[Long-term results of surgical management of patellar fractures. Conservative versus resection procedures].

In a retrospective analysis of 70 patellar fractures the-long-term results after surgery were followed-up for periods of up to twelve years. Subjectively, excellent or satisfactory results were reported by 89% (n = 62) of patients. Clinical evaluation in the Lysholm score showed excellent or good results in 74.2% (n = 52) of cases. The results in the Lysholm score correlated well with the radiological evidence of arthrosis (p < 0.014) and with the clinical signs of chondropathy (p < 0.0005). Poor results (79.2 Lysholm points) were seen for comminuted fractures; however, the results for patient

The Role of External Fixation in Trauma and Reconstruction of the Mandible in the Age of Rigid Fixation.

Internal rigid fixation is the gold-standard treatment for facial fractures, but there are some specific cases that are more amenable to external fixation (ex-fix) application. Herein, we discuss advantages and disadvantages to ex-fix in the modern treatment of comminuted mandible fractures, infected mandible fractures, fractures of the condylar region, oncologic mandibular resection, pediatric mandible fractures, and fractures in the edentulous patient.

The Initial Characteristics of Paediatric Supracondylar Humerus Fractures Led to Cubitus Varus Deformity and Delayed Displacement.

Background: Supracondylar humerus fractures are the most common type of elbow fracture in children, with a variety of complications such as cubitus varus deformity. The most important goal of the initial treatment is to avoid complicated deformities. In the present study, we investigated cubitus varus deformity and discussed the ideal initial treatment for supracondylar humerus fractures. Methods: There were 58 conservative cases (26 girls and 32 boys) and 115 operative cases (40 girls and 75 boys) with supracondylar humerus fractures. We evaluated the correlation between the following factors

Surgical results of zones I and II fifth metatarsal base fractures using hook plates.

The purpose of this study was to evaluate the results of surgical treatment of fifth metatarsal base fractures using a mini-hook plate. Seventeen patients with Lawrence classification zones I (n=6) and II (n=11) fifth metatarsal base fractures with an initial fracture displacement more than 2 mm and a small (less than 2 mm) comminuted avulsion fragment were included in the study. Patients treated using a mini-hook plate fixation method were prospectively evaluated. A mini-hook tubular plate was designed so that the last hole functioned as a hook for the application of compression force, graspi

Percutaneous cerclage wiring technique for phalangeal fractures.

Comminuted fractures of phalanx are challenging to reduce properly. Commonly, the fragments are distracted by various ligaments as well as flexor and extensor tendons. Splints, skeletal traction, Kirschner wire, plate and screws, and cerclage wires have all been used to manage such fractures. Among these, the cerclage wires are able to effectively contain the distracting forces and maintain a proper concentric reduction of the hollow tube structure of phalangeal bones. The resulting tissue-hardware construct is stable and allows early rehabilitation. Despite the obvious mechanical advantages,

Can we predict long-term sequelae after fractures of the clavicle based on initial findings? A prospective study with nine to ten years of follow-up.

The aim of this long-term, prospective study was to identify risk factors associated with the outcome of clavicular fractures. During 1989 through 1991, 245 patients aged 15 years or older with a radiographically verified fracture of the clavicle were included. Clinical and radiographic examinations were standardized. Of the 208 patients seen at the 9- to 10-year follow-up, 112 (54%) had recovered completely whereas 96 (46%) still had sequelae. Nonunion occurred in 15 patients (7%). No bony contact was the strongest radiographic predictor for sequelae. Comminuted fractures with transverse frag

Minimization of zygomatic complex fracture treatment.

The aims of this non-randomized prospective study were to establish and justify minimized therapy for zygomatic complex fractures. Fifty-two consecutive patients were examined and classified with conventional routine radiographs. Preoperative symptoms were recorded. Treatment of zygomatic fractures was by percutaneous hook reduction and miniplate fixation along the frontozygomatic suture. Exploration of the orbital floor was carried out only in cases of primary diplopia or comminuted fractures. Postoperatively, patients were followed for 12 months. Clinical and radiologic assessment of reducti

Radiographic analysis of tibial fracture malalignment following intramedullary nailing.

Intramedullary nailing of the tibia was performed on 145 tibiae (137 patients) for fracture or nonunion from 1985 to 1992. There were 133 cases available for radiographic analysis of postoperative tibial alignment. Of the 133 nailings, 16 (12%) were malaligned (12 acute fractures and 4 nonunion-malunions). Malalignment was defined as 5 degrees angulatory deformity in any plane. Malalignment was seen in 58% of proximal third fractures, 7% of middle third fractures, and 8% of distal third fractures. Of the malaligned fractures, 83% were either segmental or comminuted. Thirteen percent of the rea

Ipsilateral proximal femur and shaft fractures treated with hip screws and a reamed retrograde intramedullary nail.

BACKGROUND: Although not common, proximal femoral fractures associated with ipsilateral shaft fractures present a difficult management problem. A variety of surgical options have been employed with varying results. QUESTIONS/PURPOSES: We investigated the use of hip screws and a reamed retrograde intramedullary (IM) nail for the treatment of this combined fracture pattern in terms of postoperative alignment (malunion), nonunion, and complications. METHODS: Between May 2002 and October 2011, a total of 95 proximal femoral fractures with associated shaft fractures were treated at three particip

[Instability of fractures of the lower end of the radius: apropos of a series of 167 cases].

