Low revision rate after total hip arthroplasty in patients with pediatric hip diseases.

BACKGROUND: The results of primary total hip arthroplasties (THAs) after pediatric hip diseases such as developmental dysplasia of the hip (DDH), slipped capital femoral epiphysis (SCFE), or Perthes' disease have been reported to be inferior to the results after primary osteoarthritis of the hip (OA). MATERIALS AND METHODS: We compared the survival of primary THAs performed during the period 1995-2009 due to previous DDH, SCFE, Perthes' disease, or primary OA, using merged individual-based data from the Danish, Norwegian, and Swedish arthroplasty registers, called the Nordic Arthroplasty Regi…

Hip stability in primary total hip arthroplasty using an anatomically sized femoral head.

Dislocation after total hip arthroplasty (THA) remains a problem despite many advances in technique and prosthetic design over the 5 decades since the introduction of total joint replacement. This article reports the short-term results (1 year of follow-up) of THA in 235 patients who received a large, anatomically sized femoral head (BFH Technology; Wright Medical Technology, Inc, Arlington, Tennessee) with modular necks for hip stability. The prosthesis allows a 6-mm differential between the size of the acetabular component and femoral head size. Patients also received a Conserve monoblock ac…

Total hip arthroplasty for treatment of fused hip with 90 degrees flexion deformity.

Conversion of a fused hip to total hip arthroplasty is a challenging procedure. The results depend on the degree of the change that occurs in the muscles and soft tissues at the hip from long immobilization, and considerable postoperative rehabilitation is necessary. Complications such as nerve injury, dislocation, infection and heterotopic ossification are common. Most of the authors recommended trochanteric osteotomy for better exposure. We are reporting a case of conversion total hip arthroplasty performed for a hip fused in 90 degrees of flexion. Surgical technique is described in detail a…

Muscular recovery of hip flexors and extensors after two-incision total hip arthroplasty.

BACKGROUND: The influence of two-incision total hip arthroplasty on muscular function has not been reported. We hypothesized that recovery could be different for the hip flexors and extensors because an intermuscular interval was used in the flexors but a trans-muscular approach was used for the extensors. METHODS: Two-incision total hip arthroplasties were performed in 10 patients (8 men and 2 women) with a mean age of 47.2 years (range, 27 to 63 years). Muscle torques were measured prospectively in the 10 patients using a cybex dynamometer preoperatively, 6 weeks postoperatively, and at the…

Sequelae of pediatric hip disorders: survey responses from experts in adult hip reconstruction.

Questions persist concerning the incidence of total hip arthroplasties (THAs) attributable to secondary osteoarthrosis and the impact of corrective pediatric hip surgeries and retained internal fixation on subsequent THAs. Hip reconstruction fellowship directors (N = 72) were mailed a survey of multiple-choice questions about pediatric hip disorders (PHDs) in their THA populations, the influence of hip osteotomies on subsequent THAs, and the recommendation to routinely remove pediatric hip internal fixation. Forty-five surgeons (62.5%) responded. The majority reported that a small proportion o…

Constrained components for the unstable hip following total hip arthroplasty: a literature review.

Patients with chronic instability or late dislocation following total hip arthroplasty often require operative management. Unfortunately, there is an increased risk of recurrent dislocation following revision in these patients. Over the past decade the use of constrained devices for patients with chronic instability has gained increased interest; however, there is a paucity of studies available in the literature regarding the use of these devices. The purpose of this study was to analyze the available literature over the past 15 years, focusing on larger, long-term studies, to obtain recommend…

Impaction grafting for acetabular deficiency in total hip arthroplasty for developmental hip dysplasia.

The pathological anatomy in neglected developmental dysplasia of the hip often demands some sort of reconstructive surgery to augment the acetabular bone stock during total hip arthroplasty. We have reviewed 11 hips in 10 patients with osteoarthritis secondary to developmental hip dysplasia who underwent cemented total hip arthroplasty using impaction grafting of the acetabulum with a morselized femoral head autograft, to bring the socket down to the anatomical hip center. The mean age at the index procedure was 43.4 years (ranging from 29 to 60 years) and the mean follow-up period was 3.8 yea…

[Development of femoral head ossification in hip joint dysplasia and hip joint dislocation].

