Accounting for variations in ART program sustainability outcomes in health facilities in Uganda: a comparative case study analysis.

BACKGROUND: Uganda implemented a national ART scale-up program at public and private health facilities between 2004 and 2009. Little is known about how and why some health facilities have sustained ART programs and why others have not sustained these interventions. The objective of the study was to identify facilitators and barriers to the long-term sustainability of ART programs at six health facilities in Uganda which received donor support to commence ART between 2004 and 2009. METHODS: A case-study approach was adopted. Six health facilities were purposively selected for in-depth study fr

Effectiveness of a conditional cash transfer programme on TB cure rate: a retrospective cohort study in Brazil.

BACKGROUND: Despite the efforts of the National Tuberculosis Programme, TB cure rates in Brazil are sub-optimal. The End TB Strategy for post-2015 identifies conditional cash transfer interventions as powerful tools to improve TB control indicators, including TB cure rate. This study aims to inform the new policy by evaluating the role of the Bolsa Familia Programme (BFP), one of the largest conditional cash transfer programmes in the world, on TB cure rates in Brazil. METHODS: We undertook a retrospective cohort study, based on an unprecedented record linkage of socioeconomic and health data

Analysis of deaths among children in the period 1996-2008 from closed claims registered by the Danish Patient Insurance Association.

AIM: We investigated the death circumstances among children in the Danish healthcare system by analysing closed claims. METHODS: This retrospective study investigated closed claims with regard to medically related deaths registered by the Danish Patient Insurance Association. RESULTS: From 1996 to 2008, 45 953 claims were made to the Danish Patient Insurance Association (DPIA) covering all medical specialties. Among these claims, a total of 3531 submitted claims were for children younger than 18 years old, and 74 of these children were registered as having died. Forty-one of the 74 deaths we

Impact of subsidizing effective anti-osteoporosis drugs on compliance with management guidelines in patients following low-impact fractures.

Early in 2000, proven-effective antiresorptive drugs (alendronate and raloxifene) were included in the national "health basket" in Israel. We carried out the present study to evaluate the effect of subsidizing antiosteoporosis drugs on the use of antiosteoporosis drugs in patients following low-impact fractures. The rates of dispensation of antiosteoporosis drugs, in the hospital and in the community, before and after an incident of a newly diagnosed low-impact fracture, respectively, were evaluated during January and February 1998 and 1999 ("pre-basket") and the corresponding months of 2000 a

Home care in New York City: providers, payers, and clients.

Estimates based on 1980 data indicate that almost two-thirds of the disabled elderly in New York City receive care only from informal sources. Another one-fifth use a combination of informal and formal sources of care. Thus, only about 15 percent are exclusively dependent on various agencies and programs that constitute the formal home care system. Over 200 organizations in 1986 offered some form of home care service in New York City. Among these were at least 100 proprietary businesses, 61 nonprofit personal care agencies, and 36 nonprofit certified home health agencies. In 1984, 166,739 diff

Facilitators and barriers to blood and blood product accessibility and use in sub-Saharan Africa: a systematic review.

BACKGROUND: Access to safe blood and blood products is a global health priority, requiring concerted public health efforts to ensure universal access to reduce the global disease burden. However, there are inequities in access to safe and adequate blood and blood products in sub-Saharan Africa (SSA), leading to marked challenges in meeting the clinical needs of patients with diverse pathologies. This study used the WHO health system building blocks to systematically synthesize the literature to understand the barriers to blood and blood product usage in SSA. METHODS: We used the PRISMA guidel

'Only systems thinking can improve family planning program in Pakistan': A descriptive qualitative study.

BACKGROUND: Family Planning (FP) program in Pakistan has been struggling to achieve the desired indicators. Despite a well-timed initiation of the program in late 50s, fertility decline has been sparingly slow. As a result, rapid population growth is impeding economic development in the country. A high population growth rate, the current fertility rate, a stagnant contraceptive prevalence rate and high unmet need remain challenging targets for population policies and FP programs. To accelerate the pace of FP programs and targets concerned, it is imperative to develop and adopt a holistic appro

Beyond Training: Systems Framework for Sustainable Health Informatics Investment in Africa.

Across Africa, substantial investment has built national health information systems (HISs), including the surveillance platforms, reporting tools, and digital infrastructure through which health data flow. However, the health informatics capacity needed to sustain those systems remains fragile: trained health informaticians leave for better-resourced organizations, platforms fall into disrepair when donor funding ends, and data systems multiply without connecting. Health informatics is the discipline that designs, governs, and sustains HISs; the two are inseparable, and investment that builds

What health reforms promise and what evaluations measure: A comparative policy analysis of reform evaluation in six predominantly tax-funded high-income countries.

BACKGROUND: Predominantly tax-funded high-income countries are undertaking large-scale reforms to address demographic, workforce and fiscal pressures, digital transformation and fragmented care. Reforms in Australia, England, Canada, Denmark, Norway and Sweden emphasise integration, primary and community care, and sustainability, but their evaluation alignment remains unclear. OBJECTIVE: To compare alignment between national reform objectives and evaluation models across six predominantly tax-funded high-income systems, identifying where reform promises translate into measurable progress. Facility-Level Access Drives Disparities in Influenza and Pneumococcal Vaccination in Long-Term Care Facilities in Southern Poland.

BACKGROUND: Vaccinations prevent severe respiratory infections in older adults, yet uptake in Polish long-term care facilities (LTCFs) remains poorly characterized. We assessed influenza and pneumococcal vaccination coverage and factors associated with uptake, including the influence of local government financing. METHODS: In this prospective observational study (January-June 2022), residents aged ≥65 years from eight LTCFs in southern Poland (four public, four private) were evaluated. Clinical data and geriatric assessments (Barthel Index, ADL, FRAIL-NH) were obtained from medical records an

Barriers and enablers in implementing integrated primary health care: A qualitative assessment in West Sumbawa District, Indonesia.

In 2023, Indonesia introduced an integrated primary health care (IPHC) model called Integrasi Pelayanan Kesehatan Primer (ILP) as part of the national primary care transformation agenda. ILP seeks to strengthen the network of public primary care providers across sub-district, village, and community levels with a five-cluster model organized around a life-cycle approach, replacing the previously fragmented, program-based model. West Sumbawa District (WSD) in West Nusa Tenggara Province was chosen as a national pilot district by the Ministry of Health (MOH), with ILP implementation expanding fro

Conservation-relevant lands in Australia have higher farmland values.

Agricultural land conversion is a major driver of global environmental degradation, contributing to habitat loss for threatened species and climate change when carbon-rich native vegetation is replaced with crops. Conversely, agricultural production remains a necessary land use to ensure food security, support regional employment, and generate foreign income. Hence, conservation interventions and nature-based solutions must compete with-and be balanced against-agricultural production, with careful consideration of the opportunity costs associated with provisioning land from agriculture to envi

Dialysis deserts in the Philippines: a national geospatial modeling study of dialysis services.

BACKGROUND: In the Philippines, dialysis facilities are clustered in urban centers, leaving patients with Kidney Failure (KF) in rural and geographically isolated regions severely underserved. Despite a national peritoneal dialysis (PD)-first policy since 2014, a decade of implementation has not closed this gap, and no national analysis has examined whether the existing organization of dialysis services drives geographic inequities or how integrating PD could reduce them. OBJECTIVE: To assess the geographic accessibility of dialysis services and estimate potential coverage gains from integrat
