Computed tomography of the hand and wrist.

CT plays a limited but distinct role in the evaluation of the hand and wrist. It is most useful in assessing bone and joint anatomy and has only a secondary role in soft tissue abnormalities. The clearest value of CT is in assessing trauma and post-traumatic complications, such as malunion, nonunion, or avascular necrosis, where careful attention to position and technique can yield important information not otherwise obtainable.

Successful Triple Flap Procedure for Thumb Reconstruction in Severe Hand Crush Injury.

BACKGROUND The thumb is crucial for the aesthetic and functional aspects of the upper extremity. A crushed thumb injury can be particularly challenging, especially for individuals with high demands. Currently, there is no consensus on the best approach for treating a crushed thumb. The scapular flap, vascularized by the subscapular artery system, shows great potential as a free flap option. This report aims to highlight the use of a triple flap procedure to preserve the function of the crushed thumb. CASE REPORT A 47-year-old man had a severe injury to his left hand, resulting in significant s

MACRODYSTROPHIA LIPOMATOSA WITH ULNAR DISTRIBUTION IN HAND: MR EVALUATION OF A RARE DISORDER.

Macrodystrophia lipomatosa is a rare, non-hereditary congenital anomaly characterized by localised gigantism in the form of macrodactyly affecting a part of limb and rarely entire limb due to overgrowth of all mesenchymal elements. Radiological and pathological hallmark is the disproportionate fibroadipose tissue proliferation in subcutaneous tissue, nerve sheaths, and periosteum that lead to soft tissue and bony enlargement. We present the case of a twenty years old female who presented with history of gradual enlargement of the medial aspect of right hand along ulnar nerve distribution since

Aneurysmal bone cyst involving the metacarpal bone in a child.

Aneurysmal bone cysts associated with tubular bones of the hand occur rarely and require particular diagnostic and therapeutic management techniques. While optimal treatment has not been established, accepted treatments range from aggressive radical treatment, including en bloc resection and excision diaphysectomy with strut bone grafting, to relatively simple techniques, such as thorough curettage followed by bone graft. Aggressive treatment approaches may be optimal for the cases with articular surface involvement, full-bone invasion of the phalanx or metacarpal, or more than 1 recurrence. W

Diffuse Large B-Cell Lymphoma Infiltrating Limb Bones With a Few Lymphadenopathy Revealed on Whole-Body 18F-FDG PET/CT.

A 74-year-old man presented to our hospital for a 2-month history of worsening feet and left hand swelling pain. Computed tomography reconstruction image and sagittal images of the left foot demonstrated multiple and irregular bone destruction. Orthopedic surgeon suspected tuberculosis or primary skeletal malignancy. ¹⁸F-FDG PET/CT revealed bone destruction and abnormal activity mainly in bones of both upper and lower extremities and a few lymphadenopathy at the region of left axilla and groin. Diagnosis of diffuse large B-cell lymphoma was eventually made. Subsequently, the patient was transf

Scaphoid injuries in children.

In order to establish an acceptable working protocol for the care of children with scaphoid injuries, an independent study was undertaken by the author. This paper describes the various aspects of this study: understanding what the scaphoid is and its function, the correct examination of the hand and wrist and the rationale behind it, the rationale behind the current treatment of suspected scaphoid injuries, and the development of the carpal bones and skeletal age. Opinions differ as to what age the scaphoid develops and therefore when it may be fractured, and so a review of research and liter

In vivo bone mineral studies on volunteers during a 370-day antiorthostatic hypokinesia test.

Bone mineral of spine, femur, tibia, forearm, calcaneus, hand, and foot was studied on nine volunteers by quantitative computed tomography, neutron activation analysis, single and dual photon absorptiometry during and after long-term hypokinesia. Conditions of long-term space flight were simulated using a 370-day antiorthostatic hypokinesia test. It is shown that the highest mineral losses occur in foot bones including calcaneus. Remedial measures undertaken delay the process of osteoporosis development but do not completely exclude it. The results obtained by different methods are often confl

What happens to the double epiphysis in the hand?

We reviewed the x-ray films of 399 patients with open epiphyses and carefully examined the records of the 65 cases found to have extra epiphyses. Extra epiphyses did not appear or fuse early. Bone growth did not appear to be affected greatly with a double epiphysis, although fractured bones grew longer. Any long bone in the hand can have an abnormal epiphysis, but the anomaly occurs more often in the index and thumb metacarpals. Double epiphyses are more common among children with other congenital abnormalities. If a double epiphysis is present in a phalanx, some type of syndrome or other anom

[Circulation in bone in Paget's disease. General review and personal results].

