Treatment of Lisfranc fracture-dislocations with primary partial arthrodesis.

BACKGROUND: The optimal method of treatment for Lisfranc fracture-dislocations remains controversial, and the role of primary partial arthrodesis for combined osseous-ligamentous Lisfranc injuries is unclear. This study reviewed the outcomes of Lisfranc injuries treated by primary partial arthrodesis. METHODS: Patients who underwent primary partial arthrodesis for a primarily ligamentous or combined osseous and ligamentous Lisfranc fracture-dislocation were reviewed retrospectively and assessed at followup according to radiographic, clinical and standardized patient-based outcomes. Twenty-fiv

Secondary correction of traumatic "dislocation orbito-nasale" fractures.

Dr. Tessier often stated that the correction of well-established posttraumatic deformities was one of the most difficult. Yet consistently and repeatedly, he was able to achieve magnificent results especially for fractures of the orbitonasal region. The principles that he taught are highlighted in this article.

Fracture of the humeral neck with intrathoracic dislocation of the humeral head.

Intrathoracic fracture-dislocation of the humeral neck is a rare and easily overlooked clinical entity. We present a case and review of the literature. The objective of this study was to summarize the clinical presentation of this injury and to provide recommendations for initial diagnostic imaging and management. A 68-year-old man presented to the Emergency Department after a fall onto an abducted arm with moderate respiratory distress, mild left shoulder pain, and palpable crepitus over the shoulder and chest wall. Radiographic examination demonstrated the humeral head to be lying within the

[Surgical treatment of old dislocated condylar process fractures].

OBJECTIVE: To analysis the result of surgical treatment on old dislocated condylar process fractures. METHODS: 22 cases of old dislocated condylar process fractures with a duration of more than 3 weeks (average 4.6 weeks) were treated by free and reimplantation of the dislocated condylar process. The postoperative result was evaluated on the basis of occluding relation and joint function, as well as radiographic assessment of condylar changes. RESULTS: Normal occluding relation was resumed in 19 cases after the operation. And there were 19 cases showing that the process was replaced in norma

Transarticular fixation with the capacity for motion in fracture dislocations of the elbow.

Post-traumatic stiffness of the elbow joint is a frequent result of immobilisation leading to severe disability in the use of the upper extremity. Recognition of the tendency to stiffness leads to the assumption that the strong self-healing forces of the capsule and ligament apparatus converts the initial instability of the joint after ligament disrupture, into a high-grade undirected stability following immobilisation. Directed stability as it is produced by the natural ligament apparatus of the joint on the other hand produces a guided movement of the joint in one direction. These theoretica

Adjacent fracture-dislocations of the lumbosacral spine: case report.

OBJECTIVE AND IMPORTANCE: Traumatic fracture-dislocations of the lumbosacral junction are rare, with all previously reported cases involving fracture-dislocations at a single level. No cases of multiple fracture-dislocations of contiguous spinal segments in the lumbosacral spine have been reported. A case of traumatic adjacent fracture-dislocations of the fifth lumbar segment is presented. CLINICAL PRESENTATION: An 18-year-old male patient sustained open lumbar spinal trauma after a motor vehicle accident. A neurological examination revealed an L4 level. Radiographic evaluation of the spine r

Injury classifications and operative approaches in hip dislocation and fractures.

During the last few decades, traumatic injuries of the hip joint have significantly increased in number, and traffic accidents have also increased. Depending on the speed, direction of the forces and the position of the femur at the moment of impact, different patterns of injury occur. Basically, they are classified as hip dislocations, dislocation fractures and acetabular fractures. These injuries have in common a high rate of concomitant lesions. Several classification systems have been developed for these injuries. Commonly, Stewart and Milford's or Levin's classification is used for disloc

[T-plate osteosynthesis in dislocated proximal humerus fractures].

From 1988 through 1995 we operated 48 patients with dislocated humeral fractures using the T-Plate (median age 66 years [20 to 90 years], 32 male, 16 female). After a median 22 months (9 to 60 months) 33 patients were followed up clinically and radiologically. Six patients had suffered a 2-part fracture, 17 a 3-part fracture, and 10 a 4-part fracture. Judged by the Constant score 6 patients reached a very good result, 8 had a good result, 4 a satisfactory and 15 a poor result. Radiologically we saw an arthrosis in 31%, an axial deviation in 50%, and a dislocation of the fragments in 72%. The r

Intermediate-term experience of Pipkin fracture-dislocations of the hip.

