BACKGROUND: Wearing high heel shoes is thought to increase the risk of knee osteoarthritis in females by altering knee joint alignment and increasing muscle activation. However, direct measurements of knee joint space width using ultrasound have been limited. This study aimed to investigate the effects of 8-cm high-heel shoes on knee joint articular space in healthy young females during standing.
METHODS: A cross-sectional study was conducted with 30 healthy females (age 22 ± 1.3 years). Participants' knee medial, lateral and patellofemoral joint space width were measured using an ultrasound …

BACKGROUND: Diabetic foot ulcer (DFU) is a common complication observed in diabetic patients, which can lead to lower limb amputation in severe cases and seriously impair the patient's mobility and even endanger life. Plantar insoles aim to redistribute pressure, yet diabetic foot tissues exhibit altered material properties, necessitating a novel approach to address vertical pressures and shear forces. This study sought to design a three-dimensional anisotropic heel cushioning pad that mitigates both vertical pressure and anteroposterior/mediolateral (AP/ML) shear forces.
METHODS: CT data of …

BACKGROUND: Calcaneal fractures (CF) often result in reduced foot mobility and plantar flexor strength, despite intensive rehabilitation. While treatment mostly focuses on bone restoration, structural and functional adaptation of the ankle plantar flexors during movement is less understood.
RESEARCH QUESTION: To investigate medial gastrocnemius fascicle behavior and ankle biomechanics in patients with CF during bipedal heel-rise.
METHODS: Repetitive heel-rise was analyzed in 18 patients with unilateral CF (BMI: 27.3 ± 3.2 kgm-2, Age: 50 ± 13 years) and 18 healthy controls (BMI: 26.3 ± 3 kgm-…

STUDY DESIGN: Case report.
OBJECTIVES: Pressure ulcers (PUs) affect many spinal cord-injured individuals and can result in serious life-threatening complications including infections and gangrene. Treatment is lengthy and extremely expensive due to recurrent and prolonged hospitalization and multiple surgeries. Consequently, for both medical and economic reasons, intensive efforts in prophylaxis and new effective therapeutic options are mandatory. This report presents the positive effects of pelvic nerve stimulation on the healing of gluteal and heel PUs.
SETTING: Hospital in Zürich/CH.

The aim of the study was to compare preoperative factors and postoperative outcomes in patients with heel ulcerations that primarily had a transtibial (below the knee) amputation (N = 38) versus vertical contour calcanectomy (n = 62). The groups had no statistical difference between their Charlson Comorbidity Index Score, a prognostic score of 10-year survival in patients with multiple comorbidities. The odds of primary closure were 21.1 times higher in patients that underwent below knee amputation compared to patients that underwent vertical contour calcanectomy (OR 21.1 [95% CI 3.89-114.21])…

OBJECTIVE: To investigate the clinical effect of human acellular dermal matrix (HADM) combined with split-thickness skin graft in repairing lacunar soft tissue defects of the lateral heel after calcaneal fracture.
METHODS: From June 2018 to October 2020, providers repaired 11 cases of lacunar soft tissue defects at the lateral part of the heel using HADM combined with split-thickness skin graft. After thorough debridement, the HADM was trimmed and filled into the lacunar defect area. Once the wound was covered, a split-thickness skin graft and negative-pressure wound therapy were applied. OBJECTIVE: Pressure injuries (PIs) result in an extended duration of care and increased risks of complications for patients. When treating a PI, the aim is to hinder further PI development and speed up the healing time. Urgo RID recently developed a new bilayer dressing to improve the healing of stages 2 and 3 heel PIs. This study aims to numerically investigate the efficiency of this new bilayer dressing to reduce strains around the PI site.
METHODS: The researchers designed three finite element models based on the same heel data set to compare the Green-Lagrange compressive and maximal shea…

INTRODUCTION: Plantar heel pain syndrome(PHPS) is a common cause of heel pain. It may worsen a patient's quality of life, and potentially lead to knee, hip or lower back problems. Previous studies have shown that electroacupuncture (EA) and manual acupuncture (MA) are effective treatments for relieving pain in patients with PHPS. However, little evidence supports the use of one intervention over the other.
METHODS AND ANALYSIS: A total of 92 patients diagnosed with PHPS will be recruited and randomly assigned to an EA group or an MA group at a ratio of 1:1. Patients in both groups will receiv…

Extracorporeal membrane oxygenation (ECMO) is life-saving for potentially reversible heart failure and respiratory injuries not responsive to conventional therapies. Technological innovations have produced over the years significant improvements in ECMO devices (pump, cannula design and oxygenator) and have allowed a better risk/benefit profile. Alongside with recognized advantages in the treatment of very sick patients, ECMO remains an invasive procedure for mechanical circulatory support (MCS) and it is associated with complications that strongly influence the prognosis. Current review was d

BACKGROUND: In this randomized multicenter trial, we compared the effect of a lower single dose of 0.5 Gy vs. a standard single dose of 1 Gy concerning pain relief and quality of life, while maintaining a uniform total dose of 6 Gy. On the basis of laboratory observations, the lower single dose would be expected to be more effective.
PATIENTS AND METHODS: A total of 127 patients suffering from painful heel spur were randomized: Patients in the standard group were treated with single fractions of 6 × 1 Gy twice a week, while the experimental group was treated with single fractions of 12 × 0.5 …

Conservative treatments such as in-shoe orthotic heel wedges to treat musculoskeletal injuries are not new. However, weak evidence supporting their use in the management of Achilles tendonitis suggests the mechanism by which these heel wedges works remains poorly understood. It was the aim of this study to test the underlying hypothesis that heel wedges can reduce Achilles tendon load. A musculoskeletal modeling approach was used to quantify changes in lower limb mechanics when walking due to the introduction of 12-mm orthotic heel wedges. Nineteen healthy volunteers walked on an inclinable wa…

There is still conflicting evidence about the effect of high-heeled footwear on posture, especially if methodological confounders are taken into account. The purpose of this study was to investigate the effect of high-heeled footwear on lumbopelvic parameters in experienced younger and middle-aged women while standing and walking. Thirty-seven experienced younger (n=19:18-25 years) and middle-aged (n=18:26-56 years) women were included in this randomized crossover study. Using a non-invasive back shape reconstruction device (rasterstereography), static (pelvic tilt and lumbar lordosis angle) a…

UNLABELLED: The causes of heel cord pain after repair of acute rupture of the Achilles tendon are unclear. The proposed etiologies include nonabsorbable suture granuloma formation, alteration of the pain receptors threshold in the tendon, and distension of the paratenon by the hypertrophied tendon, underlying tendinopathy, postrepair neovascularization, and peritendinous fibrous adhesion. We present an endoscopic technique of adhesiolysis of the Achilles tendon to deal with the various possible causes of postrepair heel cord pain.
LEVELS OF EVIDENCE: Therapeutic, Level 4: Case report.

The events leading to a tissue loss in the lower limbs are varied. The aetiology can be traumatic, infective, neoplastic, iatrogenic, vascular or systemic. Five patients with lesions involving the distal third of the lower limb, the ankle and the heel, traumatic in origin, presented at our service within the Faculty of Plastic and Reconstructive Surgery, University Hospital "La Sapienza" in Rome. In such patients, the surgical approach involved the repair of tissue loss through the use of the neurocutaneous distally based sural artery flap. The use of the reverse flow island sural flap, compar…
