Onychomycosis is a common disease, and there are a number of factors that may affect the duration and dosage of treatment including the type of onychomycosis, the area and thickness of nail involvement, the age of the patient, and the location of the digit that is affected. We report a composite index, the Scoring Clinical Index for Onychomycosis (SCIO) that combines these factors to give an index of the overall severity of onychomycosis. The use of the SCIO may have treatment implications; by matching patients with similar SCIO scores, it may be possible to better compare the clinical respons

Myrmecia are viral warts that result from the coalescence of plantar or palmar warts into large plaques. Treatment of these warts involves physical or chemical destruction of the verrucae, potent keratolytics or immunotherapy. Imiquimod 5% cream is a novel topical immunomodulator that has been used successfully in the treatment of genital and common warts. We report its successful use in a 35-year-old immunocompetent man who had had resistant plantar warts for 15 years.

Two cases of tinea pedis due to Scytalidium hyalinum, the first to be described in Italy, are reported. The patients were a 41-year-old woman and a 35-year-old man who had spent periods in the Caribbean. The clinical manifestations were indistinguishable from those caused by dermatophytes. In the women they were striking with 'moccasin foot' type lesions, whereas in the man they were less evident, with minor plantar desquamation and interdigital maceration. Diagnosis was based on direct mycological microscopic examination and culture. Clinical and mycological remission were obtained with syste

The structure of the weight-bearing plantar skin and the subcutaneous cushion is adapted to mechanical loads of 3-10 kp/cm2 under walking conditions. Such loads, however, are only tolerated when compensated by constant alternation of loaded points. If the surgical therapy with free skin grafts or pedicled flaps does not take into account the vascular and neural architecture of the plantar skin, healing without problems may result--but only for a short time. As a rule, such grafting is later followed by painful keratotic lesions or non-healing "trophic" ulcers, which are caused by loss of sensi

In a group of 84 adult male workers in an industrial plant, who used a common shower room, the applicability of the adhesive tape-KOH technique for routine mycologic examination was investigated. The tape technique was compared with the conventional cover slip technique in interdigital and plantar lesions. The tape technique gave more frequent positive results than the cover slip technique (74% and 57% respectively). However, examination of the tapes was more difficult than that of the cover slips. The latter method therefore appears more appropriate for routine microscopical mycologic examina

The first case of subcutaneous zygomycotic infection caused by Saksenaea vasiformis in a rice mill worker from India is described. The infection, confined to the man's left foot, showed multiple draining sinuses, inflammation, and intermittent low-grade fever following a crushing injury when a log fell on his foot. Histopathologic examination of two biopsy specimens, taken at 3-wk intervals, revealed the presence of broad, sparsely septate, branched, hyaline hyphae characteristic of a zygomycete. When they were grown on a nutritionally deficient medium, two cultures isolated from the biopsied

The first case in Uruguay of 'tinea nigra' is described in a 44-year-old male patient with a maculous pigmented lesion on the right foot. It represents the most meridional case of the disease yet recorded in South America. Exophiala werneckii was isolated in cultures (strain 1905 IHM).

Infestation by the flea Tunga penetrans, although quite prevalent in Central America, South America, and Africa, is rarely recognized or reported in North America. This report describes a case of tungiasis occurring in a New York resident who acquired the disease in Africa. The patient manifested characteristic lesions of white papules with central black pits on the dorsa of her toes. Treatment consisted of removal of the offending parasites.

Report on two women suffering from chronic painful ulceration of the feet combined with interdigital synechia, loss of nails, alopecia and oral lesions of lichen planus. Hint at certain clinical parallels of dystrophic skin symptoms with epidermolysis bullosa hereditaria dystrophica yet demonstrating different mode of blister formation.

We present a case report of a man who had an abscess-like lesion on his left foot. The lesion discharged pus, enlarged, and became thicker, so that we could not differentiate the real toes from the out-growths. Moraxella phenylpyruvica and Pseudomonas putrefaciens were identified; direct agglutination tests were postive M. phyenylpyruvica. He was given antibiotics and antituberculosis therapy and had topical treatments. The foul odor disappeared with treatment, and the lesions were removed surgically 3 months later. The M. phenylpyruvica finally disappeared after the surgery. The final conditi

Erythermalgia is a distinctive clinical entity characterized by local redness, warmth, and burning pain in the distal portions of the extremities. In this study, erythemalgia was observed in two patients with myeloproliferative disease. A new theory suggesting abnormally rapid platelet aggregation as a cause of erythermalgia was advanced. The following evidence is presented: (1) histologic evidence of thrombi occluding the arterioles and probably composed of platelet aggregates; (2) reversal of the clinical findings with platelet aggregating drugs, such as heparin sodium and acetylsalicyclic a