The purpose of this study was to review our intermediate-term experience with posterior fracture-dislocations of the hip. We evaluated the accuracy of the Pipkin and Brumback classification schemes as predictors of outcome, assessed the importance of certain treatment variables, and determined the complication rates of this injury in a group of 33 patients followed for an average of 49 months. The overall results were 67% good, 18% fair, and 15% poor, with no excellent results. The Pipkin classification scheme was a useful predictor of outcome because patients with less severe Pipkin 1 or 2 in

Early motion in the treatment of fractures and dislocations in the hand and wrist.

The concept of early motion is valuable in the treatment of fractures of the hand and wrist. It is a means to an end, however, and should be employed principally when the benefits outweigh the risks. The goal is a hand that functions as near to normally as possible, considering the injury. All treatments need to be tailored to the individual patient.

Surgical treatment of the fractured and dislocated condylar process of the mandible.

Most fractures of the mandible can be managed conservatively. This report is a retrospective evaluation of the clinical and radiological results in 17 patients with 21 dislocated fractures submitted to open reduction and fixation, employing steel wires and maxillomandibular fixation. Follow-up ranged from 7 to 55 months, (mean 29.5). Functional assessment showed good opening movements (mean 41.9 mm). There were no cases of ankylosis, pain, or paralysis of the facial nerve. Radiological assessment was normal when the lateral pterygoid muscle was maintained adherent to the fractured proximal seg

Patterns of hand fractures and dislocations in a district general hospital.

In a retrospective survey of patients with fractures and dislocations attending the Accident department of a District General hospital, comparison was made between patients with fractures and dislocations in the hand and fractures and dislocations of other sites. Patients with hand injuries accounted for 28% of the total patients seen. They were more likely than other fracture patients to be male and between the ages of 10 and 40 years. Almost a quarter of hand injuries attended on Monday. Leisure activities outside the home formed the commonest aetiological factor. Only 66% of patients with h

Shoulder dislocations associated with a displaced fracture of the humeral head or glenoid require different treatment than shoulder dislocat … is no possibility of the small fragments healing due to their placement on the joint cartilage. In fracture dislocations, the blood supply of the humeral head is seriously jeopardized if the fracture is situated in the anatomical neck, whereas this is not the case in a fracture of the surgical neck. The number of displaced fragments allows a prediction concerning the …

[Vascular injuries as a complication of fractures, dislocations and surgical interventions].

This is a presentation of 34 patients with traumatic or intraoperative vascular injuries treated at the Rural Clinic am Plattenwald during the timeperiod from 1978 until 1988. Limb amputation was performed in three cases. Two patients died. Our goal is to show the importance in early recognition and treatment of such injuries which requires an efficient interdisciplinary cooperation between orthopedic and vascular surgeons, as well as the radiologist. The employment of angiography should be generous.

Cotrel-Dubousset instrumentation for reduction of fracture dislocations of the spine.

The utilization of the Cotrel-Dubousset (CD) instrumentation for reduction of fracture dislocations of the spine requires special techniques unique to this new rod/hook configuration. A rotational manipulation of the CD rod with special application of a "fulcrum" hook is essential to reduce and maintain reduction in fracture dislocations of the spine.

Vascular injury associated with fracture-dislocations of the lower extremity.

Vascular damage occurring in conjunction with orthopedic injury of the lower extremity is uncommon. This relative rarity is further complicated by the potentially subtle nature of vascular injury. Doppler signals and even palpable pulses do not exclude vascular damage. Missed or delayed diagnosis with a subsequent avoidable amputation is a too-frequent result. Displaced fractures of the distal femur and proximal tibia and particularly dislocation of the knee are associated with the highest risk of concomitant vascular injury and the poorest collateral circulation to support the distal limb. [Fracture types and fragment dislocations of the scaphoid bone of the hand].

Fractures of the scaphoid bone are nowadays better known and treated. However, the types of fractures and fragment dislocations are not well described. The authors analysed 325 fresh and old scaphoid fractures and describe six basic types, clearly defined by location and direction of the fracture line. In fresh cases, the dislocation is mainly rotational. In older cases there is an additional bone resorption. The six basic types of fracture lines are demonstrated radiologically and the course of untreated cases is shown.
