Hope and Acceptance at the End of Life: Impact of Spirituality on Patients in Palliative Care.

IntroductionPalliative care aims to improve the quality of life of patients with serious illnesses. In this type of care, spirituality is placed alongside other dimensions: physical, psychological, and social. The end of life brings confrontation with dilemmas such as "none of this makes sense" or "was it worth it?" The acceptance or rejection of the journey presents spirituality as a dimension that accompanies the person in their existence. Interventions that allow for a review of life help facilitate reflection on regrets, unresolved issues, and an understanding of the legacy that generates

Ensuring the Safety of Hospitalized Oncology Patients During a Pandemic.

Patients with hematologic malignancies present a unique set of circumstances during the COVID-19 pandemic; they are at increased risk of complications and death from COVID-19 infection, but the treatment of their cancer cannot be delayed. This article highlights some of the practice changes made by an inpatient hematology/oncology advanced practice provider team at a large academic institution in Philadelphia, Pennsylvania, at both a hospital-wide and service level. Practice changes have included restructuring the rounding process, imposing visitor restrictions, adjusting blood transfusion par

Potential health and economic impact of achieving Kenya's overweight and obesity reduction target: a modelling study.

IntroductionKenya has adopted the WHO target of halting the rise of overweight, including obesity, by 2025. This paper assesses the potential impact of achieving the set target on health, healthcare cost and productivity.MethodsWe used a proportional multistate life table model (Kenya Obesity Model) to simulate the 2019 population of Kenya over their lifetime. We compared a scenario in which body mass index (BMI) distributions stabilise in 2025 against one whose BMI distributions stabilise in 2044, and quantified changes in disease-specific health outcomes, healthcare costs and productivity. The predictors of spiritual dryness among Iranian cancer patients during the COVID-19 pandemic.

BackgroundSpiritual struggles affect the wellbeing of religious people. Among them are strugglers with God which is perceived as non-responsive and distant. These perceptions were so far analyzed predominantly in Western societies with a Christian background, but not in Muslims from Iran. The aim of this study was to determine the predictors of spiritual dryness among cancer patients in Iran during the COVID-19 pandemic.MethodsCross-sectional study with standardized questionnaires (i.e., Spiritual Dryness Scale, WHO-5, BMLSS-10, Awe/Gratitude Scale) among 490 cancer patients (mean age 49.50 ±

Management of Acute Myeloid Leukemia: Current Treatment Options and Future Perspectives.

Treatment of acute myeloid leukemia (AML) has improved in recent years and several new therapeutic options have been approved. Most of them include mutation-specific approaches (e.g., gilteritinib for AML patients with activating FLT3 mutations), or are restricted to such defined AML subgroups, such as AML-MRC (AML with myeloid-related changes) or therapy-related AML (CPX-351). With this review, we aim to present a comprehensive overview of current AML therapy according to the evolved spectrum of recently approved treatment strategies. We address several aspects of combined epigenetic therapy

Multiple myeloma, a quintessential malignant disease of aging: a geroscience perspective on pathogenesis and treatment.

Multiple myeloma (MM) is an incurable plasma cell malignancy, which is predominantly a disease of older adults (the median age at diagnosis is 70 years). The slow progression from asymptomatic stages and the late-onset of MM suggest fundamental differences compared to many other hematopoietic system-related malignancies. The concept discussed in this review is that age-related changes at the level of terminally differentiated plasma cells act as the main risk factors for the development of MM. Epigenetic and genetic changes that characterize both MM development and normal aging are highlighted

Pontine Tuberculoma Mimicking an Astrocytic Tumor: A Case Report.

Tuberculomas located in the brainstem are rare entities. This article presents a case study detailing the diagnosis and treatment process of a patient with a pontine tuberculoma, as well as a literature review of tuberculomas located in the pons. A 39-year-old male patient showed progressive quadriparesis and deficits in VII, XI, and XII cranial nerves. MRI image initially suggested a pontine astrocytoma. A second biopsy revealed the presence of acid-fast bacilli. Following six months of anti-tuberculous treatment, the patient showed successful recovery.

Liquid biopsies in myeloid malignancies.

