Transforming biomedical uncertainty: the sociohistorical origins of myalgic encephalomyelitis (ME) and chronic fatigue syndrome (CFS).

Myalgic encephalomyelitis (ME) and chronic fatigue syndrome (CFS) are serious and disabling long-term conditions characterised by uncertainty surrounding their aetiology, diagnosis and treatment. People with ME/CFS struggle to be understood, taken seriously and supported with their illness. At least since the 1990s, ME and CFS have been considered by many to be related, overlapping or synonymous with one another. However, the concepts originated from different sociohistorical contexts. This article disentangles the histories of ME and CFS, roots them in the UK and the USA respectively, and com

Changing contact patterns in Newfoundland and Labrador, Canada in response to public health measures during the COVID-19 pandemic.

The provincial government of Newfoundland and Labrador, Canada implemented a contact tracing program as part of a containment strategy during the COVID-19 pandemic. A high proportion of cases were detected and contact traced, and our analysis provides insights into secondary case distributions and contact patterns in Newfoundland and Labrador. We used a heuristic approximation of secondary cases to account for ambiguities in who infected whom. These approximate values provide an empirical distribution of secondary cases. These distributions are compared against the stringency of public health

A Structure-Revealing Tensor Network Paradigm and Its Applications.

Tensor network (TN) decomposition is a powerful data representation tool widely used in machine learning and computer vision. However, existing TN decomposition methods are limited by fixed topologies, which constrain their representational capacity. To overcome this limitation, we introduce, for the first time, an energy-based framework that allows us to reveal a data-adaptive TN structure through the optimization of an energy function. Specifically, we propose a novel structure-revealing TN paradigm, termed SVD-inspired TN decomposition (SVDinsTN). This paradigm enables the formulation of an

The eye's coral reef: toward a planetary-health agenda for ocular-microbiome stewardship.

Coral reefs and the human ocular surface represent ecologically distinct yet structurally comparable microbial ecosystems in which resilience depends on finely regulated host-microbe interactions. In coral reef science, microbial shifts precede visible bleaching and ecosystem collapse, enabling the development of predictive stress indices such as Degree Heating Weeks (DHW). Comparable principles are emerging in host-associated, low-biomass microbiomes, where subtle perturbations may trigger disproportionate functional consequences. Here, we propose a systems-level conceptual framework linking

Optimal control approach to developing sex-structured release strategies for dengue prevention.

Dengue remains a public health challenge due to the persistence of Aedes aegypti populations and the limited effectiveness of medical and vector control interventions. The release of Wolbachia-infected mosquitoes has been used in multiple regions; yet, release protocols are often heuristically structured, with limited optimization. In particular, the impacts of mosquito sex structure and strain-specific effects remain unclear. This study develops a sex-structured dynamical model that links within-vector Wolbachia dynamics, such as cytoplasmic incompatibility and vertical transmission, with bet

When the Story Has Only One Ending: Narrative Foreclosure, Genre, and Suicidal Meaning in Nursing.

Suicide-focused nursing often depends on the language of risk, crisis, symptoms, and safety. These languages are necessary, yet they do not fully explain how a life can become narratively organized around only one plausible ending. This conceptual paper develops narrative foreclosure as a structural account of suicidal meaning and uses genre as a heuristic for examining the architecture of expectation within a person's story-world. Drawing from narrative identity, autobiographical memory, self-continuity, suicidology, relational theories of selfhood, and genre theory, the paper distinguishes f

Mining Stored-Specimen Studies for Information about Cancer Natural History.

The advent of new multicancer early detection tests and publication of early diagnostic results have generated expectations of clinical benefit from multicancer screening. The clinical benefit of a cancer screening test depends critically on disease natural history, which is typically learned from prospective screening studies. Retrospective studies of stored blood specimens are important in learning about a test's preclinical diagnostic performance but have rarely been used to infer natural history. The extent to which these studies might be harnessed to also learn natural history is discusse

Attribute reduction based on classes in incomplete ordered decision systems.

Dominance-based rough set approach (DRSA) has attracted much attention in multi-criteria decision-making problems for data with preference-ordered relations since its inception. However, in the real world, there is often the challenge of incomplete information. This paper addresses approaches to attribute reduction in an incomplete ordered decision system (IODS). Attribute evaluation is crucial in the attribute reduction process. Traditional evaluation functions primarily depend on the relationships between features, neglecting their impact on classification, and incur high computational costs

Trojan horse in obstetric complications: A case report of concurrent uterine perforation, incarcerated fallopian tube and cesarean section scar pregnancy.

RATIONALE: Uterine perforation complicated by incarcerated fallopian tube and cesarean scar pregnancy (CSP) constitutes an exceptionally rare and clinically challenging condition. The diagnostic dilemma arises from elevated β-hCG levels combined with parauterine mass formation, which frequently mimics tubal ectopic pregnancy presentation. This case report aims to highlight the critical importance of differential diagnosis in preventing clinical mismanagement of this complex obstetric emergency. PATIENT CONCERNS: A 35-year-old woman (gravida 3, para 2) with recent Dilation and Curettage (D&C)

Modeling the volume of tissue activated in deep brain stimulation and its clinical influence: a review.

