Thyroid pathology is a specialist area but is often encountered by the general pathologist in a variety of forms including cytology, frozen sections, and resection specimens. In the thyroid gland, as for other endocrine organs, many aspects of diagnosis are unique to this area of histopathology; thus, the aims of this paper are to set out best practice guidelines which, although not entirely comprehensive, will be of practical use.

The clinical features, diagnostic work-up and difficulties in establishing a diagnosis on endoscopic biopsy specimens were retrospectively analysed in five patients with ampullary tumors. In addition, the technique of local papillary resection is reported. Indications for local resection are: elderly patients with severe co-existing diseases, patients with benign ampullary tumors which are either adenomatous or show epithelial dysplasia or patients with inconclusive preoperative endoscopic biopsy or peroperative frozen sections.

A patient with a long history of celiac sprue developed a pleomorphic intestinal lymphoma. Cell suspensions of tumor cells and immunoperoxidase labeling of cell surface antigens in frozen tissue sections clearly showed the lymphoma to be of T-cell origin, despite the presence of other nontumor cells bearing enzyme markers of histiocytes. These findings may have important implications on the cellular origin of lymphomas developing in patients with celiac sprue.

Sarcocystosis was diagnosed in an aborted bovine fetus. Immature and mature schizonts of Sarcocystis were disseminated in the vascular endothelium of all organs, but especially the brain. Microscopic granulomas, focal gliosis, and petechial hemorrhages in the neuropil were scattered in the brain. Multifocal collections of mononuclear cells were observed in the kidney, liver and heart. Organisms in sections of frozen tissues were demonstrated by immunofluorescent techniques to be Sarcocystis.

Histochemical assay for ornithine transcarbamylase (OTC) activity in fixed frozen hepatic sections from a woman heterozygous for OTC deficiency revealed two populations of hepatocytes: those with normal activity and those with no activity. This observation, in conjunction with data from previous family studies, confirms the hypothesis that the gene for OTC is X-linked. It also provides the first cytologic demonstration of cellular mosaicism for a liver-specific cell product.

A simple technique using immunoperoxidase-labelled antibodies is reported for the ultrastructural localization of fixed immunoglobulins in tissue lesions. We wanted to find out whether ultrasound (US) can be used to assess the deep resection margins after excision of squamous cell carcinoma (SCC) of the tongue, as intraoperative feedback on their condition might help to prevent them being too close. AIM: The authors investigated the diagnostic value of intraoperative assessment of myometrial invasion in endometrial cancer patients. Following hysterectomy, the uterus was sectioned and macroscopically examined in order to assess the depth of myoinvasion, which was classified as <50% and >50%. In patients with macroscopic depth of invasion>30% and <50%, a frozen section of this area was carried out. The results of intraoperative evaluation were compared with the results of postoperative pathological examination. The agreement between methods was developed as generalized Kappa type statistic.
