MALDI-TOF-MS for rapid detection of staphylococcal Panton-Valentine leukocidin.

Toxin-producing gram-positive bacteria are responsible for emerging and life-threatening infections in humans worldwide. Both rapid toxin detection and adapted therapy are essential to limit the morbidity due to such toxins, especially staphylococcal Panton-Valentine leukocidin (PVL). Here we describe the use of a mass spectrometry profile generated by matrix-assisted laser desorption/ionisation time-of-flight (MALDI-TOF) followed by ClinProTools 2.0 software analysis to find a reproducible model able to identify PVL in Staphylococcus aureus strains. Eighty-one S. aureus strains were used and

[Spinal disorders and associated CNS anomalies - tethered cord and Arnold-Chiari malformation].

Tethered cord syndrome (TCS) and the Chiari malformation (CM) are generally related to congenital malformations, but both entities can be acquired.TCS often presents with progressing sensory and motor symptoms that can be attributed to increased tension of the spinal cord. Fixation of the spinal cord can occur congenitally (primary TCS) or in association with other intraspinal pathologies or postoperative scarring (secondary TCS). Exact diagnosis in the adult can be difficult if symptoms are interpreted as being related to degenerative disorders of the spine. In the presence of neurological sy

[Endocrine disruptor compounds and their role in the developmental programming of the reproductive axis].

Different perturbations during fetal and postnatal development unleash endocrine adaptations that permanently alter metabolism, increasing the susceptibility to develop later disease, process known as "developmental programming." Endocrine disruptor compounds (EDC) are widely spread in the environment and display estrogenic, anti-estrogenic or anti-androgenic activity; they are lipophilic and stored for long periods in the adipose tissue. Maternal exposure to EDC during pregnancy and lactation produces the exposure of the fetus and neonate through placenta and breast milk. Epidemiological and

The natural histories of bone dysplasias in adults--vignettes, fables and just-so stories.

The bone dysplasias are a heterogeneous group of disorders arising from intrinsic abnormality of bone and cartilage growth and function. All are genetic. Most result in extreme small stature (dwarfism). Historically, emphasis was primarily on diagnostic identification of specific disorders in infants (including differentiating lethal and non-lethal forms), and on the clinical history to be anticipated in infants and children with each of these specific processes. Even in children there is exceedingly limited information of quality and virtually no controlled studies of the effects of intervent

Repeated visual hallucinations in Parkinson's disease as disturbed external/internal perceptions: focused review and a new integrative model.

Visual hallucinations (VH) in Parkinson's disease (PD) are a chronic complication in 30 to 60% of treated patients and have a multifaceted phenomenology. Flickering, faultive impressions, and illusionary misperceptions precede the core syndrome of stereotyped, colorful images. The patient variably recognizes these images as hallucinations, being rarely irritated or frightened and more often amused as a bystander. Although studies on VH in PD focus on several research domains, no comprehensive, unified theory has been developed to study their pathophysiology. We have adapted Hobson's work on th

Children, sleep, and behavior: a complex association.

Pediatric sleep physiology begins with development of the sleep/wake cycle, and the origins of active versus quiet sleep. The 24-hour circadian cycle becomes established at 3-6 months. Sleep disorders during infancy commonly include mild, usually self-limited conditions such as sleep-onset association disorder, excessive nighttime feedings, and poor limit-setting. These require behavioral management to avoid long-term deleterious sleep habits. In contrast, other sleep disorders are more ominous, including SIDS, central congenital hypoventilation syndrome, and sleep apnea. Childhood is generall

How endolymph pressure modulates semicircular canal primary afferent discharge.

Histological observation of endolymphatic hydrops in subjects suffering from Meniere's disease/syndrome and the presence of vestibular symptoms in experimentally induced endolymphatic hydrops have led to considerable debate regarding the potential role of inner-ear pressure. Could a condition influencing endolymphatic volume regulation lead to transient changes in translabyrinthine pressure sufficiently large to alter primary afferent discharge rates? To investigate this question we built a highly sensitive laser-based pressure sensor and recorded endolymphatic pressure modulations within the

Panic disorder among Vietnamese refugees attending a psychiatric clinic: prevalence and subtypes.

