Generalized peripheral nerve failure during thoracic spine surgery: a case report.

OBJECTIVE: Intra-operative systemic changes impairing peripheral nerve function are not commonly detected with electrophysiology. This case presentation illustrates how somatosensory evoked potential (SSEP) monitoring can detect global changes in peripheral nerve excitability during spine surgery. METHODS: A posterior thoracic spine fixation was performed on a young male with multiple traumatic injuries. Bilateral tibial nerve SSEPs were intraoperatively recorded, along with the right median nerve SSEP for control. RESULTS: A rapid, progressive loss of tibial and median nerve potentials (fol

American Association for the Surgery of Trauma emergency general surgery guideline summaries 2018: acute appendicitis, acute cholecystitis, acute diverticulitis, acute pancreatitis, and small bowel obstruction.

In April 2017, the American Association for the Surgery of Trauma (AAST) asked the AAST Patient Assessment Committee to undertake a gap analysis for published clinical practice guidelines in emergency general surgery (EGS). Committee members performed literature searches to catalogue published guidelines for common EGS diseases and also to identify gaps in the literature where guidelines could be created. For five of the most common EGS conditions, acute appendicitis, acute cholecystitis, acute diverticulitis, acute pancreatitis, and small bowel obstruction, we found multiple well-referenced g…

When the clock strikes eight: Nighttime admissions and worse outcomes of patients admitted with emergency general surgery conditions.

INTRODUCTION: Evidence on the impact of nighttime admission for emergency general surgery (EGS) in low- and middle-income countries is limited. We evaluated the association between admission timing and inpatient outcomes at a tertiary care center. METHODS: We analyzed 32,278 adult EGS admissions from 2010 to 2019, categorized as daytime (8:00 a.m.-7:59 p.m.) or nighttime (8:00 p.m.-7:59 a.m.). Inpatient mortality was the primary outcome. Multivariable logistic regression was used to assess adjusted associations. RESULTS: Nighttime admissions accounted for 42.4% of cases and were more likely …

Characterizing the general surgery workforce in Puerto Rico: A descriptive analysis of licensure, subspecialty distribution, and demographic characteristics.

BACKGROUND: Physician emigration has substantially reduced Puerto Rico's healthcare workforce. Comprehensive data characterizing the active general surgery workforce and surgeon-to-population ratios remain limited. METHODS: A cross-sectional descriptive analysis was performed using data from the Puerto Rico Medical Discipline and Licensure Board, supplemented by publicly available workforce sources. RESULTS: Of 315 licensed surgeons, 291 (92.4%) were actively practicing in Puerto Rico, yielding a density of 8.86 per 100,000 population. This estimate overstates the effective general surgery w…

Analgesic Efficacy of Bilateral Superficial Cervical Plexus Block Using Levobupivacaine vs Ropivacaine for Thyroid Surgery Under General Anesthesia.

Background Thyroidectomy is commonly associated with moderate postoperative pain, which may delay recovery and discharge. Bilateral superficial cervical plexus block (BSCPB) is an established component of multimodal analgesia for thyroid surgery. Levobupivacaine and ropivacaine are long-acting aminoamide local anesthetics with improved safety over bupivacaine, but comparative data on their efficacy for BSCPB in thyroidectomy remain limited. Aim This study aimed to compare, in a real-world government teaching hospital cohort, the postoperative analgesic efficacy and duration of analgesia of 0.2

A Narrative Review of Regional vs. General Anesthesia in Cancer Surgery: Immune Pathways and Long-Term Oncologic Outcomes.

The choice between general anesthesia (GA) and regional anesthesia (RA) for oncologic surgery has been widely debated because of the implications for perioperative safety, patient comfort, cancer recurrence, and long-term survival. RA may attenuate surgical stress and preserve immune function, whereas GA, particularly with volatile agents and opioids, may impair immune surveillance. Clinical findings, however, remain inconsistent. The most consistent signals of benefit have been observed in non-muscle-invasive bladder cancer undergoing transurethral resection of bladder tumors (TURBT), where o