The occurrence and development of the ossification nucleus of the femoral head on X-ray films were studied in 85 children with hip joint dysplasia/dislocation. Delayed development was found to be related to treatment modalities or the severity of the dysplasia. Delayed development of the ossification nucleus of the femoral head is probably of considerable prognostic importance.

Dislocation after total hip arthroplasty. Treatment with an above-knee hip spica cast.

A study was undertaken of 32 dislocations occurring during a nine-year period in 25 of 1280 postoperative total hip arthroplasty patients. Dislocations occurred in 0.6% of 1030 primary procedures, 1.6% of 165 conversions of hemiarthroplasties to total arthroplasties, and 20% of 85 revisions of previous total hip arthroplasties. Average time to dislocation was 31 days after primary procedures and 106 days after revisions. Acetabular cup malposition and poor patient cooperation appeared to be the causes of dislocation after primary procedures. Trochanteric absence or compromise was a factor in d…

Arthroscopic Hip Remplissage Using a Greater Trochanter Autograft: Preserved Hip at 14 Years.

CASE: A 21-year-old woman with a surgical history of 2 prior right hip surgeries presented with ongoing right hip pain. Radiographic findings demonstrated a labral tear, osteoarthritic changes, and cam over-resection. Arthroscopic iliotibial band autograft labral reconstruction and bony remplissage of the femoral defect using a greater trochanter autograft were performed. CONCLUSION: Arthroscopic bony hip remplissage is a viable treatment option for young patients who present after failed hip arthroscopy with cam over-resection.

Three-Dimensional Evaluation of Hip Center Shift After Periacetabular Osteotomy for Hip Dysplasia.

To quantify the three-dimensional shift of the femoral head after periacetabular osteotomy and to identify the relevant morphological factors. This retrospective study included 26 patients (30 hips) who underwent periacetabular osteotomy at a single institution. Pre- and postoperative supine and standing anteroposterior two-dimensional images and pelvic computed tomography scans were obtained. Using three-dimensional-to-two-dimensional model-image registration techniques, we quantified acetabular and femoral head center shifts in both the supine and standing positions along three axes based on…

Rethinking preservation: The case for timely hip arthroplasty in young adult hip pathology.

Young adults with hip pathology present a therapeutic challenge requiring careful consideration of treatment options that will affect decades of future function. Historically, the orthopedic community has maintained a strong preservation bias, often pursuing multiple preservation attempts before considering arthroplasty because of concerns about implant longevity. This narrative review critically examines current evidence regarding hip preservation surgery and total hip arthroplasty in young adults to inform evidence-based decision making. The literature reveals that successful hip preservatio…

Decreasing Hip Offset in Total Hip Arthroplasty Results in Decreased Physical Function Scores.

BACKGROUND: Acetabular and femoral offset play an important role in total hip arthroplasty (THA) for postoperative stability and biomechanical function. However, it is unknown whether offset impacts patient-reported outcomes (PROs). This study evaluated patients undergoing direct anterior (DA) THA with the hypothesis that patients who have a decrease in hip offset postoperatively would have lower physical function scores and higher pain interference. METHODS: There were 499 patients who underwent DA THA at a single tertiary academic institution who were retrospectively evaluated. Preoperative…

Vitamin D and hip protectors in osteosarcopenia: a combined hip fracture preventing approach.

Osteosarcopenia is an emerging clinical condition highly prevalent in the older people. Affected subjects due to their intrinsic skeletal fragility and propensity to falls are at elevated risk of hip fractures which can increase morbidity and mortality. Strategies for attenuating the impact of predisposing factors on hip fractures are not yet well defined and should derive from multidisciplinary care and collaborations. Our aim was to narratively review available data on the preventive role of vitamin D and hip protectors on hip fractures in older patients with sarcopenia. Older subjects are a…

Hip adduction angle during wider step-width gait affects hip adduction moment impulse.

BACKGROUND: Decreasing an external hip adduction moment (HAM) impulse during stance is important to prevent the progression of hip osteoarthritis. A hip adduction angle (HAA) during walking influences the HAM impulse. Although a wider step-width (WS) gait is a gait modification to decrease a peak HAM, no study has reported the HAM impulse and HAA. RESEARCH QUESTION: We investigated whether the HAA influences the peak HAM and HAM impulse during WS gait. METHODS: Twenty-six healthy young adults walked with normal step-width (NS) and WS comfortably. They were not instructed about hip adduction …