On the basis of a review of work concerned with the circulation in bones affected by Paget's disease and from their own results, the authors show that the vascularization of affected by Paget's disease is completely different from that of normal bone. The periostal vessels are abnormally developed and result in a high flow in the affected limb. The cortical vessels are numerous and of all types, forming a system of canalization of considerable volume. On the other hand the medullary circulation is encumbered by stasis, oedema, and fibrosis. The earliness of the vascular lesions makes it possib

Unveiling Guyon's Canal: Insights into Clinical Anatomy, Pathology, and Imaging.

Guyon's canal, or the ulnar tunnel, is a critical anatomical structure at the wrist that houses the ulnar nerve and artery, making it susceptible to various pathological conditions. Pathologies affecting this canal include traumatic injuries, compressive neuropathies like ulnar tunnel syndrome, and space-occupying lesions such as ganglion cysts. Ulnar tunnel syndrome, characterised by numbness, tingling, and weakness in the ulnar nerve distribution, is a prevalent condition that can severely impair hand function. The canal's intricate anatomy is defined by surrounding ligaments and bones, divi

Presentation of malignancy by metastasis to the carpal navicular bone.

Although peripheral metastases of many malignancies to bone are common, metastases to the hand and carpus are rare. This is the first report of a silent primary malignancy of the lung presenting as a metastasis to the carpal navicular bone. Only eight instances of carpal bone metastases secondary to all tumor sources were revealed in a search of the literature. The presentation of metastatic disease in the hand in an occult malignancy may be deceptive, often mimicking pulp space infection, osteomyelitis, septic arthritis, gout, acute rheumatoid monoarticular arthritis, tenosynovitis, or sympat

Short stature, ulnar deviation of hands with absent carpals and joint contractures: a new syndrome.

We report on a case of short stature with facial dysmorphism, ulnar deviation of hands and joint contractures. The X-rays of hands showed dislocation of the distal end of the radius and growth of the radius over the head of the ulna with total absence of carpal bones and hypoplastic tarsal bones. This is first report of such an association.

Thyroid disorders and bone mineral metabolism.

Thyroid diseases have widespread systemic manifestations including their effect on bone metabolism. On one hand, the effects of thyrotoxicosis including subclinical disease have received wide attention from researchers over the last century as it an important cause of secondary osteoporosis. On the other hand, hypothyroidism has received lesser attention as its effect on bone mineral metabolism is minimal. Therefore, this review will primarily focus on thyrotoxicosis and its impact on bone mineral metabolism.

Gliding Tissue Reconstruction Using a Dorsal Digital Adipofascial Flap in Complex Extensor Injury.

BACKGROUND: Postoperative scar adhesions between tendons and phalanx bones cause persistent disability in complex injuries involving tendons and bones of the hand. Although gliding tissue reconstruction is effective in preventing peritendinous adhesion formation and a dorsal digital adipofascial flap is a reliable method to prevent scar adhesion between tendon and bone after extensor tendon repair, no comparative clinical reports exist. This study aimed to determine the usefulness of a gliding tissue reconstruction method by comparing postoperative range of motion between patients who underwen

Corrective Osteotomies of Phalangeal and Metacarpal Malunions Using Patient-Specific Guides: CT-Based Evaluation of the Reduction Accuracy.

BACKGROUND: Surgical planning of corrective osteotomies is traditionally based on conventional radiographs and clinical findings. In the past 10 years, 3-dimensional (3D) preoperative planning approaches with patient-specific guides have been developed. However, the application of this technology to posttraumatic deformities of the metacarpals and phalangeal bones has not yet been investigated. Our goal was to evaluate the feasibility of the surgical application to the latter and to evaluate the extent and precision of correction. METHODS: We present results of 6 patients (8 osteotomies) trea

Significance of Lateral Pillar in Osteonecrosis of Femoral Head: A Finite Element Analysis.

BACKGROUND: The lateral pillar of the femoral head is an important site for disease development such as osteonecrosis of the femoral head. The femoral head consists of medial, central, and lateral pillars. This study aimed to determine the biomechanical effects of early osteonecrosis in pillars of the femoral head via a finite element (FE) analysis. METHODS: A three-dimensional FE model of the intact hip joint was constructed from the image data of a healthy control. Further, a set of six early osteonecrosis models was developed based on the three-pillar classification. The von Mises stress a