Hematologic malignancies are the most common type of cancer affecting children and young adults, and encompass diseases, such as leukemia, lymphoma, and myeloma, all of which impact blood associated tissues such as the bone marrow, lymphatic system, and blood cells. Clinical diagnostics of these malignancies relies heavily on the use of bone marrow samples, which is painful, debilitating, and not free from risks for leukemia patients. Liquid biopsies are based on minimally invasive assessment of markers in the blood (and other fluids) and have the potential to improve the efficacy of diagnosti

Sexual dysfunction and its impact on quality of life among female haematological cancer patients: a cross-sectional study in Malaysia.

BackgroundS exual dysfunction (SD) and quality of life (QoL) are significantly impacted by cancer and its treatments. However, information on sexual function is scarce, particularly among female patients with underlying haematological malignancies in Asia. This study is aimed at determining the prevalence of SD in these patients, identifying associated factors, and examining their effects on QoL.MethodsThis cross-sectional study, conducted in Malaysia from 2014 to 2016, involved female patients with haematological malignancies who were on active treatment or had completed active treatment. The use of virtual nominal groups in healthcare research: An extended scoping review.

IntroductionThe Nominal Group Technique (NGT) is a consensus group method used to synthesize expert opinions. Given the global shift to virtual meetings, the extent to which researchers leveraged virtual platforms is unclear. This scoping review explores the use of the vNGT in healthcare research during the COVID-19 pandemic.MethodsFollowing the Arksey and O'Malley's framework, eight cross-disciplinary databases were searched (January 2020-July 2022). Research articles that reported all four vNGT stages (idea generation, round robin sharing, clarification, voting) were included. Media Synchron

Physical activity and ageing: The role of physiotherapy in promoting healthy ageing.

BackgroundThe global rise in the older population, especially in sub-Saharan Africa, has heightened the impact of non-communicable diseases (NCDs), responsible for 74% of global deaths and the leading cause for years lived with disability. Physical activity (PA) has proven to manage NCDs; however, 80% of older adults in sub-Saharan Africa engage in low-to-moderate PA levels.ObjectivesThis literature review explores current evidence on the effects of PA on ageing and NCDs in older people in sub-Saharan Africa. Insights gained will enable physiotherapists to refine their PA prescriptions, aligni

Characteristics and Components of Self-Management Interventions for Improving Quality of Life in Cancer Survivors: A Systematic Review.

Self-management can improve clinical and psychosocial outcomes in cancer survivors. Which intervention characteristics and components are beneficial is unclear, hindering implementation into practice. We systematically searched six databases from inception to 17 November 2021 for studies evaluating self-management interventions for adult cancer survivors post-treatment. Independent reviewers screened for eligibility. Data extraction included population and study characteristics, intervention characteristics (TIDieR) and components (PRISMS), (associations with) quality of life (QoL), self-effic

Cancer: a looming public health crisis in Africa-a call to accelerate equitable access to medicines.

The global incidence of cancer is on the rise, with Africa experiencing a particularly steep increase in both cancer incidence and mortality. This upward trend is driven by a combination of factors, including population growth, poverty, aging, urbanization, and the adoption of lifestyles that increase the risk of cancer. It is projected that from 2022 to 2045, the incidence of cancer in Africa will increase by 106.8%, while cancer mortality will rise by 111.7% if no action is taken. In contrast, Europe is expected to see increases of only 22.5% in incidence and 32.2% in mortality. Despite glob

Perceptions and Experiences of Hematopoietic Cell Transplantation Patients During the COVID-19 Pandemic.

ObjectivesSince March 2020, the severe acute respiratory syndrome coronavirus-2 pandemic has affected the global community, but poses unique challenges for individuals with cancer. Patients diagnosed with hematologic malignancies undergo aggressive therapies followed by hematopoietic cell transplantation (HCT) as a potential curative treatment. HCT recipients can be immunocompromised for extended periods of time, and even pre-pandemic, transplant patients reported depression and anxiety due to restrictions and infection prevention measures they had to adhere to as part of transplant precaution

Post-pandemic recommendations for the management of COVID-19 in patients with haematological malignancies or undergoing cellular therapy, from the European Conference on Infections in Leukaemia (ECIL-10).

In the post-pandemic years, SARS-CoV-2 morbidity and mortality declined due to less pathogenic variants, active and passive immunization, and antiviral therapies. However, patients with hematological malignancies and/or undergoing hematopoietic cell transplantation (HCT) remain at increased risk for poor outcomes. Therefore, adherence to contact and droplet precautions is essential to avoid transmission, especially during epidemic waves. Detection of viral RNA by nucleic acid testing of naso-oro-pharyngeal samples is the gold standard for diagnosis due to its high sensitivity and specificity.
