Biomechanical comparison of internal fixation techniques for the treatment of unstable basicervical femoral neck fractures.

OBJECTIVES: The optimal method of internal fixation of basicervical femoral neck fractures is controversial. This area represents a transition zone between the intracapsular femoral neck fracture, usually fixed with multiple cancellous screws, and the extracapsular interochanteric fracture, fixed with a sliding screw device [sliding hip screw (SHS)] and derotation screw (DRS) device. The authors' specific aim was to biomechanically compare these two methods of fixation in a cadaveric model of a basicervical femoral neck fracture with posteromedial comminution. DESIGN: The authors compared the

Displaced scapular fractures: indication and long-term results of open reduction and internal fixation.

Displaced scapular fractures are often found in polytraumatized patients. In emergency treatment they assume a minor role. Advances in dealing with severely injured patients in most instances allow us to perform an operation on the fractured scapula within the first 2 weeks after injury. A differentiated approach is necessary as exclusively conservative treatment does not always bring about good results. From 1981-1991 we performed open reduction and internal fixation (ORIF) in 25 patients with displaced fractures of the scapula. The long-term results could be assessed in 20 patients after an

Alignment of supracondylar/intercondylar fractures of the femur after internal fixation by AO/ASIF technique.

Fifty-nine supracondylar-intercondylar fractures of the femur in 57 patients were evaluated after a mean follow-up of 5 years 7 months (range 2 years to 11 years 3 months) after internal fixation using AO/ASIF technique. Axial alignment was compared with that of the uninjured side by orthoroentgenography in the upright position and by bilateral anteroposterior (AP) and lateral views of the femur. Identical values for varus/valgus were noted in 24%, for ante/recurvation in 72%, and for rotation in 61%; differences were within 5 degrees of varus/valgus in 74%, of ante/recurvation in 78%, and of

Immediate internal fixation of open, complex tibial plateau fractures: treatment by a standard protocol.

A retrospective review of 46 consecutive patients with complex (Schatzker V and VI) tibial plateau fractures treated at Harborview Medical Center between 1984 and 1989, disclosed a subset of 14 grade II or III (Gustilo) open injuries. We wished to determine the incidence of infection, union rate, and the number of operations required to achieve a satisfactory result, based on a treatment protocol: alignment and splinting of fracture at the scene of injury if possible, antibiotics administered in the emergency room (ER) and continued for 48 h, and admission of patient to the operating room as q

Open reduction and internal fixation of acetabular fractures: heterotopic ossification and other complications of treatment.

Open reduction and internal fixation was performed on 50 displaced acetabular fractures in 49 patients by nine different attending surgeons over a 10-year period. At an average follow-up of 38 months, poor results were noted clinically in 38% and radiographically in 40%. The incidence of short- and long-term complications was greater than in other studies. Particularly distressing was the 58% incidence of heterotopic ossification (HO). Twenty-four percent had grade III or IV; five hips were autofused and the remainder had 40-60% loss of motion. There was no correlation of HO with age, sex, fra

The internal skeletal fixation system. A new treatment of thoracolumbar fractures and other spinal disorders.

A new internal skeletal fixation system (ISFS) for stabilizing thoracolumbar fractures and other spinal disorders was developed by Dick in Switzerland. The ISFS is a modification of the Magerl external skeletal fixation system of the spine. The Dick modification consists of a threaded rod connected by clamps to Schanz screws and is located in the vertebral pedicles on both sides of the spine. Due to a stable angle between the rod and Schanz screw, a one-point fixation above and below the diseased vertebra is sufficient. The ISFS provides a short fixation and promotes fusion. Control of lordosi

A reproducible approach to the internal fixation of adult ankle fractures: rationale, technique, and early results.

1. Open treatment of ankle fractures is discussed. 2. It is our opinion that operative treatment affords the most predictable way to carry out an absolutely anatomic reduction of the fractures. In addition, the joint may be inspected, cartilage damage documented, and loose fragments removed. 3. A logical method of fixation has been discussed. It is stressed that the fibula should be fixed anatomically first and then associated instabilities corrected. 4. A series of cases with a relatively short follow-up period has been presented showing excellent results and a low complication rate when this

Anterior subcutaneous internal fixation for a complex vertical shear pelvic fracture with Morel-Lavallée lesion: a case report.

A 32-year-old male sustained a complex vertical shear pelvic fracture after a motor vehicle accident, associated with hemodynamic instability and a large Morel-Lavallée lesion involving the anterior pelvis and right thigh. Initial management included external fixation and percutaneous sacroiliac screw placement as part of damage-control orthopedics. Two weeks later, anterior subcutaneous internal fixation (INFIX) was performed to provide anterior pelvic stability while limiting further soft-tissue dissection. Postoperative imaging showed satisfactory reduction and implant positioning. The INFI

Internal fixation vs manual reduction in the treatment of ankle fracture healing and inflammation: A case-control study.

Ankle fractures are among the most common bone injuries, which are often accompanied by soft tissue injuries. Proper management of these fractures is crucial to promote healing and minimize complications. This study explores the effects of 2 treatment methods for ankle fractures: open reduction and internal fixation and manual reduction followed by plaster external fixation. A retrospective analysis was conducted on 124 patients with ankle fractures admitted between March 2020 and September 2022. Patients were divided into 2 groups: 62 received internal fixation and 62 received manual reductio

Risk factors for nonunion in femoral neck fracture patients with internal fixation: A multicenter (TRON group) retrospective study.

INTRODUCTION: Femoral neck fractures (FNFs) are common in elderly individuals. When minimally displaced, they are typically treated with internal fixation. Nonunion is a complication of internal fixation of FNF, that sometimes necessitates reoperation. Radiographic parameters, including the Pauwels angle and posterior tilt angle, are risk factors for nonunion. However, these parameters are assessed solely in a two-dimensional context. We developed a novel radiographic parameter named the "Kindex." This multicenter investigation aimed to identify risk factors for nonunion following FNF fixation

Primary Arthrodesis versus Open Reduction and Internal Fixation for Lisfranc Joint Fracture-Dislocations in Adults: A Systematic Review.

BACKGROUND: The Lisfranc joint is an intricate podiatric medical structure that when injured can prove difficult to treat. No consensus has been established on optimal surgical management for this injury. It is widely debated whether open reduction and internal fixation or primary arthrodesis provides better outcomes for patients. Although literature has been published on this subject, no generalized guidelines have been created. The goal of this study was to analyze high-level meta-analyses to draw conclusions about surgical interventions for Lisfranc joint injuries. METHODS: A literature re

Open Reduction and Internal Fixation of a Type IV Capitellum Fracture Through a Transolecranon Tunnel: A Case Report.

CASE: A 60-year-old woman sustained a type IV capitellum fracture from falling on an outstretched arm. Open reduction internal fixation (ORIF) was performed using an anconeus approach, and a transolecranon tunnel was created to place a trochlear screw. The patient showed good clinical outcomes with almost full range of motion at 6 months. CONCLUSION: With type IV capitellum fractures, the olecranon often obstructs the screw trajectory necessary for anterior-to-posterior fixation of trochlear fragments. Drilling a transolecranon tunnel through the proximal olecranon with the elbow flexed creat
