BACKGROUND: Interdepartmental consultations are essential for managing complex inpatient care but are often inefficient. Hospital-wide, data-driven analyses are needed to guide process improvements; yet, most existing studies have focused on single departments or specific diseases, leaving a gap in understanding hospital-level collaboration networks. Understanding these patterns is crucial for optimizing clinical workflows, reducing delays, and improving patient outcomes in large tertiary hospitals.
OBJECTIVE: This study aimed to analyze the distribution and network characteristics of interde

IMPORTANCE: Clinicians commonly experience moral distress related to potentially nonbeneficial life-sustaining treatments (LST). Hospitals' institutional culture (eg, shared beliefs, values, and practices), structures (eg, policies, practices, resource allocation), and societal-level factors (eg, national culture, local and national policies, medical hierarchies) may contribute to moral distress related to potentially nonbeneficial LST.
OBJECTIVE: To investigate clinicians' perspectives on how hospitals' institutional culture and structures might exacerbate, prevent, or mitigate the influence

BACKGROUND: Patient classification systems (PCSs) are frequently used to estimate hours of nursing care to support budgeting and guide staff … RSING MANAGEMENT: This PCS provides a scientific basis for nurse staffing decisions and may support hospital administrators and health authorities in the development of data-driven nurse workforce allocation policies.

OBJECTIVES: This study aims to evaluate the association between prehospital intravenous therapy and clinical outcomes in the patients with acute heart failure.
METHODS: We conducted a prospective, multicenter observational study of consecutive AHF patients. Univariate and logistic regression analysis were performed to determine association between prehospital furosemide or nitrates administration and hospital outcome (death, length of stay).
RESULTS: Data on a total of 1239 patients were processed. The mean age in the whole cohort was 71 ± 11.8 years with a gender distribution (M/F) of 634/6

INTRODUCTION: While moderate to severely elevated blood pressure (BP) is present in nearly half of all emergency department (ED) patients, the incidence of true hypertensive emergencies in ED patients is low. Administration of bolus intravenous (IV) antihypertensive treatment to lower BP in patients without a true hypertensive emergency is a wasteful practice that is discouraged by hypertension experts; however, anecdotal evidence suggests this occurs with relatively high frequency. Accordingly, we sought to assess the frequency of inappropriate IV antihypertensive treatment in ED patients wit

In California, one hospital decided the time was right to inoculate staff volunteers with the smallpox vaccine. Weighing the same pros and cons--civic responsibility, health risk, cost and reluctance of staff--other hospitals opted out of inoculations, at least for now. What led these organizations to such divergent decisions?

INTRODUCTION: The increasing number of patients receiving systemic anticancer therapies has placed growing pressure on oncology day hospitals and may negatively impact patients' quality of life due to frequent hospital visits. Home administration of selected antineoplastic treatments has emerged as a potential strategy to reduce hospital burden while maintaining treatment safety. This study aimed to evaluate the safety and feasibility of a nurse-led, home-based program for the administration of systemic anticancer therapies.
MATERIAL AND METHODS: We conducted a prospective observational study

INTRODUCTION: The need for alternative analgesia to opioids in the emergency department (ED) has become increasingly urgent due to the opioid epidemic. Ketamine, traditionally used for sedation, has emerged as a promising alternative for pain control in the acute care setting. This scoping review evaluates the use of intranasal (IN) ketamine compared with intravenous (IV) opioids for analgesic efficacy in treating acute pain in the ED.
METHODS: A search of PubMed, CINAHL and PRIMO databases was conducted for randomized controlled trials (RCTs) published between 2015 and 2025. A total of 561 r

Defined daily dose (DDD) units were used to analyze the antibiotic utilization in a University Military Hospital for a one-year period. The data were collected from hospital records. The mostly prescribed drug groups were penicillins (broad-spectrum), systemic sulfonamides and combinations and tetracyclines. Amoxycillin, doxycycline and co-trimoxazole were most frequently used. Circannual variations were observed only in the utilization of amoxycillin and doxycycline during the year.

This empirical and qualitative study focuses on COVID-19 crisis management in a French hospital, analyzing it from the perspective of organizational resilience to understand its evolution over time. The study identifies adaptation factors during pandemic management, supported by success factors, and it also identifies difficulty factors associated with resilience. These factors are analyzed at the different waves of the pandemic to understand the evolution of organizational resilience across various crisis management temporalities. The results highlight how certain factors initially considered

Hospitals around the country have stepped up their efforts to train staff and implement procedures to ensure the safe identification and management of any patients with signs of Ebola virus disease (EVD). Ronald Reagan UCLA Medical Center in Los Angeles, CA, held an "Ebola preparedness exercise" to give staff an opportunity to walk through the hospital's protocol for handling a simulated patient with EVD. The University of Alabama at Birmingham (UAB) Medical Center has held similar exercises, and is now holding twice-weekly meetings of its leadership team to make sure that all new developments

Sabotage of MCBIS by hospital staff endangers and disrupts the delivery of inpatient health care. Wide-spread interference, along with other implementation problems, cripples the ability of MCBIS to function properly and affects health care costs and quality of services.

BACKGROUND: Parkinson's disease patients are more likely to be hospitalized, have higher rates of hospital complications, and have an increased risk of deterioration during hospitalization. Length of stay is an important underlying factor for these increased risks. We aimed to investigate potential medication errors that may occur during hospitalization and its impact on length of hospital stay.
METHODS: A cross-sectional chart review of 339 consecutive hospital encounters from 212 PD subjects was performed. Medication errors were defined as wrong timing or omission of administration for dopa

BACKGROUND: Interhospital transfers account for 1.5 million inpatient admissions annually. Yet post-transfer patient outcomes and resource utilization vary significantly. Visualizing these data can help health systems optimize transfer practices. The objective of this project was to develop a novel Interhospital Medical ICU (MICU) Transfer Dashboard displaying data on incoming medical MICU transfers to a tertiary health system and create a blueprint for other hospitals to follow.
METHODS: After establishing a multidisciplinary group of physicians, nurses, administrators, data analysts, and in

AIM: To explore the hidden emotional burdens faced by nurses in acute care, focusing on experiences with 'unpopular' patients, compassion fatigue, and the second victim phenomenon, and how these intersect to influence patient safety.
DESIGN: A qualitative study using a narrative inquiry approach.
METHODS: Semi-structured, in-depth interviews were conducted between September and November 2025 with 24 nurses from hospital departments. Data were analysed using narrative and conventional content analysis within a hermeneutic framework.
RESULTS: Content analysis identified emotional solitude in

BACKGROUND: Same-day HIV preexposure prophylaxis (PrEP) implementation remains challenging, especially in high-volume settings such as emergency departments (EDs). METHODS: In 2024-2025, we conducted a cross-sectional qualitative study involving key informant interviews (N = 20) with physicians, nurses, pharmacists, HIV counselors, and administrators at Kenyatta National Hospital in Na