Deep brain stimulation (DBS) is a neuromodulatory therapy that has been FDA approved for the treatment of various disorders, including but not limited to, movement disorders (e.g., Parkinson's disease and essential tremor), epilepsy, and obsessive-compulsive disorder. Computational methods for estimating the volume of tissue activated (VTA), coupled with brain imaging techniques, form the basis of models that are being generated from retrospective clinical studies for predicting DBS patient outcomes. For instance, VTA models are used to generate target-and network-based probabilistic stimulati

Explanatory Journeys: Visualising to Understand and Explain Administrative Justice Paths of Redress.

Administrative justice concerns the relationships between individuals and the state. It includes redress and complaints on decisions of a child's education, social care, licensing, planning, environment, housing and homelessness. However, if someone has a complaint or an issue, it is challenging for people to understand different possible redress paths and explore what path is suitable for their situation. Explanatory visualisation has the potential to display these paths of redress in a clear way, such that people can see, understand and explore their options. The visualisation challenge is f

Predicting the Feasibility of Copper(I)-Catalyzed Alkyne-Azide Cycloaddition Reactions Using a Recurrent Neural Network with a Self-Attention Mechanism.

The copper(I)-catalyzed alkyne-azide cycloaddition (CuAAC) reaction, a major click chemistry reaction, is widely employed in drug discovery and chemical biology. However, the success rate of the CuAAC reaction is not satisfactory as expected, and in order to improve its performance, we developed a recurrent neural network (RNN) model to predict its feasibility. First, we designed and synthesized a structurally diverse library of 700 compounds with the CuAAC reaction to obtain experimental data. Then, using reaction SMILES as input, we generated a bidirectional long-short-term memory with a sel

Temporal and Spatial Variability of Surface Motor Activation Zones in Hemiplegic Patients During Functional Electrical Stimulation Therapy Sessions.

The goal of this study was to investigate surface motor activation zones and their temporal variability using an advanced multi-pad functional electrical stimulation system. With this system motor responses are elicited through concurrent activation of electrode matrix pads collectively termed "virtual electrodes" (VEs) with appropriate stimulation parameters. We observed VEs used to produce selective wrist, finger, and thumb extension movements in 20 therapy sessions of 12 hemiplegic stroke patients. The VEs which produce these three selective movements were created manually on the ergonomic

Oscillations, Timing, Plasticity, and Learning in the Cerebellum.

The highly stereotyped, crystal-like architecture of the cerebellum has long served as a basis for hypotheses with regard to the function(s) that it subserves. Historically, most clinical observations and experimental work have focused on the involvement of the cerebellum in motor control, with particular emphasis on coordination and learning. Two main models have been suggested to account for cerebellar functioning. According to Llinás's theory, the cerebellum acts as a control machine that uses the rhythmic activity of the inferior olive to synchronize Purkinje cell populations for fine-tuni

Learning to rank atlases for multiple-atlas segmentation.

Recently, multiple-atlas segmentation (MAS) has achieved a great success in the medical imaging area. The key assumption is that multiple atlases have greater chances of correctly labeling a target image than a single atlas. However, the problem of atlas selection still remains unexplored. Traditionally, image similarity is used to select a set of atlases. Unfortunately, this heuristic criterion is not necessarily related to the final segmentation performance. To solve this seemingly simple but critical problem, we propose a learning-based atlas selection method to pick up the best atlases tha

Adaptive neuro-fuzzy inference system for acoustic analysis of 4-channel phonocardiograms using empirical mode decomposition.

The heart's mechanical activity can be appraised by auscultation recordings, taken from the 4-Standard Auscultation Areas (4-SAA), one for each cardiac valve, as there are invisible murmurs when a single area is examined. This paper presents an effective approach for cardiac murmur detection based on adaptive neuro-fuzzy inference systems (ANFIS) over acoustic representations derived from Empirical Mode Decomposition (EMD) and Hilbert-Huang Transform (HHT) of 4-channel phonocardiograms (4-PCG). The 4-PCG database belongs to the National University of Colombia. Mel-Frequency Cepstral Coefficien

PRODUCTION OF SOUND BY UNSTEADY THROTTLING OF FLOW INTO A RESONANT CAVITY, WITH APPLICATION TO VOICED SPEECH.

An analysis is made of the sound generated by the time-dependent throttling of a nominally steady stream of air through a small orifice into a flow-through resonant cavity. This is exemplified by the production of voiced speech, where air from the lungs enters the vocal tract through the glottis at a time variable volume flow rate Q(t) controlled by oscillations of the glottis cross-section. Voicing theory has hitherto determined Q from a heuristic, reduced complexity 'Fant' differential equation (G. Fant, Acoustic Theory of Speech Production, 1960). A new self-consistent, integro-differential
