Gynecomastia (enlargement of the male breast) is a common occurrence during puberty and usually resolves. Poland's syndrome or hypoplasia of the chest wall is a rare congenital anomaly associated with hypoplasia of the ipsilateral extremity. Simultaneous presentation of gynecomastia and Poland's syndrome in a 26-year-old man increased the extent of asymmetry and the perception of each deformity. Surgical reduction of the enlarged breast greatly reduced the asymmetry, giving a satisfactory result.

Using Webster's technique for treating gynecomastia, the removal of the excess breast tissue is facilitated by the use of liposuction introduced through the axillary incision. In the case of pseudogynecomastia, only liposuction via the axillary approach is recommended.

Sixty-one Chinese men with idiopathic gynecomastia were treated with 40 mg of tamoxifen daily for one of four months (median, two months). Eighty percent had complete regression of their breast swelling. No long-term side effects of tamoxifen were observed over a median follow-up period of 36 months.

Four boys with persistent pubertal gynecomastia were given intramuscular dihydrotestosterone heptanoate (DHT-hp) at 2 to 4-week intervals for 16 weeks. By the end of treatment, breast size in all four boys had decreased 67% to 78%. Initial plasma levels of gonadotropins, estradiol, testosterone, and dihydrotestosterone (DHT) were normal. Mean plasma DHT concentration rose with the injections of DHT-hp, and remained elevated throughout the treatment period. Sex hormone profiles were studied in serum and tumor extracts of a man with pulmonary choriocarcinoma and gynecomastia. Although levels of serum estrogens were elevated as expected, serum androgen levels were uncharacteristically quite high. A modification of the standard periareolar incision is described. It provides better access for augmentation mammoplasty and gynecomastia, as well as allowing for correction of asymmetry in the size and siting of the nipple-areola complex. Finally, it achieves a very slight tightening of the skin envelope in cases of minor skin laxity.

Mammographic features of 22 pathologically proved cases reflecting disorders of the male breast are described.
