFR) provides an objective measure of neural speech encoding and may offer insight into auditory function in this population. METHODS: Twenty-five normal-hearing participants were included: 11 individuals with FASD and 14 controls. Speech-evoked FFRs were recorded using a 160 ms /da/ stimulus at 80 dB SPL with a

Overcoming barriers to care for individuals with fetal alcohol spectrum disorder: challenges and solutions in New Mexico and beyond.

Fetal Alcohol Spectrum Disorder (FASD) encompasses a range of neurodevelopmental impairments caused by prenatal alcohol exposure, affecting cognitive, behavioral, and social functioning. Despite its high prevalence, FASD remains underdiagnosed, particularly in regions like New Mexico (NM), where high rates of alcohol use, poverty, and unplanned pregnancies exacerbate the burden of the disorder. Individuals with FASD often face significant challenges in adaptive functioning, education, and social integration, with many experiencing adverse childhood events that compound developmental difficulti

Analysis of risk factors associated with suicidality in children and adolescents with fetal alcohol spectrum disorder in Western Australia.

BACKGROUND: Individuals with fetal alcohol spectrum disorder (FASD) are at an elevated suicide risk compared with those in the general population. This public health issue warrants further research attention to help inform the development of prevention and intervention efforts. Our study is the first to characterize suicidality (i.e., suicidal ideation/suicide attempt) and explore associated risk factors in young individuals with FASD within the Australian context. METHODS: Retrospective file reviews from a diagnostic clinic in Western Australia obtained data on demographic variables and risk

Neuroprotective Effects of Erythropoietin in Fetal Alcohol Spectrum Disorder: BDNF Upregulation, Reduced Astrogliosis, and Anxiety Relief.

OBJECTIVE: The term fetal alcohol spectrum disorder (FASD) refers to a variety of negative consequences that may develop in children born to women who have consumed alcohol during pregnancy. Anxiety disorders are common in FASD. Animals exposed to alcohol exhibit anxiety-like traits. Erythropoietin (EPO) is produced by the kidneys and liver. The synthesis of EPO by immature neurons also plays a crucial role in the embryonic stage. In addition, EPO inhibits astrogliosis and cell death while increasing the number of brain-derived neurotrophic factor (BDNF)-positive cells, which is linked to redu

Psychosocial, neurocognitive, and physical development in Eastern European adopted adolescents with and without fetal alcohol spectrum disorder.

BACKGROUND: Most internationally adopted individuals have been exposed to an array of developmental risk factors, including early institutionalization and other adverse experiences. Adopted children from Eastern Europe tend to show worse neurodevelopmental outcomes than adopted individuals from other areas of origin. Previous studies have shown a high incidence of prenatal alcohol exposure and fetal alcohol spectrum disorders (FASD) among this population, a factor that may explain some of their complex needs. However, direct comparisons of adoptees (from Eastern Europe or elsewhere) with and w…

Altered neuronal network connectivity in children with fetal alcohol spectrum disorder and its association with inhibitory function.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is associated with widespread neurocognitive deficits, including impairments in executive function, attention, and inhibitory control. However, understanding of the neural mechanisms underlying these deficits in young children 6-8 years of age remains limited. This study investigated functional connectivity (FC) alterations in key brain networks related to inhibitory control and executive function in children with FASD compared to typically developing controls (TDC). METHODS: Seed-based connectivity (SBC) analysis was conducted in 27 children

Rethinking Local Economic Development for Fetal Alcohol Spectrum Disorder in Renosterberg Local Municipality, South Africa.

Two towns in Renosterberg Local Municipality (RLM) in the Northern Cape Province of South Africa, Petrusville and Philipstown, have high Fetal Alcohol Spectrum Disorder (FASD) prevalence rates. FASD is linked to poverty and imposes high national economic costs. Thus, it is critical to understand the local economic development (LED) strategies used to mitigate the high prevalence of FASD. Moreover, there is sparse literature examining adult communities where FASD children reside. Understanding these adult communities is important because FASD cannot exist without adult gestational exposure to a

Knowledge, attitudes, and practices of fetal alcohol spectrum disorder in health, justice, and education professionals: A systematic review.

BACKGROUND AND AIMS: Fetal alcohol spectrum disorder (FASD) is one of the most common forms of developmental disability, and yet, anecdotally, is poorly understood by both the public and professionals across health, justice, education, and social services. This review aims to understand the knowledge, attitudes, and practices of professionals who work across a range of sectors - specifically health, education and justice - where they may encounter people with FASD, their families and caregivers. METHOD: We conducted a systematic search for research using surveys or questionnaires to address k

Diagnosing intellectual disability in people with fetal alcohol spectrum disorder: A function of which diagnostic manual is used?

Fetal alcohol spectrum disorder (FASD) is the largest known cause of intellectual disability (ID), and forensic experts are often called upon to determine if a defendant with FASD qualifies for a diagnosis of ID. Whether such a diagnosis is made may depend upon the diagnosing expert's choice of diagnostic manual: guidelines published by the American Association on Intellectual and Developmental Disabilities (now in its 12th edition [AAIDD-12]) or the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Although both manuals use the same three diagnostic "prongs"

Four-year follow-up of a randomized controlled trial of choline for neurodevelopment in fetal alcohol spectrum disorder.

BACKGROUND: Despite the high prevalence of fetal alcohol spectrum disorder (FASD), there are few interventions targeting its core neurocognitive and behavioral deficits. FASD is often conceptualized as static and permanent, but interventions that capitalize on brain plasticity and critical developmental windows are emerging. We present a long-term follow-up study evaluating the neurodevelopmental effects of choline supplementation in children with FASD 4 years after an initial efficacy trial. METHODS: The initial study was a randomized, double-blind, placebo-controlled trial of choline vs. pl

Difficulties in Daily Living Experienced by Adolescents, Transition-Aged Youth, and Adults With Fetal Alcohol Spectrum Disorder.

BACKGROUND: Individuals with fetal alcohol spectrum disorder (FASD) experience a range of problems in their cognitive, affective, and physical functioning following prenatal alcohol exposure (PAE), in addition to multiple complex difficulties in daily living that impact well-being. Using the Canadian National FASD Database, we sought to profile a range of difficulties in daily living, along with risk factors, in a large cross-sectional cohort of adolescents, transition-aged youth, and adults with PAE, of which a subset was ultimately diagnosed with FASD. METHODS: We summarized data for 9 curr

Detection of Children/Youth With Fetal Alcohol Spectrum Disorder Through Eye Movement, Psychometric, and Neuroimaging Data.

Background: Fetal alcohol spectrum disorders (FASD) is one of the most common causes of developmental disabilities and neurobehavioral deficits. Despite the high-prevalence of FASD, the current diagnostic process is challenging and time- and money- consuming, with underreported profiles of the neurocognitive and neurobehavioral impairments because of limited clinical capacity. We assessed children/youth with FASD from a multimodal perspective and developed a high-performing, low-cost screening protocol using a machine learning framework. Methods and Findings: Participants with FASD and age-mat…

The past, present, and future of fetal alcohol spectrum disorder work in Newfoundland and Labrador: A landscape paper for change.

OBJECTIVES: In this paper, we provide an overview of best practices in FASD prevention, diagnostic, and interventions and supports. In Canada, people diagnosed with Fetal Alcohol Spectrum Disorder (FASD) represent a fraction people living with FASD. While social stigma may deter people from seeking an FASD diagnosis, other deterrents include the lack of screening and diagnostic referrals, cost of travelling to a clinic, and lack of clarity of how a diagnosis may improve supports and services. Preventing FASD and improving lifelong outcomes for people living with FASD requires a coordinated app