One-hundred-sixty-seven consecutive cases of distal radius fracture have been treated in our department from October 1985 until November 1987. All cases were rated according to anatomical criteria defined on the first radiogram. These criteria were the presence of a dorsal tilt greater than 20 degrees, dorsal comminution, intra-articular radiocarpal involvement, an associated fracture of the ulna and comminution of the distal radius. Age greater than 60 years was also considered as a factor in determining severity. With regard to fractures treated conservatively (from which comminuted fracture

[Guidelines: treatment of distal radius fracture].

The treatment aim is restoration of adequate length of the radius, adjustment of the distal radioulnar joint, alignment of the distal radius joint surface, and prevention of reflex dystrophy syndrome. Stable fractures are treated by close reduction and a cast for 3-4 weeks, depending on bone stock quality. Instable extra-articular fractures are treated by K-wiring using various methods. In case of a comminuted dorsal or palmar area, K-wiring is unstable, so additional fixation is necessary (autogeneous bone graft + external fixator + plate or cast). The external fixator alone or in combination

[Therapy for radial head fractures].

The treatment of radial head fractures is challenging for the trauma surgeon due to the complex functional anatomy of the elbow. Satisfactory results can only be achieved if the function and stability of the humeroradial joint are entirely restored. Thus, depending on the fracture type, ranging from non-displaced Mason type I fractures to comminuted Mason type III fractures, an individual treatment concept has to be established. Treatment protocols range from conservative treatment with short-term cast immobilization to open reduction and internal fixation according to AO-principles or to the

Plate fixation versus intramedullary nailing of completely displaced midshaft fractures of the clavicle: a prospective randomised controlled trial.

AIMS: This is a prospective randomised controlled trial comparing the functional outcomes of plate fixation and elastic stable intramedullary nailing (ESIN) of completely displaced mid-shaft fractures of the clavicle in the active adult population. PATIENTS AND METHODS: We prospectively recruited 123 patients and randomised them to either plate fixation or ESIN. Patients completed the Quick Disabilities of the Arm, Shoulder and Hand (DASH) score at one to six weeks post-operatively. They were followed up at six weeks, three and six months and one year with radiographs, and their clinical outc

Reconstructive and rehabilitation challenges following a cranio-orbital gunshot wound.

We present a case of a 26-year-old man who sustained a close-range gunshot wound to the head. His injuries included significant left orbital injury resulting in a ruptured, blind eye and severely comminuted fractures of the left orbital roof, superior and inferior orbital rims, and orbital floor. Associated injuries included left frontal lobe injury, anterior and posterior table fractures of the left frontal sinus, and a comminuted left zygomaticomaxillary complex fracture. We employed an interdisciplinary surgical approach with collaboration among the Otolaryngology, Neurosurgery, and Oculopl

Acromial approach for treating glenoid fractures: A report of two cases and a literature review.

Surgery is usually recommended for displaced glenoid fractures, with open reduction and internal fixation as the standard operative treatment. Three approaches have been recommended in the reduction of glenoid fractures: Anterior, posterior and combined; however, a traditional approach may be difficult due to a high position or a comminuted Ideberg type III fracture. The combined approach results in a longer incision and more soft tissue injury when associated with an acromial fracture. The present study describes two complicated glenoid fractures: One case was a comminuted Ideberg type III fr

Posteromedial Approach for Open Reduction and Internal Fixation of Talar Process Fractures.

UNLABELLED: Fractures of the posteromedial process of the talus are frequently the result of high-energy trauma to the lower extremity. The treatment of these uncommon injuries may be unfamiliar and challenging to many surgeons. Significant pain and disability can result if these injuries are not recognized or treated appropriately. Open reduction and internal fixation via a posteromedial approach with screws is a mainstay of operative treatment for simple fractures. In cases of fracture impaction and/or comminution, a medial uniplanar external fixator may be used to improve intraoperative fra

[Hot topics and challenges in the treatment of Pilon fractures].

Pilon fractures are comminuted fractures of the distal tibial articular surface caused by high-energy axial loading, typically resulting from falls from height, motor vehicle accidents, or sports injuries. The patients with Pilon fractures are often associated with articular cartilage damage, metaphyseal comminution, and severe soft tissue injury, which can lead to poor long-term prognosis. Pilon fractures are among the most challenging types of fractures to treat. At present, there are still some controversies regarding the treatment of Pilon fractures. This article aims to explore hot-topic

Preoperative radiography versus computed tomography for surgical planning for ankle fractures.

PURPOSE: To review preoperative radiography and computed tomography (CT) of the ankle in 69 patients who underwent surgery for ankle fractures to determine the value of CT in diagnosis and surgical planning. METHODS: Preoperative radiography and CT of the ankle of 46 women and 23 men aged 17 to 90 (mean, 48.8) years were reviewed. CT was deemed necessary when radiographs showed the following features: (1) comminuted fracture of the medial malleolus involving the tibial plafond, (2) comminuted fracture of the posterior malleolus, (3) presence of loose bodies, and/or (4) suspected Chaput or Vol