This study surveys Vietnamese refugees attending two psychiatric clinics to determine both the prevalence of panic disorder (PD) as well as panic attack subtypes in those suffering PD. A culturally valid adaptation of the SCID-panic module (the Vietnamese Panic Disorder Survey or VPDS) was administered to 100 Vietnamese refugees attending two psychiatric clinics. Utilizing culturally sensitive panic probes, the VPDS provides information regarding both the presence of PD and panic attack subtypes during the month prior to interview. Of 100 patients surveyed, 50 (50%) currently suffered PD. Amon

Late direct and transneuronal effects in mice with targeted expression of a toxin gene to D1 dopamine receptor neurons.

Detailed analysis of a novel transgenic model of basal ganglia disease has been undertaken. In this model the expression of an attenuated form of the diphtheria toxin gene was tightly controlled by D1 dopamine receptor regulatory domains. The behavioral and both direct toxin-mediated and transneuronal effects observed in pups in the first postnatal week have been described. Although younger pups are bradykinetic, older pups have a hyperkinetic syndrome with gait abnormality, postural instability and myoclonic jerks typical of human basal ganglia diseases such as Huntington's disease. As expect

The scapula: principles of construction and stress.

According to current generally accepted theories, analysis of the shape of a bone provides clues to the stresses acting upon it. Although many investigations have ensured that understanding of the lower limb is relatively advance, knowledge relating to the upper limb, and particularly to the scapula, is still far from complete. We have therefore endeavoured to interpret the shape of this bone morphometrically. To this end various scapular parameters have been standardised and statistically analysed, and the results examined both from the functional and the clinical points of view. Our investig

This technique was derived from Nissen-Rossetti's fundoplication operation. Anatomical bases for the method are described which allow the simple performance of a regulated and measured version (in spite of regional anatomical variations), that is non-calibrating (to avoid exposure to conventional sequelae of fundal plication) and reproducible (essential when comparing results assessed for global series). Technical features are described in detail and discussed, while emphasizing the importance of the following rules; respect of regional fascial structures, avoidance of vagal trunks, observatio

Justo Gonzalo (1910-1986): a pioneer of brain dynamics.

The present work offers an overview of the pioneering contributions of the neuroscientist Justo Gonzalo to the study of the human cerebral cortex, within the historical context of his time and in relation to current research. Gonzalo initially trained under Gonzalo Rodríguez Lafora (a disciple of Santiago Ramón y Cajal) and always maintained a close relationship with him and his circle, connecting him to the Spanish Neurological School (Cajal's school). He also trained in neurology in Austria and Germany (1933-1935). The research he called brain dynamics began during the Spanish Civil War (193

"Internal carotid artery-type cervical spondylosis" is not supported as an independent subtype: a perspective on diagnostic classification and clinical differentiation.

Cervical spondylosis is a common degenerative disorder, yet its diagnostic boundaries may be inappropriately expanded when nonspecific symptoms such as dizziness coexist with age-related cervical imaging changes. This Perspective evaluates the hypothetical concept of "internal carotid artery-type cervical spondylosis" and whether it qualifies as an independent clinical subtype. Using an adapted validity-utility framework, the proposed entity was assessed according to five criteria: a specific anatomical relationship, a reproducible pathophysiological mechanism, a recognizable clinical phenotyp

Treatment of patent foramen ovale.

After extensive debate, the percutaneous closure of patent foramen ovale (PFO) has been established as a first-line treatment for the secondary prevention of PFO-related stroke in patients between 18 and 60 years old, whereas the role of PFO closure for primary prevention remains controversial. Additionally, in selected cases, PFO closure may be considered beyond these age limits and for other indications such as the treatment of systemic deoxygenation syndromes and the secondary prevention of systemic embolism or decompression sickness, when the PFO has been determined to be causative in the
