A case of herpetic whitlow associated with erythema multiforme.

We report a 6-month-old girl with erythema multiforme minor occurring during the course of a herpetic whitlow. Erythema multiforme minor is unusual in infants, and in this population herpes simplex virus has not been reported as a significant etiologic factor. The clinical coexistence of erythema multiforme minor and the herpetic lesion is also atypical. To our knowledge, this is the first report of erythema multiforme minor associated with a concomitant HSV infection in an infant.

Knuckle pads from boxing.

A 21-year-old man developed painful hyperkeratotic, fissured, callosities on his knuckles ("knuckle pads") after having boxed for five evenings a week for six months. Boxer's knuckle pads are one type of traumatic skin injury regularly found in athletes, such as "jogger's toe", "athelete's nodule", "jog-ger's nipple", "runner's rump", "runner's bump", "rower's rump", "tennis toe", "skier 's shin", "surfer's nodule" "black heel", "black palm", "jazz ballet bottom" and "stria distensae". These types of traumatic skin injuries are briefly reviewed here.

Hand eczema herpeticum.

Eczema herpeticum consists of a primary herpes simplex virus infection superimposed on a pre-existing dermatosis. Localized forms of this condition also occur. A twenty-three-year-old woman showed a primary herpes simplex virus infection in the areas of her residual atopic hand eczema after contact with her eighteen-month-old son's oral cavity, which was infected with herpes simplex type I. Possible theories about pathogenesis include a temporary deficit in cell-mediated immunity, defective interleukin production, or persistent alteration of the affected skin. Use of acyclovir resulted in rapi

Ultrastructural signs of altered intracellular metabolism in acral persistent papular mucinosis.

An asymptomatic 24-year-old woman presented with multiple discrete papules on the extensor surfaces of the hands and wrists. Light microscopy revealed focal increase in the amount of dermal fibroblasts as well as deposition of hyaluronidase-labile mucoid substance. The collagen and elastin were decreased. The changes were consistent with acral persistent papular mucinosis (APPM). In electron microscopy, the intercellular glycosaminoglycans showed small ruthenium red-positive granules and thin filaments indicating normal morphology. The fibroblastic cells, however, were conspicuously altered. E

Sheehan's syndrome presenting with type III hyperlipoproteinaemia.

A patient is described in whom the presenting feature of hypopituitarism was the development of palmar xanthomata associated with type III hyperlipoproteinaemia. Treatment of her secondary hypothyroidism with thyroxine caused resolution of the xanthomata and hyperlipidaemia, but the underlying compositional abnormality of the lipoproteins could still be observed.

Treatment of tinea nigra palmaris with miconazole.

A patient with tinea nigra palmaris was successfully treated with 2% miconazole nitrate cream. In vitro studies demonstrated sensitivity of the causative agent, Exophiala werneckii, to this antifungal agent.

Case report. The nonspecific rheumatoid subcutaneous nodule: its presence in fibrositis and scleroderma.

"Rheumatoid" subcutaneous nodules that were at one time considered diagnostic of rheumatoid arthritis or rheumatic fever have also been found in individuals withoug known disease, in patients with systemic lupus erythematosus, and in other conditions. In this report, subcutaneous nodules were described in one patient with fibrositis and in another scleroderma. Multiple pathogenic mechanisms are probably responsible for development of these nodules.
