In 3935 patients who underwent upper fiberpanendoscopy in two Zurich hospitals the coincidence of different active peptide lesions has been investigated. There was an association of reflux esophagitis and duodenal ulcer in old age: in patients with reflux esophagitis aged over 60 years, duodenal ulcer was found twice as often as in elderly patients without esophagitis. No association of reflux esophagitis and gastric ulcer or of gastric ulcer and duodenal ulcer was observed.

We report the case of a patient with functioning bronchial carcinoid tumor found as a solitary nodule. Acute carcinoid syndrome developed during transbronchial biopsy, and life-threatening hypotension occurred during operation. We emphasize that carcinoid tumors should be considered in the differential diagnosis of solitary nodules, and such a complication should be kept in mind when endobronchial or transbronchial biopsy is performed.

A photoluminescent thermometer, based on the transient emission response of a europium-activated calcium sulphide phosphor, is described in detail. This optical thermometry system has special advantages for temperature measurements in microwave and RF fields and potential application in electromagnetically induced clinical hyperthermia. A laboratory system has been constructed which utilises a fibreoptic probe with external diameter 0.8 mm. A system temperature resolution of 0.1 degree C has been achieved in the range 37-47 degrees C.

Endoscopic sphincterotomy (EST) has been used as a treatment of 33 patients with bile duct stones and 3 patients with benign papillary stenosis. In 83% the papilla was cannulated and an adequate, 15 mm long, sphincterotomy was performed. In 72% the retained bile duct stones were extracted. All 3 patients with papillary stenosis had a free bile flow to the duodenum after the procedure and no complications were seen. Complications occurred in 4 patients (11%) with bile duct stones. The complications consisted of pancreatitis in 2 patients, bleeding in one patient and cholangitis in one patient.

Fiberscopic examination was carried out to investigate the condition of the stoma and bile duct and to assess the postoperative effects of the procedure on the upper gastrointestinal tract in 15 patients who received external side-to-side choledochoduodenostomy. The stoma remained patent, with a round or oval shape, in all 15 patients. Residual sutures were found at the stoma in 4 of 15 patients, 3 of whom had inflamed stomal mucosa. In 306 patients, fiberpanendoscopy and radiology were performed within a time interval of 7 days or less. In 196 cases, radiology was performed before endoscopy and in 110 cases after endoscopy. In cases with histologically unproven divergent findings, follow-up studies were performed. Fifty-five patients were investigated using transbronchial lung biopsy through the flexible fibreoptic bronchoscope under fluoroscopic guidance, 51 by the transnasal route under local anaesthesia, two via an endotracheal tube whilst undergoing assisted ventilation and two by the same route under general anaesthesia. Multiple specimens of lung tissue subjects with diffuse pulmonary disease and in 16 of 32 (50%) with localized lesions, giving a positive yield of 64% of all cases. The development of plastic fiberoptic catheters, cheaper and less fragile than glass ones, has enabled the more widespread use of oxyhaemoglobin saturation (SO2) monitoring. They allow direct determinations of SO2 to be made, using reflection spectrophotometry. The purpose of the present work was to evaluate the accuracy of SO2 measurements as provided by these catheters. The studies were performed in dogs, with large variations of SO2, acid-base balance and haematocrit levels.
