Congenital duplication of the foot and toes.

Congenital duplication of the foot and toes is a commonly diagnosed condition in newborn nurseries across the United States. The foot involvement varies from simple fleshy tags to complex attempts at whole extremity duplication. We reviewed 59 patients (82 feet) with a variety of duplications (excluding skin tags). During our review, we devised a new descriptive system of classification of congenital duplications of the foot based primarily on the type and complexity of the skeletal duplication. Variables related to sex, bilaterality, age at surgery, family history, and follow-up results were …

[Eumycetic mycetoma of the foot (maduromycosis)].

An eumycetoma pedis presenting as a circumscribed nodular form of maduromycosis in a 26-year old man from the People's Republic of Yemen is described. Clinical and light microscopic aspects of this "imported" deep mycosis are discussed. Histologic finding of a septate mycel permitted characterization of the microorganism as a eumycota.

Talectomy for club foot in arthrogryposis.

Talectomy was performed on 10 patients (15 feet) for club foot deformity in arthrogryposis multiplex congenita. These were reviewed after an average follow-up of eight years. At follow-up nine feet were plantigrade, and six had less than 15 degrees residual equinus at the ankle. All the feet were asymptomatic but had mild residual adduction of the forefoot and marked stiffness of the hindfoot. Seven feet developed spontaneous bony ankylosis in the tibiotarsal joint. The common technical errors were incomplete removal of the talus and incorrect positioning of the calcaneus in the ankle mortise.

Ciliated cutaneous cyst of the foot.

We report a 36-year-old woman with a cutaneous cyst of the dorsal aspect of the left foot. Because of the symptoms of pain and paresthesias attributed to the lesion, it was clinically felt to be a ganglion. After excision, these symptoms abated. Yet histologic examination showed this lesion to be a rare tumor, the ciliated cutaneous cyst. This is the third patient with this distinctive lesion on the foot or ankle.

Amebiasis in red-footed tortoises.

Of 500 red-footed tortoises imported to southern Florida, approximately 200 died during a 2-month period. Clinical signs were nonspecific and included anorexia, listlessness, and watery diarrhea, with lingering death. Necropsy consistently revealed thickened duodenum, with necrotic mucosa and multifocal to diffuse areas of hepatic necrosis. Histologic evaluation of tissues demonstrated numerous amebae in intestinal and hepatic lesions.

Office care of the insensitive foot.

Diabetes is the most common cause of foot insensitivity. Its treatment requires in depth education of the patient in the cause, effects, and danger signs of the disease. Circulatory studies are outlined with particular emphasis on the use of the ultrasonic Doppler in monitoring the course of any foot lesions particularly with regard to their healing.

[The rheumatoid foot. Origin of deformations].

Deformations of the foot are a logical and predictable function of the biomechanics of the foot and the constraints undergone by the articulations of the foot, that are unstabilized by the inflammatory process. They result from the combination of three factors : anevolutive teno-articular synovitis, predictible forces (the weight of the extrinsic muscle, the anti-physiological foot), and the congenital morphotype of the foot. Typical deformations (peroneal " coup de vent " of the toes, triangular metatarsus), differ on the clinical level in keeping with the morphotype but respond to the same m…

[The rheumatic foot and its treatment].

The alterations of form and function of the rheumatoid foot are derived from the pathomechanical conditions. The latter are used as a measure for the treatment. Type and frequency of inflammatory alterations are studied by x-ray examination and new aspects are stressed. The deformities of the rheumatoid foot can be explained conclusively through the interaction of functional, gravitational, inflammatory and anatomical influences. The deformities of the toes are induced to a great extent by the deformities of the proximal part of the foot. The possibilities of surgical treatment are discussed i…

Hemophilic arthropathy of the foot and ankle.

The major deformities in hemophilic arthropathy of the foot and ankle fall into the three groups of equinus, varus, and cavus. These pathologic positions develop through a repetitive pattern of intra-articular and intramuscular bleeding within the area of the distal calf, foot, and ankle. Appropriate infusion therapy with factor VIII concentrate and factor IX concentrate plus splinting, bracing, and logical rehabilitative maneuvers can delay or prevent the advent of permanent deformity. The actual articular damage seems to be directly related to the release of digestive enzymes from leukocytes…

The foot in chronic rheumatoid arthritis.

The feet of 200 consecutive admissions with classical or definite rheumatoid arthritis were studied. 104 were found to have pain or deformit … elected group of patients they were all under specialist care. The increased expenditure on special footwear would benefit the patient, firstly by improving ambulation, and secondly perhaps by reducing the number of operations necessary. Hallux valgus was very common and occurred with similar frequency to disease in the other metatarsophalangeal joints …
